I previously reported a safety issue re. Brakes not working in park. Vehicle rolled back and car almost hit mom and young daughter walking behind vehicle in parking lot. When I brought it to BellFord in [XXX] they told me it was not covered under the recall since it wasn’t being caused by rear axle. Instead they wanted to charge me $3,000 to diagnose their manufacturing defect. In addition when I notified Ford rep who never said anything other then I must get car diagnosed and pay for it and then “we will see .” Ford complaint XXX and I believe this is the NHTSA Complaint # XXX. Here is my initial Ford complaint Cxh-XXX was closed without explanation no one could tell me why no one had contacted me. I am very concerned that Ford is not taking responsibility for defective cars. My car now has only 47k. Brake issue. I had to replace front boots due to leaking and front axle was leaking. I had that fixed and now rear back up camera isn’t working. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am writing to formally report a series of catastrophic and life-threatening mechanical and electronic failures in my 2025 Ford Explorer Active (VIN: [XXX] ]), purchased in Bogotá, Colombia. Despite immediately notifying the Ford Post-Sales Support Line after the first incident, the technical response was insufficient, leading to a second, more severe event that nearly resulted in a fatal high-speed collision on February 9, 2026. Incidents Summary: 1. Total Power Loss: Engine and all electrical systems shut down while in motion (40 km/h), losing steering and braking assistance. 2. Brake System Failure: At 70 km/h, The vehicle presented a multi-system failure of sensors, audible alarm and illumination of warning lights (ABS, Traction Control, Pre-collision Assistant, hill start assist not available, check brake system, BRAKE, ESC check, driving modes not available). the vehicle experienced a total loss of brake pedal response followed by a sudden, uncontrolled autonomous "hard braking" event. This happened on a high-traffic highway (Autopista Norte). Global Safety Concern: I am formally requesting a Global Technical Investigation into this unit and the 2025 Explorer Active series. The failure of the electronic brake booster and the total system shutdown represent a "Product Liability" risk that could lead to mass-casualty events. I demand: • Direct intervention from Ford Global to Ford Colombia. • A formal safety alert or recall investigation for similar units. THIS INVESTIGATION REVEALED THAT CONSOLE FIRES IN THE SUBJECT VEHICLES MOST OFTEN OCCURRED WHILE VEHICLES EQUIPPED WITH THE LAMP OUTAGE MODULE (LOM), WHICH IS PART OF THE MESSAGE CENTER, WERE TOWING A TRAILER AND WHICH HAD BEEN MODIFIED BY ADDING AN AFTERMARKET TRAILER WIRING HARNESS INTO THE VEHICLE'S TAILLIGHT CIRCUITS.THE OWNER'S MANUAL STATES NEVER AND MORE THAN OTHER TRAILER LIGHT BULB TO ONE CIRCUIT HOWEVER TRAILERS FREQUENTLY CONTAIN SEVERAL BULBS.THE RESULTING EXCESS CURRENT PASSING THROUGH THE LOM COULD CAUSE IT TO OVERHEAT RESULTING IN A FIRE.PROPER AFTERMARKET INSTALLATION OF TRAILER WIRING REQUIRES THE USE OF A BYPASS MODULE, WHICH OBTAINS POWER FOR THE TRAILER LIGHTS DIRECTLY FROM THE BATTERY RATHER THAN FROM THE VEHICLE'S TAIL LIGHT AND STOP LAMP CIRCUITS, OR THE USE OF THE MANUFACTURER'S WIRING HARNESS.IN A LETTER DATED SEPTEMBER 2, 1997, FORD NOTIFIED ODI THAT IT INTENDS TO TAKE ACTION TO PRECLUDE FURTHER INCIDENTS INCLUDING ISSUING A TECHNICAL SERVICE BULLETIN TO ITS DEALERS, NOTIFYING TRAILER RENTAL ASSOCIATIONS AND AGENCIES WHERE KNOWN, AND ADVISING OWNERS BY LETTER OF THE PROPER METHOD FOR CONNECTION OF TRAILER TOW WIRING ON THE SUBJECT VEHICLES.THERE WAS NO DEFECT TREND IN THE SUBJECT VEHICLES IDENTIFIED AS A RESULT OF THIS INVESTIGATION, AND FURTHER EXPENDITURE OF AGENCY RESOURCES IS NOT WARRANTED TO CONTINUE THE INVESTIGATION.

THE CONTACT ASSEMBLY FOR THE REMOTE POWER MIRROR CONTROL SWITCH MAY DEVELOP A SHORT CIRCUIT BETWEEN THE SWITCH POSITIVE PAD AND CHASSIS GROUND PAD AS A RESULT OF WATER INTRUSION.EXCESS CURRENT FLOW RESULTING FROM THE SHORT CIRCUIT CONDITION MAY OVERHEAT THE REMOTE POWER MIRROR CONTROL SWITCH CIRCUIT BOARD AND CAUSE A FIRE IN THE DRIVER'S DOOR.FORD HAS INDICATED THAT CURRENT IN EXCESS OF 15 AMPS CAN OVERHEAT THE CIRCUIT BOARD ENOUGH TO CAUSE A FIRE.CIRCUIT PROTECTION IS PROVIDED BY A 30 AMP FUSE.FORD IS RECALLING ALL 1993-94 4-DOOR EXPLORER VEHICLES EQUIPPED WITH THE SUBJECT ELECTRONIC OPTION COMBINATION TO INSTALL AN IN-LINE 10 AMP FUSE IN THE POSITIVE BATTERY FEED TO THE REMOTE POWER MIRROR CONTROL SWITCH ASSEMBLY.NO 2-DOOR EXPLORER VEHICLES WERE BUILT WITH THE SUBJECT COMPONENTS.ACCORDING TO FORD, VEHICLES BUILT PRIOR TO THE RECALL PRODUCTION RANGE WERE EQUIPPED WITH LOWER CAPACITY CIRCUIT BOARDS FOR THE SUBJECT SWITCH ASSEMBLY THAT WOULD FAIL FROM AN OPEN CIRCUIT IF A SIMILAR SHORT OCCURRED.A NEW SWITCH ASSEMBLY WITH 10 AMP FUSE PROTECTION WAS INTRODUCED BEGINNING WITH THE 1995 MODEL YEAR EXPLORER.MAZDA WILL CONDUCT A PARALLEL COMPAIGN (97V-083.002) TO REMEDY THE SUBJECT CONDITION IN APPROXIMATELY 14,000 NAVAJO VEHICLES.THE NAVAJO VEHICLES WERE AVAILABLE IN ONLY THE 2-DOOR VERSION WHICH, UNLIKE THE EXPLORER VEHICLES, DID USE THE SUBJECT OPTION COMBINATION IN THAT BODY STYLE.
