On [XXX], while driving my 2024 Toyota Grand Highlander at approximately 45-50 mph on a gusty, high-wind day, the rear panoramic moonroof/sunroof panel blew out and shattered completely without warning. The failure occurred as I rolled up the vehicle windows during the wind gust. Rapid window closure created a sudden spike in cabin air pressure — a documented pressure differential mechanism — that forced the moonroof glass outward until it shattered. There was no external impact of any kind. The glass, which is not laminated safety glass, scattered throughout the interior with occupants present. Component is available for inspection. This failure is consistent with NHTSA Investigation PE13-035 (Kia Sorento panoramic sunroof spontaneous breakage), in which ODI found glass shattering while in motion to be a safety hazard warranting engineering analysis. The same pressure-based failure mechanism applies here. This is a documented pattern on this specific model. Lemberg Law [XXX] ) analysis of NHTSA complaint data confirms spontaneous sunroof shattering among the top defects on both 2024 and 2025 Grand Highlanders, with Toyota denying warranty claims by alleging "outside influence" — disputed by owners and attorneys alike. No recall currently exists for this component. The rear seat directly below this glass is a primary location for child safety seats. Non-laminated glass shattering over rear occupants — including infants — represents an acute and unacceptable injury risk. I request NHTSA open a formal defect investigation into the panoramic moonroof/sunroof glass on 2024-2025 Toyota Grand Highlanders and evaluate whether a safety recall is warranted. While driving, my panoramic moonroof exploded. There were no rocks or big vehicles on the road, but the panoramic moonroof simply just exploded. This could've been very dangerous and could've caused serious injuries for those inside of the vehicle, especially if the vehicle was moving.

Our vehicle is less than 2 months old, and it had less than 5,000 miles before highway debris (gravel) chipped and cracked the windshield on two separate occasions. I have never had a chip or crack caused by rocks before, and my previous vehicles have been hit plenty of times. Talking to people that own the Lexus equivalent of my Grand Highlander (Lexus TX), they have similar experiences with what appears to be very weak windshields. I will have to replace the windshield.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Consequence & remedy

Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. The driver and passenger side curtain air bags may not unroll as intended and could partially deploy outside of an open window. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 226, "Ejection Mitigation."

Consequence & remedy

Consequence: An improper deployment of the side curtain air bag can reduce occupant protection during a crash, increasing the risk of injury.

Remedy: Dealers will replace the driver and passenger curtain shield air bag assemblies, free of charge. Owner letters were mailed November 4, 2024. The Office of Defects Investigation (ODI) received a Petition dated July 20, 2026. The Petition requests an investigation of road debris / rocks damaging the heat exchangers in model year (MY) 2024-2026 Toyota Grand Highlander vehicles. The Petition contends that the Grand Highlander’s front-end grill has large, open apertures and no protective screen between the grill and the vehicle’s heat exchangers. The Petition further alleges that as a result, during normal driving, the damage to heat exchangers results in coolant leakage and sudden loss of propulsion. The Petitioner cites thirteen (13) complaints received by ODI alleging this failure, which, when added to the petitioner’s own complaint, bring the total under consideration at this time to fourteen (14). ODI will evaluate these complaints to determine if they are related to the failure cited in the Petition and conduct a technical review under 49 CFR Part 552. This Defect Petition is being opened to evaluate whether to grant or deny the petition. The petition can be reviewed at NHTSA.gov under ODI number 11754975.
