On August 25, 2026 when I went to turn my vehicle on in the morning, and I started to drive the vehicle alert the LTA malfunction Visit your dealer on the dash. Call dealer and scheduled service for the next day August 26, 2026. In the morning I went to go start my vehicle to take it in and it wont start at this point on the dashboard was showing Not Ready to Drive and it would not start and I reached out again to the dealer and the had roadside assistance to tow the vehicle in to the dealer. When roadside assistance I gave him my keys and the vehicle started up but now vehicle dash alerted Drive-start Control Malfunction visit your dealer. Roadside did nothing but up the key in the ignition. Vehicle once towed to the dealer was running and no alerts showed on the dashboard and the dealer informed that the multi- point vehicle everything checked out. Jayleen informed that tech ran the computer and saw the alert but could not do no repair due to at the time of service there was no sign on the dash and if where to happen to bring the vehicle in to the dealer. Jayleen also mention that wire my have been chewed.I asked it they inspected for that, she informed No due to no alerts where I dashboard. Then she when on to say that it was my dash camera that was draining my battery. I mention to her that the vehicle was not coming in for battery issues. So I uninstalled my dash camera to see if that was the issues. My vehicel has been parked mostly. Now today September 7,2026 car did not start and show pre-Collision System Malfuntion.

On July 31, 2026, while traveling approximately 55 mph on a Maryland highway, my 2025 Toyota RAV4 unexpectedly lost power and shut off while in motion. There was no check-engine warning before the failure. The vehicle became disabled in a travel lane. Police responded, and the lane had to be closed because the vehicle could not be safely moved. There was no crash, fire, or injury. The vehicle was taken to an authorized Toyota dealership. The dealer documented that it had “shut off mid drive,” reported finding no codes in the engine ECU, and performed service campaign 26TC03 concerning an incorrect mass airflow sensor reading. The dealer replaced a radiator air guide and stated that the vehicle should return for further diagnosis if it stalled again. On August 2, only approximately 36 miles after that service, the vehicle failed to restart at approximately 40,529 miles and required a jump. LTA and pre-collision system malfunction warnings appeared. The pre-collision warning returned while the vehicle was moving, and the Toyota connected app also recorded the malfunction. The vehicle was then towed back to the dealership and is available for inspection. The cause remains unknown. The concern involves an intermittent engine shutdown/no-start condition, a possible 12-volt electrical or charging-system failure, and the loss of forward-collision and lane-assistance functions. The dealer’s prior multipoint inspection marked the battery as acceptable, but no actual CCA battery-test result was documented.

UNKNOWN - On XXX, at just 7,741 miles, my 2025 Toyota RAV4 XLE Premium AWD (VIN: [XXX]) suffered a catastrophic and life-threatening complete mechanical and hydraulic transmission failure. The vehicle completely lost forward propulsion when placed in Drive mode, rendering it entirely immobile. When placed in Reverse gear, the transmission violently over-pressurized, causing the vehicle to suddenly surge and jump uncontrollably. Simultaneously, the dashboard displayed 6 critical safety system failures: 1. Check Engine — electronic control malfunction 2. Vehicle Stability Control System (VSC/TRAC) malfunction 3. Secondary Collision Brake System malfunction 4. Pre-Collision System malfunction 5. Parking Brake System malfunction 6. Advanced Ultrasonic Detecting and Ranging System malfunction Ourisman Toyota Chantilly (RO# XXX) has confirmed complete transmission failure. Fault codes found: P28167F — Pressure Control Solenoid H Actuator Stuck Off, P08BA7F — Pressure Control Solenoid L Actuator Stuck Off. Estimated repair cost: $11,347.42. While this failure fortunately occurred on a local road, had it occurred while merging onto I-66 or I-95 at highway speeds, it would have inevitably caused a catastrophic high-speed collision. This represents an extreme manufacturing defect and life-threatening safety hazard. Toyota Corporate Cases opened: #XXX (original case) and #XXX (full replacement request). SUPPLEMENTAL TO COMPLAINT ID: 11728745. My 2025 RAV4 failed at 39,919 miles. Toyota Corporate is acting in bad faith to deny the 60,000-mile Powertrain Warranty. My 2025 Toyota RAV4 (39,919 miles) has a permanent "Check Engine" light. On March 18, 2026, diagnostic codes P24517F and P214502a were found, indicating a failure in the switching valve and fuel tank pressure sensor. Toyota USA and the dealer are refusing to honor the 60,000-mile Powertrain Warranty and Federal Emissions Warranty. I am reporting this as a safety defect because the vehicle may lose power or stall on the highway, creating an immediate crash risk. The manufacturer's refusal to repair a known emission/fuel defect in a new 2025 model has left me with an unsafe vehicle for over 15days.

We were on a funeral procession when suddenly the car accelerated at a very high speed and hit the end of guard rail head on. My wife on the passenger side and two people at the back were all taken to the emergency room. I got pinned on the steering column and taken out via the roof. I had severe abrasions on both legs, wife has 3 broken ribs,the 2 passengers suffered whiplash and complains of chest pains. Car was purchased 11/3/2025 and I took it for a recall fix about a month ago at Burien Toyota dealership where we bought it. Incident # 2026-55959 Seattle police. At present our car insurance is investigating and our claim # is 082 0356939 Allstate.

On [XXX], at approximately [XXX], in Greenwich CT, while I was very slowly driving into a parking space, the engine suddenly and unexpectedly surged,. The vehicle overcame the brakes and raced forward and collided with a stone wall, about 3 ft away. This was a new (since May 2025) vehicle with only approx 3,500 miles on it. I had not had any previous problems with it and there were no warning lights on the instrument panel. At the time, I was proceeding at a very low rate of speed, 1 - 3 mph. I drive only with my right foot which was on the brake, not on the gas. The vehicle accelerated with NO action on my part and collided with a stone wall approx 3 ft away and bounced back several feet. Fortunately I was close to the wall so the vehicle did not have a long distance to accelerate and I was wearing my seat belt which probably protected me from injury. No other vehicle was involved. I was the sole occupant of the vehicle. I was wearing my seat-belt, the OEM rubber mats were securely in place. The airbags did not deploy and I was not injured. I had the car towed to the Toyota dealership by AAA because I was afraid it might accelerate into traffic at a red light or stop sign if I drove it before the problem was repaired. I have reported this incident to Toyota Motor Corporation USA, through their Brand Engagement Center (as instructed by the dealership) and they are sending out an engineer to the dealership to inspect the vehicle. I do not know which part malfunctioned or failed. The vehicle is at a Toyota dealership and presumably could be inspected (unless they replace some parts). Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 RAV 4 vehicles. The front seat brackets for the seat rails may be welded improperly.

Consequence & remedy

Consequence: Improperly welded seat brackets can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and repair the seat bracket welds as necessary, free of charge. Owner notification letters were mailed June 19, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB09 and 26TA09.

25V744000 · Back Over Prevention:display Function

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Consequence & remedy

Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.
