I purchased a 2024 Toyota RAV4 in March 2024 with the expectation that its advertised safety and driver-assistance systems would function as designed. The vehicle has experienced a failure of the forward camera/ADAS system, resulting in the loss of multiple Toyota Safety Sense features. The following systems are currently unavailable or malfunctioning: • Dynamic Radar Cruise Control / Adaptive Cruise Control • Pre-Collision System • Lane Tracing Assist / Lane Keeping Functions • Road Sign Assist (speed limit recognition) • Related driver-assistance and safety warning functions When cruise control is activated, the vehicle displays a warning that cruise control is unavailable. The dashboard reports multiple safety-system malfunctions. As a result, I have lost essentially all advanced driver-assistance features that were included with the vehicle. Toyota’s dealer diagnosed the failure and Toyota approved the repair under a goodwill case. However, I was informed that the required replacement component is on national backorder with an estimated wait of approximately two months and no guaranteed delivery date. Despite acknowledging the defect and approving repair, Toyota has not provided a comparable loaner vehicle or alternative transportation. I am being asked to continue operating the vehicle for an extended period without the safety technologies that were a significant factor in my purchase decision. I am concerned about the loss of collision-avoidance, lane-assistance, and adaptive-cruise functions, the indefinite repair delay, and the lack of transportation assistance while awaiting parts. I am requesting investigation, assistance, and consideration of repurchase or other appropriate remedies.

Everything working properly and just fine until Monday 3/2/26 when I started the vehicle to go to work, the main radio multimedia screen was black and did not display anything. No sound was playing either along with when I placed the car in reverse, there was not the backup camera display as well. Everything was pitch black. I then proceeded to drive the car to work and then decided to take it to the Toyota dealership near my house at the earliest convenience which was today (3/4/26). In the mean time until I brought the car to the dealership, I researched information and videos on how to diagnose it myself and try troubleshooting my self. Also, when car is running, I got a message stating malfunction with AEB/FCW. Tried the fuse route and tried disconnecting the negative battery terminal for 15 minutes and tried holding the main power knob for 30 plus seconds. All of those did not work. I then took into dealership and once I got there, the service technician saw the screen working and then also saw the screen acting up and turned on but had lines in the middle of the screen. The technician had me turn vehicle off and once we turned back on it went back to the black screen. So once I went the route of having them diagnose it, they said they could not duplicate concern that we saw with the lines in the screen etc., therefore the technician gave the vehicle back with the screen not working still until we do have it working with the lines. I was about to leave and then the issue with the screen and lines cam up on the screen and I was able to have the technician look at it and diagnose. That is when advisor came to me and said the radio head is completely not working and cannot be fixed or saved. Repair is $2,769 with labor for a new radio head per the advisor. Car is not under warranty because of how many miles are currently on vehicle so I cannot get this fixed for free under warranty. Would have to be out of pocket and I denied that. Please help me get this fixed.

On 12/24/25 I was pulling into a parking space, took foot off accelerator to brake and vehicle suddenly accelerated and engine revved. I kept pushing the brake, but the vehicle kept lurching forward and slammed into car parked in front of me before finally stopping. My vehicle was going so hard forward that it pushed the car I hit 2 to 3 feet out of it's parking space and even punctured the radiator. It was not raining, the floor mat was not in the way, I did not have cruise control engaged. On 12/10/25 I took my car to Toyota dealership because I had a slow leak in one tire, which had to be replaced. At that time, the service department also updated parking assist software (could that be the problem?). In answer to above questions: * brakes and accelerator malfunctioned and it is available for inspection upon request. * no one was in the car I hit. I was not injured. * the problem has not been reproduced to my knowledge or confirmed. * the vehicle has been inspected by a third-party that Toyota engaged and also Erie Insurance. A police report is available. * there was no warning lamps, messages or other symptoms of the problem prior to the failure on 12/24/25.

I was heading south on I-495 (Massachusetts) in right lane at just under 65 mph. I had just passed an entrance ramp and overpass. A car was entering I495 on my right and coming up to speed. The car entered my lane from his entrance ramp to the right most lane in front of me. The car was about 20 feet in front of me. The entering car's speed was was greater than mine. When the second the car left the entrance ramp and pulled into my lane about 20-30 feet in front of me, my brakes slammed on, causing me and my passenger to lurch forward. My seat belts protected me and my passenger from harm, but it scared the **** out of us. This kind of nonsense of so-called "safety features" has to stop. Its bad enough that I have to put up with a car that stalls out when the car stops, even for a second, but what happened above is unacceptable as a "safety feature." I could have easily reacted by jerking the steering wheel to the right and gone off the road at full speed, fast enough to roll the car causing considerable harm to life.

On my way home from work it started to rain, and my pre-collision light came on. The rain slowed down as I was home put the vehicle in park to take a picture of the pre-collision light, and the light went off. This happens around October to December of 2024. I checked and cleaned Front windshield and front Toyota emblem. I couldn't find anything wrong. One day in January 2025 after church on a sunny day as soon as I put my vehicle in gear the pre-collision light came on again which I was able to get a picture of. Each time I checked windshield and front emblem and found nothing wrong. I took the vehicle to Hendrick Toyota to have the pre-collision light checked January 23, 2025. I was told no codes were found and the reason my light came on it because my vehicle sensor is dirty. I told the service adviser when my light went of the vehicle was clean including all the sensor. The only reason the vehicle was dirty was because it had snowed the day before and I had to drive in the snow, slush, and salt to get to the dealership. I also informed them this not my first vehicle with forward Collison sensor. I had owned 3 Toyota Corolla with the same tocology. I was also told by another service advisor that She has seen other Rav4 with pre-collision light problems. About one week after dealership checked the pre-collision sensor it came on again and once again, I checked and made sure all sensors were clean. April 4/28/2025 I brought the vehicle to the dealership again to have the pre-collision sensor checked. This time they said they found codes from way back. I asked them how far back and were these codes back from my first service. Service adviser didn't know. But on my paper receipt it said they didn't find any codes but notice a very small rock chip in the glass and blamed it on that, which was not there when my problem had started with my issue of the pre-collision light. They said replace the windshield and that should fix it. I was told by another service advisor,

The contact owns a 2024 Toyota Rav4. The contact stated while driving at an undisclosed speed, the pre-collision warning light illuminated. In addition, the contact stated that the pre-collision warning light illuminated; however, the warning light did eventually go off. The contact stated that the vehicle was taken to the dealer, however the cause of the failure was not determined. The contact was informed that the failure was due to a dirty screen and a rock chip in the windshield. The contact was informed that the front windshield needed to be replaced. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 4,000.

My pregnant wife is parking her car at her work when she accidentally step on the gas pedal instead of the brakes. She panicked and she drove all the way to the wall. Airbags were not activated. 911 was dispatched. Police report made. Only minor injury finger fracture and chest bruise. There was a severe frontal damage to the car. Insurance deemed it totaled due to frame damage. I also complained to Toyota. They hired a software engineer to investigate. Just received official letter from Toyota there weee no defects in their safety system. The issues are the airbags not being deployed at the time of impact and pre collision auto braking non existent. The impact on the wall according to the people inside the nursing she works felt like an earthquake inside. But our main concern is the auto brake feature did not function. This accident could have been worse. Good thing seatbelt was not an inch lower from her chest preventing injury to our unborn child. The impact could have been worse if it was made of wood and could have injured other people in the nursing home. The auto brake feature should have worked because there were many obstacles at her front including first the bushes and eventually the wall. Their response letter is frustrating for us because they are claiming there were no problems in the car. But our concern is their safety system is not reliable compare to other car companies. Kia and Honda safety system is more reactive based from my driving experience. I think there are lapses in their system.

25V744000 · Back Over Prevention:display Function

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Consequence & remedy

Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.

Consequence: A damaged brake line can cause a brake fluid leak, reducing brake performance and increasing the distance required to stop the vehicle. Wheel detachment can result in a loss of vehicle control. Either condition increases the risk of a crash.

Remedy: Dealers will inspect and tighten the front brake caliper mounting bracket and hub bearing bolts, as necessary, free of charge. In addition, any damaged components will be replaced, free of charge. Owner notification letters were mailed January 23, 2025. Owners may contact Toyota customer service at 1-800-331-4331.
