The contact owned a 2008 Toyota Highlander. The contact stated that while driving at approximately 20 MPH, an abnormal, booming sound was emitted from the vehicle as heavy, black smoke entered her vehicle. Due to the smoke, the contact's vision became obstructed, and the contact immediately shifted into park and jumped out of her vehicle for safety. It was unknown if there were any warning lights. As a result, the contact injured her right hip and strained her vocal cords. The vehicle quickly became engulfed in flames as authorities were called to the scene. The fire department extinguished the flames as both a fire and a police report were filed. The vehicle was towed to an independent mechanic, where it was deemed a total loss. There was no property damage. The origin of the fire was unknown, and an investigation was opened by the insurance company. The contact sought medical attention the next day and was treated for the strain on her right hip. The manufacturer was notified of the failure, and the contact was given a case number. The contact was also informed that she would receive a response from them within 45 days. The vehicle was destroyed. The failure mileage was approximately 100,000.

The engine burns Coolant and yet doesn't overheat. The engine on cyl 4 misfires. Changing the head gasket didn't repair the issue, replacing all fuel Injectors and ignition parts didn't either. On January 19, 2021, after driving for about two hours, I suddenly started to hear an unusual knocking noise from the engine. I slowed down, but I was unable to pull over immediately because I was driving on a very dark two lane highway, with no safe shoulder. After the same noise continued for about five to ten minutes, a light began to flash indicating extremely low oil pressure, and warning that the car should stop immediately. At that point, fortunately, I was able to pull over at a gas station, where I discovered that oil was gushing from my engine onto the ground. I checked my oil dipstick and noted that the car had no oil, and was thus inoperable. I was extremely concerned that the engine had been damaged while being driven with little to no oil. In 2014, Toyota issued a notice of a Warranty Enhancement Program – ZE2, related to extending warranty coverage for repairs related to a leaking or seeping engine oil cooler pipe. I never received any owner notification of this issue in 2014. Toyota refused to pay for the repair in 2021, and now in November 2021, the engine has quit due to the damage incurred in January 2021. The engine oil cooler pipe is defective and my family could have been seriously injured or killed by its catastrophic failure.

On September 25, 2021, my husband was driving our 2008 Highlander locally to the store. The low pressure light came on and said pull over immediately. He pulled over and and checked the oil dip stick and our car was totally out of oil. He also notice oil under our car and a line of oil going up the street. The following day, I contacted Toyota Experience Center and was informed their were no service campaigns on my Highlander. I had the car towed to my local mechanic and he verified that the car had no oil and replaced the failed oil cooler hose. My concern is that the oil cooler hose failed, draining all of the oil in minutes. I feel this is a safety issue because of how rapidly it occurred and If we couldn't immediately stop the car our engine could have seized up and made the car stop.

I THEN DISCOVERED THAT THE ENTIRE UNDERCARRIAGE OF THE VEHICLE AS WELL AS THE BACK HATCH OF THE VEHICLE WERE COVERED WITH OIL. MY DAUGHTER AND I ARE VERY FORTUNATE THAT THE OIL THAT COVERED THE VEHICLE DID NOT CATCH FIRE AND AS FAR AS I KNOW THE OIL SLICK ON THE HIGHWAY DID NOT CAUSE ANY ACCIDENTS. I AM NOW BEING TOLD BY TOYOTA THAT THIS IS A KNOWN ISSUE, BUT IT IS NOT LISTED AS A RECALL. NOT ONLY DID THEIR KNOWN ENGINEERING ISSUE PUT MY FAMILY AND OTHER FAMILIES' LIVES IN DANGER, THEY ARE ALSO NOT WILLING TO PAY FOR THE REPAIRS OF THE DAMAGE TO THE VEHICLE THAT THEIR KNOWN DEFECT CAUSED. THERE IS A "WARRANTY ENHANCEMENT" THAT WAS ISSUED BY TOYOTA FOR THIS DEFECT. I HAVE ALSO HAD OTHER WORK PERFORMED AT THE DEALERSHIP SINCE THEN AND WAS NEVER MADE AWARE OF THE DEFECT OR WARRANTY ENHANCEMENT. THANKFULLY THE CAR CONTINUED TO ROLL AND I WAS ABLE TO PULL THE CAR TO THE SIDE OF THE ROAD AND TURN ON THE EMERGENCY BLINKERS AND CALLED FOR ASSISTANCE. WHEN FINALLY WE GOT THE CAR TOWED TO THE NEAREST TOYOTA DEALERSHIP WE WERE INFORMED THAT THE OIL COOLANT LINE HAD "BLOWN" AND OUR ENGINE HAD SEIZED FROM A LOSS OF ENGINE OIL. TWO WEEKS BEFORE THIS INCIDENT THE CAR ALSO DISPLAYED THE VSC WARNING SO WE HAD THE CAR TOWED TO BEAMAN TOYOTA WHERE WE PAID 1818.60 TO HAVE, AMONG OTHER THINGS, THE WATER PUMP AND DRIVE BELT REPLACED. TOYOTA KNEW THAT THE RUBBER HOSE WILL FAIL ON ALL VEHICLES DUE TO THE HEAT/COOL CYCLE BY ITS CLOSE PROXIMITY TO THE CATALYTIC CONVERTER (ABOUT 6 INCHES AWAY) SO THEY REDESIGNED THE OIL COOLER LINES TO BE ALL METAL. Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Consequence & remedy
Consequence: If damaged, the copper strand heating element may short circuit, increasing the risk of a fire.
Remedy: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.
Consequence & remedy
Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.
Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.

The affected vehicles may experience compression of the seat cushion which may damage the seat heater wiring.
Consequence & remedy
Consequence: Damage to the seat heater wiring could cause the wires to short, increasing the risk of the seat burning and causing personal injury to the occupant.

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.
Consequence & remedy
Consequence: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.
Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. General Motors sent notices to the Pontiac Vibe owners on September 15, 2014, that a remedy is available. The recall began on October 27, 2014. Owners may contact Toyota at 1-800-331-4331. Owners may contact General Motors at 1-800-762-2737.

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.
Consequence & remedy
Consequence: If the switch overheats, it may melt, possibly resulting in a fire.
Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. THE NON-DRIVER DESIGNATED SEATING POSITIONS (DSP) ON THESE VEHICLES IS EQUIPPED WITH SEAT BELT ASSEMBLIES THAT HAVE A FEATURE THAT CONVERTS THE RETRACTOR FROM AN EMERGENCY LOCKING RETRACTOR (ELR) TO AN AUTOMATIC LOCKING RETRACTOR (ALR). THE BELTS CONVERT TO THE ALR MODE AFTER THE WEBBING HAS BEEN FULLY WITHDRAWN FROM THE RETRACTOR. THE SYSTEM IS DESIGNED SO THAT AS THE BELT IS RELEASED INTO THE RETRACTOR, THE SYSTEM REMAINS IN THE ALR MODE SO THAT A CHILD RESTRAINT SYSTEM (CRS) CAN BE TIGHTLY SECURED.

Consequence: HOWEVER, FOR THE TWO THIRD ROW DSP'S ON THESE VEHICLES, THE ALR MODE DEACTIVATES BEFORE THE WEBBING IS FULLY RETRACTED, SO THAT IT WILL NOT LOCK WHEN CERTAIN REAR-FACING CRS'S THAT HAVE A SHORT SEAT BELT ROUTING PATH ARE INSTALLED IN THE VEHICLE. THIS CAN ALLOW THE SEAT BELT WEBBING TO SPOOL OUT DURING NORMAL DRIVING MANEUVERS, WHICH COULD AFFECT THE STABILITY OF A CHILD RESTRAINT INSTALLED AT THOSE SEATING POSITIONS. AS SUCH, THIS COULD DISCOURAGE THE USE OF A CHILD RESTRAINT AND IN THE EVENT OF A CRASH, THE CHILD MAY NOT BE ADEQUATELY PROTECTED POSSIBLY RESULTING IN INJURY.

Ten (10) additional incidents of steering column separation were identified. Four (4) of these instances occurred while driving. During PE18-005, ODI confirmed that the steering columns in the subject vehicles are equipped with breakaway capsules that are designed to collapse/shear during certain types of vehicle collisions. This design feature aims to reduce driver injury severity from impact with the steering wheel in frontal vehicle collisions. There is no direct method for detecting when the capsules in the subject vehicles have sheared. If the driver operates the tilt/telescoping feature after the breakaway capsules have separated, the steering column may drop out of position and could subsequently detach entirely from the rest of the steering assembly. Loss of steering while driving may cause a loss of vehicle control, which could lead to a vehicle crash.To date, ODI has identified 14 incidents involving complete steering column separation, 6 of which occurred while driving. One of these incidents involved a minor crash. Additionally, there have been 16 events where the steering column dropped from the dashboard mounting brackets, but column separation did not occur. Four of these incidents happened while driving.
