The contact owns a 2020 Ford Explorer. The contact stated that while driving at an undisclosed speed on a six-hour trip, the low tire message was displayed, and the tire quickly deflated. The contact slowed down and pulled into a service center and inflated the tire. The vehicle was then driven without failure. While pulling into a parking lot, the driver's side rear wheel detached from the vehicle. The previous tire lug nut had been factored out for installation at a tire center. The towing company became aware that all five bolts had sheared off, and some of the bolts were in the parking lot. The vehicle was inspected by the local dealer, and it was determined that the rear passenger’s side wheel was missing lug nuts as well. The contact stated that three of the five lug nuts had sheared off. The failure recurred. The manufacturer was not contacted. The failure mileage was approximately 57,065.

I keep feeling a clunk or thump towards the back of the car at random times, towards the back axle. Sometimes it's when i stop and drive forward, other times it happens while I am just driving normally, usually at slow speeds around 5-45mph. I have an extended warranty and the ford dealer has said they can;t replicate it and want to give my car back until it gets worse, and then try fix it. I am highly concerned about this process....

Wheels / Tire Pressure Monitoring System The tire pressure monitoring system (TPMS) on my 2020 Ford Explorer intermittently fails to detect the tire pressure in one or more tires. The system will show missing pressure readings or fail to display a reading for a tire. The vehicle may operate without accurate tire pressure information being displayed to the driver. This creates a potential safety risk because the driver may not be alerted to a low tire pressure condition. The vehicle had approximately 56,000 miles when this began, and the issue appeared without prior warning. The vehicle is scheduled for inspection by a Ford dealership.

The tire pressure monitoring system has failed. Per the dealership, each tire needs a new battery, which run around $150/each. The first sensor went out approx 1 month ago, with the other 3 going out shortly after. While this does not pose an immediate risk for the driver, it does appear that numerous other 2020 explorers are having the same issue. If the batteries provided are faulty, a recall should be issued.

I am filing a complaint regarding a safety issue with my 2020 Ford Explorer that was not properly addressed during a recall repair, leading to a dangerous situation. Vehicle Details Make: Ford Model: Explorer Year: 2020 Recall Repair Details In January 2024, I took my vehicle to Coconut Point Ford in Estero, FL, for a recall repair related to the rear axle horizontal mounting bolt, which was at risk of fracturing. The recall required the replacement of the subframe bushing and rear axle bolt to prevent driveshaft disconnection. Incident Description Recently, while driving under normal conditions, the bolt fractured, causing the differential to nearly detach from the vehicle. This failure could have resulted in a catastrophic accident. I am now facing a $6,000 repair bill for an issue that should have been resolved under the recall. Safety Risk The failure of the bolt and the near detachment of the differential posed a severe safety risk, as it could have led to a loss of control of the vehicle, especially at highway speeds. Dealership Response When I brought this to the attention of Coconut Point Ford, they stated that "bolts are not covered under the extended warranty." However, since this is a recall-related issue, I believe Ford is obligated to cover the repair costs. The service writer, Steve, has been helpful, but the resolution has been delayed. Additional Context I am aware of class action lawsuits against Ford regarding this defect, which allege that the recall fix does not fully address the issue and that the design is inherently flawed. Request I request that the NHTSA investigate this matter to ensure that Ford is held accountable for properly addressing this safety recall and to prevent similar incidents from occurring to other vehicle owners. On Thursday, [XXX], while driving in a snowstorm with my two children, my 2020 Ford Explorer AWD system failed completely, leaving us stranded on an icy hill with no traction or ability to move. • My vehicle lost all AWD functionality, causing it to become immobilized on an active roadway in hazardous conditions. • We were trapped between two semi-trucks that were also stuck, creating a high-risk situation where a collision or multi-vehicle pileup could have occurred. • With freezing temperatures and no way to move, we were at risk of hypothermia while waiting for a tow truck. This was not an isolated issue. Ford acknowledges this AWD system defect under TSB 23-2174, meaning this failure can happen to any driver under similar conditions, creating a major safety hazard. 3. Yes. The issue was diagnosed and confirmed by Zeigler Lincoln Ford, who determined it was covered under TSB 23-2174. • Ford has officially acknowledged this defect but has not issued a recall. 4. Yes. The vehicle was inspected by Zeigler Lincoln Ford, and the AWD failure was confirmed as a front axle disconnect actuator/intermediate shaft defect. • Ford issued TSB 23-2174 to address the defect, proving that this is a known issue. • However, instead of issuing a recall, Ford is making owners pay for repairs out of pocket, despite the serious safety risk. 5. Yes. Prior to the AWD failure, I received a wrench icon warning (Powertrain Malfunction Indicator) on my dashboard. • Error codes C0631 and C0633 were later retrieved by the dealership, confirming AWD disconnect failure. • I had experienced intermittent AWD traction loss in winter conditions, but the system failed completely during the snowstorm incident. Consequence & remedy

Consequence: A seat that moves unexpectedly may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and replace the bezel around the switch as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters explaining the safety risk were mailed August 19, 2026. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence & remedy

Consequence: An electrical short circuit can increase the risk of a fire.

Remedy: Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Owners will also have a alternative option to replace engine block heater element with a threaded blanking plug, and remove the block heater electrical cord. Consequence & remedy

Consequence: A disconnected driveshaft can result in vehicle rollaway when the vehicle is placed in park without the parking brake applied. A disconnected driveshaft can also result in a loss of drive power. Either of these scenarios can increase the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2021 Explorer and Aviator vehicles. The seat belt buckle anchor bolts at one or more seating positions may be improperly secured. Additionally, vehicles may have an improperly secured seat belt retractor anchor bolt and/or seat belt anchor bolt at the second-row center seating position, if equipped.

Consequence & remedy

Consequence: A loose seat belt or seat belt buckle may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect seat belt buckle anchor bolts in all seating positions. Additionally, dealers will inspect the seat belt retractor anchor bolt and seat belt anchor bolt at the second-row center seating position, if equipped. If loose anchor bolts are found, the affected seat components will be replaced. Repairs will be performed free of charge. Owner notification letters were mailed on June 5, 2025. Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles equipped with the Police Interceptor Utility Package. In the event of an engine failure, engine oil and fuel vapor may be released into the engine compartment and accumulate near ignition sources such as hot engine or exhaust components, possibly resulting in an engine compartment fire.

Consequence & remedy

Consequence: An engine compartment fire increases the risk of injury.

Remedy: Owners are advised to park and shut off the engine as promptly as possible upon hearing unexpected engine noises, or after experiencing an unexpected engine power reduction, or seeing smoke from the engine compartment. Dealers will update the Powertrain Control Module (PCM) software. In addition, dealers will inspect for connecting rod bearing failure and if necessary, replace the engine long block. Consequence: A disconnected driveshaft can result in a loss of drive power or a vehicle rollaway if the parking brake is not applied. Either of these scenarios can increase the risk of a crash.

Remedy: Dealers will replace the subframe bushing and rear axle bolt. They will also inspect the rear axle cover for damage near the bolt hole location and replace with a new cover if any damage is found. Repairs will be performed free of charge. Ford Motor Company (Ford) is recalling certain 2020-2022 Lincoln Aviator and Ford Explorer vehicles equipped with 3.0L PHEV engines. A manufacturing defect in one or more of the high voltage battery cells may result in an internal short circuit and battery failure.

Consequence & remedy

Consequence: Battery failure can cause a loss of drive power, increasing the risk of a crash. Battery failure can also increase the risk of a fire.

Remedy: Dealers will perform a Battery Energy Control Module (BECM) diagnostic test and replace the high voltage battery pack, as necessary, free of charge. Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer Police Interceptor Utility vehicles equipped with a 12.1-inch Integrated Computer Screen (ICS). When the Display On/Off Button is used to turn the display to "Off" prior to placing the vehicle in reverse, the backup camera rearview image will not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury.

The Office of Defects Investigation (ODI) received a petition on August 22, 2025, requesting a defect investigation into an alleged defect of the “underbody shields” on model year (MY) 2020 Ford Explorer vehicles. The petition alleges premature degradation and detachment of the plastic underbody shields, potentially leading to road debris. The petition itself can be reviewed at NHTSA.gov under ODI number 11682326. A search of the subject vehicle population was undertaken using complaints submitted to ODI. No related complaints were found in NHTSA’s database for the subject vehicle population. Subsequently, ODI conducted a broader search for related complaints and field reports involving MY 2020-2025 Ford Explorer and Lincoln Aviator models, as these vehicles utilize substantially similar underbody shields. No complaints or reports were found citing any allegations pertaining to any of the underbody shields. ODI also reviewed manufacturer information and data and did not identify a trend related to the Petitioner’s allegations. Based on the absence of consumer complaints and reports, ODI has not identified a material risk of a complete detachment or a projectile hazard related to this condition. The petitioner's description of detectable symptoms - including noise and vibration that progressively worsen and visible sagging of the shields underneath the vehicle - indicates a failure mode that provides drivers with ample notice to seek service before a potential detachment. In view of the insufficient evidence of a vehicle-based defect pertaining to the underbody shields of the subject vehicle, ODI is denying this Defect Petition. On July 17, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (MY) 2020-2022 Ford Explorer vehicles (subject vehicles). The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. Ford stated that the Explorer powerpack variants not covered under Recalled 23V-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque.  According to Ford, this is also evidenced by the lower current and projected failure rates. In view of the recall action being taken by Ford, ODI is closing this RQ. NHTSA will continue to monitor complaints from the customer satisfaction program vehicles and reopen this issue, if necessary. On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs.
