I own a 2020 Ford Explorer XLT AWD, VIN 1FMSK8DH6LGA35229. While driving, my vehicle displayed “4WD Fault – Service Required” along with the powertrain wrench warning. I then became unable to select/change drive modes. Pre-Collision Assist became unavailable, I experienced Lane Keeping/Lane Assist problems, and additional stability/traction warnings appeared. A professional diagnostic scan found AWD codes C0631:7E-68 and C0631:7F-6C related to the front axle disconnect actuator. The ABS module also stored U0102 (Lost Communication With Transfer Case Control Module) and U0155 (Lost Communication With Instrument Panel Cluster). My mechanic diagnosed failure of the front axle disconnect actuator/intermediate-shaft system. The repair requires removal of the original disconnect system and installation of Ford’s revised intermediate shaft/axle components, replacement of the vent/breather hose, and AWD programming. I was quoted approximately $1,337. The mechanic will then determine whether the transfer case/AWD control module also requires replacement, which would cost approximately $828 additional. Ford has issued TSB 25-2373, superseding TSB 23-2174, for certain 2020-2021 Explorer AWD vehicles. The bulletin specifically identifies C0631 and/or C00A6 and the front axle disconnect actuator/intermediate shaft and directs replacement with a revised intermediate shaft. I am concerned this is a safety defect because the failure resulted in AWD/drive-mode malfunction while multiple driver-assistance systems became unavailable. I request that NHTSA investigate this known failure and determine whether a safety recall is warranted.

On August 14, 2026, the power liftgate on my 2020 Forde Explorer unexpectedly began closing while I was loading my dog into a kennel in the rear cargo area. I did not press the liftgate close button and no one else activated a liftgate control. My key fob was inside my purse on the front seat of the vehicle. The liftgate came down while I was underneath it and struck me directly on the head. The impact knocked me to the ground. I saw stars and was confused immediately afterward and sustained a scalp laceration with significant bleeding. I did not lose consciousness and did not require stitches. One person directly witnessed the incident and was also struck by the closing liftgate. Because the liftgate struck my head first, its force was reduced before it contacted the other person, who was not injured. Other people arrived immediately afterward and can confirm my condition following the impact and that the key was in my purse on the front seat. This was not the first time this has happened. It has occurred on multiple occasions while loading without intentionally activating. I have been hit in the head before but not enough to cause bleeding. The incident occurred Friday evening on 8/14/2026. Vehicle has not yet been inspected because the service department is closed for the weekend. I plan to contact Ford once open on Monday. I am reporting this because I am concerned there may be a defect involving unintended activation of the power/hands-free liftgate and/or its safety obstacle-detection system. The unexpected closure has now caused an injury and also struck a second person.

The contact owns a 2020 Ford Explorer. The contact stated that the vehicle experienced intermittent starting failures, and upon starting the vehicle and shifting into gear, the vehicle shut off. No warning light illuminated. Additionally, the electronic parking brake was unintendely engaged. The contact replaced the battery; however, the failure persisted. The vehicle was not diagnosed or repaired by a dealer or an independent mechanic. The manufacturer was not notified of the failure. The failure mileage was approximately 100,000.

My wife was driving and the horn started blowing and the rear wiper started ejecting all the wiper fluid. When I showed up to assist, it smelled super hot and I had to pull the fuse and negative lead to make the wiper pump and horn stop. It was towed to a Ford dealer, who found the main engine harness had MELTED going into the engine compartment fuse box. The horn and wiper fluid pump also had to be replaced. It's fortunate this did not result in a fire. Ford dealer had the vehicle for TWO MONTHS to repair (and we had to take it back for the horn and washer pump). The Ford dealer said they could not give any reason for why this happened. The entire main harness and fuse panel had to be replaced. No check engine or warning lights appeared when this happened. My wife had to pull over on the side of the road.

With people in the backseat, the seats decided to fold out of nowhere from the previous recall where the seat released buttons on the second row get stuck. Even though it’s very clear that these buttons need to be replaced and I took it to a Ford dealership they said that they were fine even though the buttons literally are stuck in no matter what and ever so slightly touching them causes the seats to fold. The passengers in the second row were not injured, but very easily could’ve been.

I am filing a complaint regarding a recurring safety-related electrical and AWD defect in my 2020 Ford Explorer. My vehicle repeatedly displays the following warnings while driving: “4WD Fault Service Required” “Service AdvanceTrac” “Pre-Collision Assist Not Available” “Hill Start Assist Not Available” The issue began about a year ago occurring intermittently every few weeks and disappearing after restarting the vehicle. It has progressively worsened and now appears almost immediately after driving only a few feet. The defect affects multiple critical safety systems including AWD/4WD, traction/stability control, hill assist, and collision avoidance systems. I have contacted Ford multiple times. Ford only opens customer service cases but has not provided a permanent repair. Ford is aware this is a common issue with 2020 Explorers specifically, and later model years appear to have been corrected. Numerous owners report the exact same failures online. Ford has issued service information involving software reprogramming, module communication faults, and wiring harness issues, but only Ford dealerships can perform the required programming. This appears to be a widespread manufacturing or engineering defect and creates a serious safety concern while driving. Vehicle Information: 2020 Ford Explorer Mileage: Under 55,000 miles I respectfully request that NHTSA investigate this issue for a potential recall or mandatory corrective action.

Both my passenger and driver headlights have failed. I have paid to have them replaced. The drivers side has failed twice. The passenger side has failed 3 times. I have taken it to the dealership and they “fixed” the issue but it keeps coming back months or weeks later. The low beams are what is failing. The message on the dash says “check headlight assembly see manual” This is a safety risk to me and other people on the road. They have failed while driving. If I am driving early in the morning or late at night I can not see if there are no street lights. People also can’t see me coming. This is a major safety risk. I bought the car in August 2024. Headlights have failed 5 times since then. The dealership helped me with my first four failures. On this last failure, they said it is not covered under warranty and that I need to pay $3000 for the headlight. Five failures of headlights seems like a manufacturer issue and not something. I am doing on my end.

Consequence & remedy

Consequence: A seat that moves unexpectedly may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and replace the bezel around the switch as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters explaining the safety risk were mailed August 19, 2026. Consequence & remedy

Consequence: Park system damage may result in a vehicle rollaway, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2016-2019 Lincoln MKC, 2016-2023 Explorer, 2019-2020 Fusion, 2019-2024 Ranger, 2020-2022 Lincoln Corsair, Escape, 2021-2024 Bronco, Bronco Sport, and 2022-2024 Maverick vehicles. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence & remedy

Consequence: An electrical short circuit can increase the risk of a fire.

Remedy: Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Owners will also have a alternative option to replace engine block heater element with a threaded blanking plug, and remove the block heater electrical cord. Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles previously repaired incorrectly under recall numbers 23V-199 or 22V-255. The rear axle horizontal mounting bolt may fracture and cause the driveshaft to disconnect.

Consequence & remedy

Consequence: A disconnected driveshaft can result in vehicle rollaway when the vehicle is placed in park without the parking brake applied. A disconnected driveshaft can also result in a loss of drive power. Either of these scenarios can increase the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2021 Explorer and Aviator vehicles. The seat belt buckle anchor bolts at one or more seating positions may be improperly secured. Additionally, vehicles may have an improperly secured seat belt retractor anchor bolt and/or seat belt anchor bolt at the second-row center seating position, if equipped.

Consequence & remedy

Consequence: A loose seat belt or seat belt buckle may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect seat belt buckle anchor bolts in all seating positions. Additionally, dealers will inspect the seat belt retractor anchor bolt and seat belt anchor bolt at the second-row center seating position, if equipped. If loose anchor bolts are found, the affected seat components will be replaced. Repairs will be performed free of charge. Owner notification letters were mailed on June 5, 2025. Consequence: Without a label that shows the proper restraint usage guidelines, the occupants may have an increased risk of injury in the event of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles equipped with the Police Interceptor Utility Package. In the event of an engine failure, engine oil and fuel vapor may be released into the engine compartment and accumulate near ignition sources such as hot engine or exhaust components, possibly resulting in an engine compartment fire.

Consequence & remedy

Consequence: An engine compartment fire increases the risk of injury.

Remedy: Owners are advised to park and shut off the engine as promptly as possible upon hearing unexpected engine noises, or after experiencing an unexpected engine power reduction, or seeing smoke from the engine compartment. Dealers will update the Powertrain Control Module (PCM) software. In addition, dealers will inspect for connecting rod bearing failure and if necessary, replace the engine long block. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that displays a blank or distorted image can reduce the drivers view of what is behind the vehicle, increasing the risk of a crash.

They will also inspect the rear axle cover for damage near the bolt hole location and replace with a new cover if any damage is found. Repairs will be performed free of charge. Ford Motor Company (Ford) is recalling certain 2020-2022 Lincoln Aviator and Ford Explorer vehicles equipped with 3.0L PHEV engines. A manufacturing defect in one or more of the high voltage battery cells may result in an internal short circuit and battery failure.

Consequence: Battery failure can cause a loss of drive power, increasing the risk of a crash. Battery failure can also increase the risk of a fire.

Remedy: Dealers will perform a Battery Energy Control Module (BECM) diagnostic test and replace the high voltage battery pack, as necessary, free of charge. When the Display On/Off Button is used to turn the display to "Off" prior to placing the vehicle in reverse, the backup camera rearview image will not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury.

The Office of Defects Investigation (ODI) received a petition on August 22, 2025, requesting a defect investigation into an alleged defect of the “underbody shields” on model year (MY) 2020 Ford Explorer vehicles. The petition alleges premature degradation and detachment of the plastic underbody shields, potentially leading to road debris. The petition itself can be reviewed at NHTSA.gov under ODI number 11682326. A search of the subject vehicle population was undertaken using complaints submitted to ODI. No related complaints were found in NHTSA’s database for the subject vehicle population. Subsequently, ODI conducted a broader search for related complaints and field reports involving MY 2020-2025 Ford Explorer and Lincoln Aviator models, as these vehicles utilize substantially similar underbody shields. No complaints or reports were found citing any allegations pertaining to any of the underbody shields. ODI also reviewed manufacturer information and data and did not identify a trend related to the Petitioner’s allegations. Based on the absence of consumer complaints and reports, ODI has not identified a material risk of a complete detachment or a projectile hazard related to this condition. The petitioner's description of detectable symptoms - including noise and vibration that progressively worsen and visible sagging of the shields underneath the vehicle - indicates a failure mode that provides drivers with ample notice to seek service before a potential detachment. In view of the insufficient evidence of a vehicle-based defect pertaining to the underbody shields of the subject vehicle, ODI is denying this Defect Petition. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published. On July 17, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (MY) 2020-2022 Ford Explorer vehicles (subject vehicles). The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. Ford stated that the Explorer powerpack variants not covered under Recalled 23V-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque.  According to Ford, this is also evidenced by the lower current and projected failure rates. On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs.
