The contact owns a 2021 Ford Explorer. The contact stated that he had received a recall notification for NHTSA Campaign Number: 22V255000 (Power Train). During a routine maintenance check, the recall was serviced; however, upon return of the vehicle, the brake pedal was depressed to the floorboard with the parking brake fault, uphill assist, and ABS warning lights illuminated. The contact also stated that while driving at various speeds, the brake pedal hardened and the vehicle failed to stop as needed. The contact's wife called the dealer and was informed that there were 5 other vehicles that had experienced the same failure. The contact attempted to reach the manufacturer but was unsuccessful. The vehicle was not repaired. The failure mileage was approximately 37,000.

I recently had the recall addressed at my local Ford dealer, Performance Ford in Randolph, NJ. Ford is only allowing dealerships to reprogram the parking break to engage whenever the vehicle is placed in park. If the axle bolt were to shear, it would disengage, allowing the vehicle to roll in park. This remedy does not adequately address the underlying issue- which was the fact that Ford’s Chicago mfg facility used a part designed for the 4-cyl engine power train, not the 4 bolt design required in the Police Interceptor and Explorer ST versions. The police version will be getting the parts upgraded while consumers are left with a band-aid solution. NHTSA should force Ford to address the issue- which is an inadequate bolt configuration for the HP/Torque output of performance version Explorers. This is a safety hazard as the bolt can snap while in use, disconnecting the drive axle from the rear differential.

My 2021 Ford Explorer should be included in safety recall NHTSA 22V255, Ford recall 22S27, Ford Program 22N06 My vehicle[XXX] was built in September 2021. Ford identified to NHTSA that Explorer vehicles built between June 4 2020 to November 10, 2021 were affected. My vehicle was built during that period. My vehicle has the V6 engine with AWD powertrain. My vehicle has the 52T trailer towing package. Trailer towing will put additional load on the powertrain and the rear axle mounting bolt. The bolt may fracture while towing the trailer leading to a serious injury and personal property damages. I have not received the recall letter from Ford. All letters were supposed to be sent by June 2022. Ford will not include my vehicle in the recall at my request. Ford will not apply the PCM update to automatically engage the electronic parking brake every time the vehicle is shifted into Park (P). Ford will not provide the one-time repair of the for 10 years or 150,000 miles. I'm concerned the vehicle may experience the rear axle mounting bolt fracture. Please help. A fractured rear axle bolt will allow the rear axle housing to move out of position, resulting in severe noise and vibration. If the rear axle bolt breaks, the driveshaft/half shafts may become disconnected, resulting in loss of transmission torque to the rear wheels which is necessary to hold the vehicle in park. Our 2021 Ford Explorer Limited was purchased in December of 2020. It currently has been driven only 6,900 miles. Our apparent transmission problem has occurred three times so far. 1. On 27 June 2022, I experienced the first problem. While turning left on to a four-lane city street, which is largely flat in that area, a loud grinding/ringing sound occurred. Right in the middle of the four-lane street, the transmission dropped into neutral. It scared the crap out of us. We could have been hit by oncoming traffic. I brought it to a stop, then shifted to park and then into drive. We drove away normally as if nothing happened. 2. On 28 July 2022, after having the "bolt failure" recall completed (22S27/22V255/22N06), the transmission again made the loud grinding/ringing sound immediately after accelerating from stopping at a city street intersection. The road grade has a slight incline in the direction we were traveling. The vehicle dropped into neutral, then the newly programmed "auto braking" brought us to an abrupt stop in the middle of this intersection! Again, a scary situation. 3. Today, 25 August 2022, while backing out of our garage today, which has a slight downward driveway, the vehicle did an abrupt stop while halfway out of the garage. The message was "Hill Start Assist Warning." The message on my phone was "The Hill Start Assist System has detected a failure." This is the first and only time that this problem has happened. I think it is related to the problems mentioned above. It is a good thing I was not backing out into a busy street.

There was no damage to the vehicle, however, there was damage to the garage back wall. The contact stated his wife saw no warning lights were illuminated. The contact's wife got back into the vehicle and observed that the gear shift lever was still in "Park" and she shut the vehicle off. The contact had not taken the vehicle to a local dealer, or an independent mechanic. The vehicle had not been diagnosed or repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 10,000.

Feedback on Manufacturer Recall Number 22S27, NHTSA Recall 22V255 Allowing Ford to reprogram the powertrain control unit (ECU) on this vehicle so that the vehicle applies the emergency brake whenever the vehicle is placed in park in order to prevent the vehicle from rolling away if/when this one bolt breaks on the rear differential is NOT sufficient. The 2020 MY Ford Explorer ST/Plantium models have two bolts attaching the rear differential to the rear subframe. The equivalent 2020-2022 Lincoln Aviator also has two bolts on this part. The issue is that Ford ran out of the correct rear subframe parts and substituted a part for a lower powered powertrain, or this was a cost cutting measure gone bad. Cross referencing Ford's own parts numbers with the VIN shows the incorrect rear subframe installed at the factory. NHTSA should require Ford to properly and securely attach the rear differential to the rear subframe of the vehicle to prevent this issue. Allowing a workaround, use of parking brake to prevent rollaway, is not ok. Additionally, the equivalent police interceptor is obtaining the correct rear subframe as part of the manufacture defect resolution. Why wouldn't the others receive the same fix?

The contact stated that he forcefully depressed the brake pedal in an effort to stop the vehicle. The manufacturer was notified of the failure and transferred him to the NHTSA. Upon investigation, the contact discovered NHTSA Campaign Number: 22V255000 (Power Train) which he linked to the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 10,000.

Ford Motor Company (Ford) is recalling certain 2020-2027 Aviator and 2020-2026 Explorer vehicles. The switch for the easy-entry second-row outer seats may bind or stick, resulting in the seats unlatching, tipping, or sliding unexpectedly.
Consequence & remedy
Consequence: A seat that moves unexpectedly may not properly restrain an occupant during a crash, increasing the risk of injury.
Remedy: Dealers will inspect and replace the bezel around the switch as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters explaining the safety risk were mailed August 19, 2026. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence: An electrical short circuit can increase the risk of a fire.
Remedy: Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Ford Motor Company (Ford) is recalling certain 2020-2021 Explorer and Aviator vehicles. The seat belt buckle anchor bolts at one or more seating positions may be improperly secured. Additionally, vehicles may have an improperly secured seat belt retractor anchor bolt and/or seat belt anchor bolt at the second-row center seating position, if equipped.
Consequence & remedy
Consequence: A loose seat belt or seat belt buckle may not properly restrain an occupant during a crash, increasing the risk of injury.
Remedy: Dealers will inspect seat belt buckle anchor bolts in all seating positions. Additionally, dealers will inspect the seat belt retractor anchor bolt and seat belt anchor bolt at the second-row center seating position, if equipped. If loose anchor bolts are found, the affected seat components will be replaced. Repairs will be performed free of charge. Owner notification letters were mailed on June 5, 2025. Consequence: Without a label that shows the proper restraint usage guidelines, the occupants may have an increased risk of injury in the event of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles equipped with the Police Interceptor Utility Package. In the event of an engine failure, engine oil and fuel vapor may be released into the engine compartment and accumulate near ignition sources such as hot engine or exhaust components, possibly resulting in an engine compartment fire.
Consequence & remedy
Consequence: An engine compartment fire increases the risk of injury.
Remedy: Owners are advised to park and shut off the engine as promptly as possible upon hearing unexpected engine noises, or after experiencing an unexpected engine power reduction, or seeing smoke from the engine compartment. Dealers will update the Powertrain Control Module (PCM) software. In addition, dealers will inspect for connecting rod bearing failure and if necessary, replace the engine long block. Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles. The rear axle horizontal mounting bolt may fracture and cause the driveshaft to disconnect.
Consequence & remedy
Consequence: A disconnected driveshaft can result in a loss of drive power or a vehicle rollaway if the parking brake is not applied. Either of these scenarios can increase the risk of a crash.
Remedy: Dealers will replace the subframe bushing and rear axle bolt. They will also inspect the rear axle cover for damage near the bolt hole location and replace with a new cover if any damage is found. Repairs will be performed free of charge. Consequence: Battery failure can cause a loss of drive power, increasing the risk of a crash. Battery failure can also increase the risk of a fire.
Remedy: Dealers will perform a Battery Energy Control Module (BECM) diagnostic test and replace the high voltage battery pack, as necessary, free of charge. When the Display On/Off Button is used to turn the display to "Off" prior to placing the vehicle in reverse, the backup camera rearview image will not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence: A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury.

Consequence & remedy
Consequence: With a loose motor mount, motor vibration may cause the axle to disconnect from the engine and could result in loss of power, increasing the risk of a crash.

On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury.
