I own a 2020 Ford Explorer XLT AWD, VIN 1FMSK8DH6LGA35229. While driving, my vehicle displayed “4WD Fault – Service Required” along with the powertrain wrench warning. I then became unable to select/change drive modes. Pre-Collision Assist became unavailable, I experienced Lane Keeping/Lane Assist problems, and additional stability/traction warnings appeared. A professional diagnostic scan found AWD codes C0631:7E-68 and C0631:7F-6C related to the front axle disconnect actuator. The ABS module also stored U0102 (Lost Communication With Transfer Case Control Module) and U0155 (Lost Communication With Instrument Panel Cluster). My mechanic diagnosed failure of the front axle disconnect actuator/intermediate-shaft system. The repair requires removal of the original disconnect system and installation of Ford’s revised intermediate shaft/axle components, replacement of the vent/breather hose, and AWD programming. I was quoted approximately $1,337. The mechanic will then determine whether the transfer case/AWD control module also requires replacement, which would cost approximately $828 additional. Ford has issued TSB 25-2373, superseding TSB 23-2174, for certain 2020-2021 Explorer AWD vehicles. The bulletin specifically identifies C0631 and/or C00A6 and the front axle disconnect actuator/intermediate shaft and directs replacement with a revised intermediate shaft. I am concerned this is a safety defect because the failure resulted in AWD/drive-mode malfunction while multiple driver-assistance systems became unavailable. I request that NHTSA investigate this known failure and determine whether a safety recall is warranted.

The 10R60 transmission is the subject of multiple Ford Technical Service Bulletins and an active Ford/NHTSA safety recall (26S48 / NHTSA 26V-402) addressing transmission-related park-system and rollaway defects on this same transmission, model, and year. My vehicle was well maintained with routine service, yet the transmission failed internally at approximately 87,000 miles. I purchased my 2020 Ford Explorer in December 2020. The first incident occurred around June 2021 while I was on a road trip. While driving, the vehicle unexpectedly shifted/downshifted on its own and caused a sudden change in the vehicle's movement. At first, I thought there might be an issue with the rotary gear selector. The problem occurred again, and in 2023 I took the vehicle to a Ford dealership and reported the issue. I was told there was a recall and recall-related work was performed. However, the unexpected shifting continued afterward. I contacted the Ford dealership multiple times about the continuing problem but was told that without an applicable recall I could be charged for diagnosis and that the problem might not be detected unless it occurred during a test drive. I reported the shifting problem again when the vehicle was at a Ford dealership for recall service in 2025. I specifically told the service advisor that the vehicle was still unexpectedly shifting while driving. The advisor told me the vehicle was driven but the problem did not occur during their test drive. The problem has continued. During a recent incident, the vehicle unexpectedly shifted/downshifted while I was driving and I nearly had an accident with my children in the vehicle. I am concerned that the sudden change in the vehicle's movement could cause me to lose control or be struck by another vehicle. I recently contacted the Ford dealership again and was informed that my vehicle has another open recall related to the transmission/park system. I was given an appointment for October 23, 2026, but was told Ford does not expect the recall remedy to be available until November. I am concerned about continuing to drive the vehicle while waiting for a remedy because this malfunction has occurred repeatedly while the vehicle is in motion and the most recent incident nearly resulted in an accident. The transmission is sporadically shifting causing the vehicle to lose or stop during acceleration and deceleration making it unsafe to pull away from a stop or during deceleration and causing it to stop in the in the rd and then it would shift extremely hard after it would gain power to the transmission again. The vehicle is available for inspection and no codes indicated a transmission issue. Only codes are vacuum leak codes. I've had my snap on scanner attached and have had it at 3 different garages. The vehicle had transmission issues repaired shortly after purchasing at a certified Ford dealer (Fred Beans) of Mechanicsburg. My family and I have been in quite a few near miss accidents stemming from this issue. I've had fluids checked multiple times for the transmission and even had a Ford tech do a complete transmission flush.

My 2020 Ford Explorer experienced a catastrophic mechanical failure directly tied to active Safety Recall 26S48 / NHTSA Campaign 26V402000. While operating the vehicle, the transmission's internal parking pawl failed/shattered, completely locking the rear wheels mechanism. The vehicle is entirely immobilized and cannot move even an inch, even when the gear selector is manually turned to Neutral or when the Manual Park Release (MPR) tether is fully engaged. [1, 2] The vehicle had to be flatbed-towed using specialized wheel dollies to prevent severe structural and drivetrain destruction because the wheels are locked solid. When attempting to have the vehicle received at an authorized Ford dealership, the dealership refused to accept the vehicle or open a formal repair ticket, claiming that because the final recall remedy parts are delayed until 2027, they "cannot take the vehicle." [1] Ford Motor Company is failing to provide a timely safety remedy or manage its dealership network to accommodate a vehicle that has already suffered a total mechanical failure due to this exact recalled defect. The vehicle is a severe rollaway and structural hazard, completely undriveable, and Ford is denying responsibility or alternative transportation assistance because the parts do not yet exist. I am requesting immediate federal intervention to compel Ford to ground this vehicle, provide a safe loaner, and initiate a manufacturer buyback.

Ford Motor Company (Ford) is recalling certain 2020-2027 Aviator and 2020-2026 Explorer vehicles. The switch for the easy-entry second-row outer seats may bind or stick, resulting in the seats unlatching, tipping, or sliding unexpectedly.
Consequence & remedy
Consequence: A seat that moves unexpectedly may not properly restrain an occupant during a crash, increasing the risk of injury.
Remedy: Dealers will inspect and replace the bezel around the switch as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters explaining the safety risk were mailed August 19, 2026. Consequence: A loose seat belt or seat belt buckle may not properly restrain an occupant during a crash, increasing the risk of injury.
Remedy: Dealers will inspect seat belt buckle anchor bolts in all seating positions. Additionally, dealers will inspect the seat belt retractor anchor bolt and seat belt anchor bolt at the second-row center seating position, if equipped. Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles equipped with the Police Interceptor Utility Package. In the event of an engine failure, engine oil and fuel vapor may be released into the engine compartment and accumulate near ignition sources such as hot engine or exhaust components, possibly resulting in an engine compartment fire.
Consequence & remedy
Consequence: An engine compartment fire increases the risk of injury.
Remedy: Owners are advised to park and shut off the engine as promptly as possible upon hearing unexpected engine noises, or after experiencing an unexpected engine power reduction, or seeing smoke from the engine compartment. Dealers will update the Powertrain Control Module (PCM) software. In addition, dealers will inspect for connecting rod bearing failure and if necessary, replace the engine long block. The Office of Defects Investigation (ODI) received a petition on August 22, 2025, requesting a defect investigation into an alleged defect of the “underbody shields” on model year (MY) 2020 Ford Explorer vehicles. The petition alleges premature degradation and detachment of the plastic underbody shields, potentially leading to road debris. The petition itself can be reviewed at NHTSA.gov under ODI number 11682326. A search of the subject vehicle population was undertaken using complaints submitted to ODI. No related complaints were found in NHTSA’s database for the subject vehicle population. Subsequently, ODI conducted a broader search for related complaints and field reports involving MY 2020-2025 Ford Explorer and Lincoln Aviator models, as these vehicles utilize substantially similar underbody shields. No complaints or reports were found citing any allegations pertaining to any of the underbody shields. ODI also reviewed manufacturer information and data and did not identify a trend related to the Petitioner’s allegations. Based on the absence of consumer complaints and reports, ODI has not identified a material risk of a complete detachment or a projectile hazard related to this condition. The petitioner's description of detectable symptoms - including noise and vibration that progressively worsen and visible sagging of the shields underneath the vehicle - indicates a failure mode that provides drivers with ample notice to seek service before a potential detachment. In view of the insufficient evidence of a vehicle-based defect pertaining to the underbody shields of the subject vehicle, ODI is denying this Defect Petition. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. Ford stated that the Explorer powerpack variants not covered under Recalled 23V-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque. According to Ford, this is also evidenced by the lower current and projected failure rates. In view of the recall action being taken by Ford, ODI is closing this RQ. NHTSA will continue to monitor complaints from the customer satisfaction program vehicles and reopen this issue, if necessary. On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs.
