I am reporting what I believe may be a safety defect involving my 2023 Honda Hybrid. I owned the vehicle for less than three years and kept up with all recommended maintenance. During my ownership, I repeatedly experienced sudden tire pressure warnings and rapid drops in tire pressure. Because I was concerned, I took my vehicle to a Honda dealership approximately four to five weekends in a row to have the tires inspected. Each time I was told that everything appeared to be normal and no problem could be identified. In March 2026, while driving on the freeway to school, the tire pressure suddenly dropped again. I then heard a very loud popping noise and initially believed that my tire had blown out. Immediately afterward, I lost control of the vehicle and crashed, and my car was declared a total loss. After the crash, I observed that the wheel had turned inward. I was later told by the responding police officers that the wheel had turned inward rather than the tire simply blowing out. I decided to report this because I have learned of other people with the same model who described similar problems involving sudden tire pressure loss or repeated tire warnings despite being told by dealerships that nothing was wrong. While I cannot verify the cause of those incidents, the similarities make me concerned that there could be an underlying safety issue affecting other vehicles. I respectfully request that NHTSA investigate whether there may be a defect involving the tires, tire pressure monitoring system (TPMS), suspension, steering, or another related component. I am willing to provide any additional information requested. Also I don't know if this was the correct channel to go through, I have never reported something like this before. I also send Honda a longer email detailing everything, so I hope they are aware of this issue. Please contact me if you have more questions, I want to help any way I can. Thank you, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

From the beginning of my lease, the vehicle experienced repeated malfunctions of its safety and proximity sensor systems, including warning alerts and sensor-related failures. The vehicle was taken multiple times to an authorized Honda dealership for diagnosis and repair. Despite these repeated service visits, the safety sensor issues were never properly repaired or resolved. I later discovered that the vehicle was subject to an active safety recall, which was never disclosed to me, even though the vehicle was serviced at the dealership on multiple occasions. Subsequently, the vehicle was involved in an accident that resulted in a total loss. Based on the unresolved sensor failures and the undisclosed recall, I believe these safety defects may have contributed to the accident. This complaint is submitted to report a serious safety concern and to request that NHTSA investigate whether similar vehicles may present a broader safety risk.

They inspected it, told me that they could not find anything wrong. I just leased it 2wks ago and only have driven it 3 times , not even a hundred miles and this happens. There were no warning signs of any kind . If I would have been approaching the freeway I could have been killed.

Subject: Complaint Regarding Steering Noise Issue on 2023 Honda Accord Hybrid Dear Honda Customer Service, I am writing to formally file a complaint regarding the steering issue on my 2023 Honda Accord Hybrid, purchased in June 2023. •In July 2024, when my vehicle had around 29,540 miles, I first reported a steering noise issue to the dealership. At that time, the dealer only applied lubrication, which did not resolve the root cause. •In July 2025, when the mileage was around 60,440 miles, the same steering noise reappeared. The dealership replaced the right inner tie rod and performed an alignment, but the noise persisted. Later, they concluded that the steering rack would eventually need replacement. •I also noticed that this issue only occurs in extremely hot weather (high summer temperatures around 90°F). Once the temperature drops to around 70–80°F, the noise disappears completely. Because of this seasonal nature, the problem is very hard to reproduce during normal conditions, and it was not fully diagnosed while the car was still under warranty. Now, Honda has denied goodwill repair, stating that the vehicle has exceeded 60,000 miles. However, I strongly believe this problem started during the warranty period (July 2024) and was not properly resolved by the dealer, despite multiple visits. The issue is clearly related to a defect that only appears in extreme temperatures, which is beyond my control as a customer. This is extremely disappointing, as I have always trusted Honda’s reputation for quality and reliability. A steering problem is a serious safety concern, and denying repair based solely on mileage—when the defect clearly originated under warranty—feels unfair and unprofessional. I am requesting that Honda reconsider this case and provide goodwill repair coverage for the steering rack replacement. If this matter is not addressed, I will have no choice but to escalate my complaint to the National Highway Traffic Safety Administration (NHTSA),

Component/System that failed or malfunctioned: Power Train: automatic transmission internal (CVT). The vehicle stops responding to acceleration, remains static while the engine continues at high revolutions, without generating forward movement. The component is available for inspection upon request. How safety was put at risk: The incident occurred while driving on the highway. By losing acceleration power and not being able to maintain the required speed according to traffic laws, I was at risk of being struck by other vehicles traveling at normal highway speed. This situation posed an immediate danger to both myself and other drivers. Problem reproduction/confirmation: The dealership inspected the vehicle and informed me that, at this time, there is no safety bulletin related to my unit. Inspection by third parties: I have scheduled an appointment with the dealership for September 5, 2025, where a more detailed evaluation of the problem is expected to be performed. Warning signs or prior symptoms: When the failure occurs, several warning lights illuminate on the dashboard and then turn off. Timeline / first occurrence: The failure occurred on August 14, 2025, at 9,215 miles. Related campaign reference: There is an existing NHTSA recall campaign for the same vehicle model: NHTSA Campaign ID: 23V588000

Vehicle Power Loss and System Malfunctions I am writing to formally express my concern regarding a recurring and potentially dangerous issue with my vehicle, 2023 Honda Accord Hybrid. Twice while driving, the vehicle suddenly shut down on the freeway and completely lost power. In both cases, I was fortunately able to steer the car to the shoulder of the road, but this posed a serious safety risk. During these incidents, multiple system error messages appeared on the dashboard, including alerts related to braking, lane departure, and other critical functions. Alarmingly, this same issue has also occurred while the vehicle was parked in my garage. Each time, the only way to temporarily resolve the issue has been to shut off the vehicle and restart it. Upon restarting, the only remaining warning is related to the braking system, along with a prompt to bring the car to the dealership. If the vehicle is restarted again, all error messages disappear. I have contacted my dealership after each occurrence and brought the vehicle in for inspection. My car is currently at the dealership, and I am awaiting information on the cause of the problem and the proposed resolution. After the first time it happened, I have taken video/pictures of the errors and supplied same to the dealership. I consider this a serious safety concern—not only for myself but also for any passengers and other drivers on the road. Quite honestly, I am now afraid to drive the car, knowing that it could unexpectedly shut down at any moment without warning.

RQ24006 · Front Seat Belt Assemblies May Be Missing The Rivet That Secures The Quick Connector And Wire Plate

Opened Apr 4, 2024 · Closed Mar 26, 2025

Status: closed (inferred from source dates) · Seat Belts; Seat Belts:front

On April 4, 2024, the Office of Defects Investigations (ODI) opened RQ24006 to investigate American Honda Motor Co.’s (Honda’s) proposed remedy for recall 23V-782 as well as scope of the vehicles involved. Honda issued recall 23V-782 on November 21, 2023, which concerns certain model year (MY) 2023-2024 Honda Accord, Honda Accord Hybrid, and Honda HR-V vehicles. Recall 23V-782 states that the driver and front passenger seat belt pretensioner assemblies may have been manufactured without a rivet that secures the pretensioner quick connector and wire plate. Recall documentation states that seat belt pretensioner assemblies that are missing the rivet will not properly restrain the occupant in the event of a collision, increasing the risk of injury.Subject defective seat belt assemblies are installed as original equipment for both the driver and front passenger positions for the HR-V, but only for the driver position for the Accord and Accord Hybrid. The recall remedy for 23V-782 instructs the technician to inspect both front seat belt pretensioner assemblies and, if necessary, replace them. The inspection procedure requires the technician to utilize a pull force gauge to determine whether the subject component is missing the subject rivet and requires replacement. Parts requiring replacement are determined by whether the pretensioner quick connector separates from the pretensioner assembly while the technician is pulling on the seat belt. If no separation occurs, then the part is determined to be not defective and not require replacement. There have been no reports alleging a failure of seat belt pretensioner assemblies after the inspect and replace remedy for recall 23V-782 was performed. Additionally, no comparable failures have been observed in field data for similar vehicles outside of the scope of the recall. As of the 4th quarter of 2024, the recall completion rate is at approximately 84% of the total recalled population. In view of the lack of failures after the remedy for recall 23V-782 was performed, and the lack of failures in similar vehicles outside of the scope of the recall, ODI is closing this Recall Query (RQ). The Agency reserves the right to take additional action if warranted by future circumstances.
