On 9/06/2026 my 2024 Toyota RAV4 Hybrid was involved in a severe collision where the impact occured on the drivers side at the passenger door. The vehicle spun several times before coming to a stop. During the spins the front passenger door flew opening. My wife was sitting in the passenger seat her seatbelt successfully restrianted her preventing ejection and injury. However the unlatching of a non impacted door under rotational forces represents a catastrophic structural or mechanical latch failure that poses an imminent risk occupant ejection.

I own a 2024 Toyota RAV4. At approximately 38,300 miles, an authorized Toyota dealership diagnosed a failed rack-and-pinion/steering rack assembly that required replacement. Before the repair, I began noticing a squeaking noise when turning the steering wheel. The noise became progressively louder over time. The symptoms continued to worsen until the vehicle was inspected by the Toyota dealership. The vehicle has been regularly maintained, and there was no collision or other known event that would explain this failure. The steering rack required replacement only about 2,200 miles beyond Toyota’s 36,000-mile basic warranty. Because the rack-and-pinion assembly is a critical steering component, I am concerned that this may represent a premature failure and potentially a safety issue if similar failures are occurring in other 2024 Toyota RAV4 vehicles. I am submitting this complaint so the issue is documented and can be compared with reports from other owners. I have dealership repair documentation confirming the diagnosis and replacement of the steering rack assembly.

The contact stated that the front camera and traction malfunction error messages were displayed. The contact stated that the failure had occurred while the contact was driving in a parking lot and attempting to park the vehicle. The vehicle was towed to a local collision shop. No Police report was filed. The contact sustained injuries to the upper chest from the impact and the seat belt. The contact had an upcoming appointment with the primary care physician. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 51,020.

The contact stated that the Headlight System Malfunction, Pre-collision System Malfunction, Road Sign Assist (RSA), and Lane Tracing Assist (LTA) warning lights were illuminated. The vehicle was taken to the dealer, who was unable to diagnose the vehicle. The contact stated that the remedy for Special Service Campaign: 26TC03, which addressed an incorrect MAF Sensor reading failure, had been performed two days prior to the failure. The vehicle was not repaired a second time. The manufacturer was informed of the issues. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 48,000.

Airbags, seats, and headrest all failed. State Farm insurance currently has the vehicle. I do not know if the people has been inspected by anybody. The vehicle had no warning lamps message or other symptoms of problems or failure before the collision. I was traveling in the far left lane when traffic came to abrupt stop and I was able to stop 10 to 15 ft from the vehicle in front of me. Two vehicles in my line behind me swerved out of the way. Then a semi truck with a trailer rear-ended me. The front end of the semi truck struck the rear end of my vehicle getting lodged inside my vehicle. My car was then pushed into the car in front of me and dragged 100 to 200 yards down The highway by the semi. The seat belts did not lock. The airbags did not deploy. Both front seat frames were bent and ripped apart. The rear seat that's became detached. The headrest for the front seats flew off. All four seats were occupied by adult males all weighing around 190 pounds or above. All four occupants were wearing seat belts. I the driver was knocked unconscious and received a concussion plus facial damage/injuries. Rear passenger side occupant received a broken jaw and a skull fracture. The other two passengers were also injured. All four passengers of the vehicle were transport to emergency room via ambulance.

2024 Toyota RAV4 Hybrid Limited : issues with vehicle detection but not limited to the Blind Spot Monitoring System (BSM) . Complaint includes problems with the vehicle's sensors and electronic control units (ECUs), which is leading to difficulties in detecting other vehicles on the road and posses ongoing safety concerns about the reliability of my vehicle's safety features, particularly when driving my car on freeways and surface streets. Sensor Malfunctions: I continue to have malfunctions with the sensors that are supposed to detect nearby vehicles. I have taken my vehicle into Wilsonville Toyota’s Service Center in [XXX] twice, once in XXX and again in XXX after having at least two near collisions due to the fact that my car did not detect an approaching vehicle. On one occasion my friend ([XXX]) was in my car with me when we almost hit another car when changing lanes on the freeway because my car DID NOT DETECT a vehicle in the lane I was attempting to change. I feel unsafe due to the inconsistent performance of my BSM System Malfunction. I have visited my Toyota dealer for diagnostics and potential software updates but have been told repeatedly that if they, Wilsonville Toyota Service Center (WTSS) cannot reproduce the issue then it does NOT exisit. However, after having and picking up my vehicle from the repair shop at WVTS on XXX I spoke with the acting Assistant Service Manager, Kevin, who stated to me me that if it happens again to return and he will go for a test drive with me to see if I could reproduce the situation so that he could see it and make documentation of it so that it will then be “a thing”. Well I did just that on XXX when my husband , [XXX] , and I returned to WVTS and took Kevin for a test drive where Kevin experienced my exact concerns with the BSM’s Malfunction on more than 4 times on our short test drive. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Consequence & remedy
Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.
Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.

Consequence & remedy
Consequence: A damaged brake line can cause a brake fluid leak, reducing brake performance and increasing the distance required to stop the vehicle. Wheel detachment can result in a loss of vehicle control. Either condition increases the risk of a crash.
Remedy: Dealers will inspect and tighten the front brake caliper mounting bracket and hub bearing bolts, as necessary, free of charge. In addition, any damaged components will be replaced, free of charge. Owner notification letters were mailed January 23, 2025. Owners may contact Toyota customer service at 1-800-331-4331.
