I own a 2020 Ford Explorer XLT AWD, VIN 1FMSK8DH6LGA35229. While driving, my vehicle displayed “4WD Fault – Service Required” along with the powertrain wrench warning. I then became unable to select/change drive modes. Pre-Collision Assist became unavailable, I experienced Lane Keeping/Lane Assist problems, and additional stability/traction warnings appeared. A professional diagnostic scan found AWD codes C0631:7E-68 and C0631:7F-6C related to the front axle disconnect actuator. The ABS module also stored U0102 (Lost Communication With Transfer Case Control Module) and U0155 (Lost Communication With Instrument Panel Cluster). My mechanic diagnosed failure of the front axle disconnect actuator/intermediate-shaft system. The repair requires removal of the original disconnect system and installation of Ford’s revised intermediate shaft/axle components, replacement of the vent/breather hose, and AWD programming. I was quoted approximately $1,337. The mechanic will then determine whether the transfer case/AWD control module also requires replacement, which would cost approximately $828 additional. Ford has issued TSB 25-2373, superseding TSB 23-2174, for certain 2020-2021 Explorer AWD vehicles. The bulletin specifically identifies C0631 and/or C00A6 and the front axle disconnect actuator/intermediate shaft and directs replacement with a revised intermediate shaft. I am concerned this is a safety defect because the failure resulted in AWD/drive-mode malfunction while multiple driver-assistance systems became unavailable. I request that NHTSA investigate this known failure and determine whether a safety recall is warranted.

I am writing to formally express serious concerns regarding the safety of a Ford vehicle that continues to carry a 5-star safety rating despite multiple known, safety-critical defects that emerged after relatively low mileage. Two major issues developed between approximately 62,000 and 72,000 miles. Both are documented by Ford Technical Service Bulletins (TSBs), yet neither has been addressed through a formal recall. My vehicle’s warranty expired at 60,000 miles, leaving me responsible for more than $3,000 in out-of-pocket repair costs. Issue 1 – Front axle seal leak (TSB 23-2174) The front axle seal began leaking fluid. According to the TSB, the original affordable seal is no longer available, requiring replacement of the entire axle assemblies at a cost of approximately $2,000. Failure to complete this repair risks axle lock-up and a potential collision. Issue 2 – All-wheel-drive module ground wire omission (TSB 23-2061) Ford failed to install a required ground wire on the AWD module. This caused multiple warning lights to illuminate, including powertrain failure and collision-assist system failure. I refused to allow my wife and children to ride in the vehicle throughout the winter because of these critical warnings. The repair, which required replacement of the AWD module, cost more than $1,000. These are not minor inconveniences. An axle that can lock up, loss of AWD capability in hazardous conditions, and the simultaneous failure of collision-assist systems present clear and foreseeable risks of serious crashes. A vehicle exhibiting these defects does not merit a 5-star safety rating, particularly when the problems are known to the manufacturer and affect used-vehicle buyers who may lack the financial means to perform the repairs. I respectfully urge the NTSB to investigate these issues, determine whether the continued 5-star rating is appropriate, and encourage Ford to issue formal recalls so that owners are not left to bear the cost and risk of repairs.

Had recall for parking brake fixed, soon after I’d be on interstate or intersection and it would lose power wrench and check power train system light came on. Took to dealership and they said it was parking pawl sylenuod. Had repaired left the shop didn’t even get home and it did the same: Mechanic said it was gas pedal sensor repaired that and same thing happened it may go real fast or not go . I worked for uber and lyft and have been out of work for a few months. Car setting at dealership and the say I need to pay for a new power train module because my cars lost it brains. So I got recall fixed and my cars went to hell in a hand basket. Now another recall is issued but no resolution. It is a huge safety issue when your gas pedal gets a not responsive to your foot touching the pedal then it accelerating without touching the gas pedal. I’m lucky I didn’t die in it. This year model just needs to be lemoned out. Oh also when it starts messing up airbag not working driver assist not working all the safety features go haywire. I feel 💯 sure this is all related to the recall and ford will be hearing from me or a lawyer one.

Three alerts appeared on my dash after starting my car. After discussing with other consumers, this system failure is happening more and more. While these alerts are displayed, the front end collision assist is completely nonfunctional, thus increasing the risk of an accident. The Advanced Traction Control System is also nonfunctional. Based on other consumers with the same experience, the only solution is to replace the AWD module.

On December 26, at approximately 7:08 p.m., I was driving on the Belt Parkway during a snowstorm. I was traveling in the middle lane and began to safely change lanes to the right. The vehicle veered to the right, struck the median, and flipped over twice before coming to rest on its side. Was good for about 8 months still had a problem when in reverse splitting out of gear then would slam back in to gear. Then the power train malfunction comes on every time I drive it, Took it back to dealership he updated the software I was told it was a bulletin not a recall and it is going to cost me over 2000.00 to fix since the update didn’t fix the problem. I am truly disappointed but stuck with ford at the moment. How can this not be a recall when you go on and see the hundreds of complaints saying the same thing.

Ford Motor Company (Ford) is recalling certain 2020-2027 Aviator and 2020-2026 Explorer vehicles. The switch for the easy-entry second-row outer seats may bind or stick, resulting in the seats unlatching, tipping, or sliding unexpectedly.

Consequence & remedy

Consequence: A seat that moves unexpectedly may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and replace the bezel around the switch as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters explaining the safety risk were mailed August 19, 2026. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence: An electrical short circuit can increase the risk of a fire.

Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles previously repaired incorrectly under recall numbers 23V-199 or 22V-255. The rear axle horizontal mounting bolt may fracture and cause the driveshaft to disconnect.

Consequence & remedy

Consequence: A disconnected driveshaft can result in vehicle rollaway when the vehicle is placed in park without the parking brake applied. A disconnected driveshaft can also result in a loss of drive power. Either of these scenarios can increase the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2021 Explorer and Aviator vehicles. The seat belt buckle anchor bolts at one or more seating positions may be improperly secured. Additionally, vehicles may have an improperly secured seat belt retractor anchor bolt and/or seat belt anchor bolt at the second-row center seating position, if equipped.

Consequence & remedy

Consequence: A loose seat belt or seat belt buckle may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect seat belt buckle anchor bolts in all seating positions. Additionally, dealers will inspect the seat belt retractor anchor bolt and seat belt anchor bolt at the second-row center seating position, if equipped. If loose anchor bolts are found, the affected seat components will be replaced. Repairs will be performed free of charge. Owner notification letters were mailed on June 5, 2025. Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles equipped with the Police Interceptor Utility Package. In the event of an engine failure, engine oil and fuel vapor may be released into the engine compartment and accumulate near ignition sources such as hot engine or exhaust components, possibly resulting in an engine compartment fire.

Consequence & remedy

Consequence: An engine compartment fire increases the risk of injury.

Remedy: Owners are advised to park and shut off the engine as promptly as possible upon hearing unexpected engine noises, or after experiencing an unexpected engine power reduction, or seeing smoke from the engine compartment. Dealers will update the Powertrain Control Module (PCM) software. In addition, dealers will inspect for connecting rod bearing failure and if necessary, replace the engine long block. Ford Motor Company (Ford) is recalling certain 2020 Explorer and Lincoln Aviator vehicles. The Manual Park Release (MPR) cover may not be installed. In addition, the vehicles may be in Factory Mode, which disables warning alerts and does not display the transmission gear selected. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 114, "Theft Prevention" and 102, "Trans Shift Lever Seq/Starter Interlock/Trans Braking Effect."

Consequence: Without the cover, the MPR lever may be unintentionally moved, allowing the vehicle to roll away. Being in Factory Mode, the vehicle will not display warnings and/or gear selection on the instrument cluster. Both conditions increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will inspect and, if necessary, install a MPR cover. Dealers will verify the instrument cluster is not in Factory Mode and clear any fault codes, free of charge. The recall began August 23, 2019. The Office of Defects Investigation (ODI) received a petition on August 22, 2025, requesting a defect investigation into an alleged defect of the “underbody shields” on model year (MY) 2020 Ford Explorer vehicles. The petition alleges premature degradation and detachment of the plastic underbody shields, potentially leading to road debris. The petition itself can be reviewed at NHTSA.gov under ODI number 11682326. A search of the subject vehicle population was undertaken using complaints submitted to ODI. No related complaints were found in NHTSA’s database for the subject vehicle population. Subsequently, ODI conducted a broader search for related complaints and field reports involving MY 2020-2025 Ford Explorer and Lincoln Aviator models, as these vehicles utilize substantially similar underbody shields. No complaints or reports were found citing any allegations pertaining to any of the underbody shields. ODI also reviewed manufacturer information and data and did not identify a trend related to the Petitioner’s allegations. Based on the absence of consumer complaints and reports, ODI has not identified a material risk of a complete detachment or a projectile hazard related to this condition. The petitioner's description of detectable symptoms - including noise and vibration that progressively worsen and visible sagging of the shields underneath the vehicle - indicates a failure mode that provides drivers with ample notice to seek service before a potential detachment. In view of the insufficient evidence of a vehicle-based defect pertaining to the underbody shields of the subject vehicle, ODI is denying this Defect Petition. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. On July 17, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (MY) 2020-2022 Ford Explorer vehicles (subject vehicles). The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs.
