The contact owns a 2010 Toyota Corolla. The contact stated that while driving at an undisclosed speed, the vehicle unexpectedly lunged forward. The brake pedal was depressed; however, the vehicle kept rolling forward, and the contact was forced to use the parking brake to stop the vehicle. The contact associated the failure with NHTSA Campaign Number: 10V017000 (Vehicle Speed Control); however, the VIN was not included in the recall. The dealer was contacted, but the contact was unsure whether the dealer had diagnosed the vehicle. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was 139,457.

While driving at highway speeds, the vehicle suffered a sudden, simultaneous failure of multiple electrical and safety systems. Without warning, the Check Engine light, VSC (Vehicle Stability Control) light, Traction Control light, and SRS Airbag light all illuminated simultaneously on the dashboard. Immediately upon illumination of these fault lights, the vehicle lost computer-assisted stability control, began swerving violently, spun out of control, and collided with another vehicle, suffering severe front and rear impact damage. The vehicle was declared a total loss. Despite severe impact forces to both the front and rear structure, the SRS airbags completely failed to deploy. Both occupants sustained injuries due to the impact and lack of airbag protection. The simultaneous illumination of the airbag and VSC lights prior to impact strongly points to a sudden failure of the steering column spiral cable (clock spring) assembly—a known safety defect under NHTSA Recall Campaign 14V-168 (Recall E0M). The illumination of the airbag fault light appears to have deactivated the SRS system prior to impact, leaving occupants unprotected during a major collision. The vehicle is currently held at IAA San Diego and contains unread Event Data Recorder (EDR) crash data proving the pre-impact system shutdown. I request that NHTSA review this safety system failure and airbag non-deployment. CHP Crash Report #[9645-2026-02967] and Insurance Total Loss Evaluation are available upon request.

My 2010 Toyota Corolla is experiencing progressive paint failure due to a known manufacturer defect involving factory-applied Blizzard Pearl or Super White paint. The affected component is the factory paint system, specifically the adhesion between the topcoat and primer, which is failing and causing the paint to peel off in sheets from exterior body panels. The defective paint is available for inspection upon request. This condition creates a potential safety concern, as sections of paint may detach while the vehicle is in motion, creating debris that could pose a hazard to surrounding motorists. In addition, exposed underlying surfaces may become more susceptible to corrosion over time, which could affect the integrity of exterior body panels. The issue was previously confirmed by an authorized dealership during inspections conducted under the manufacturer’s Customer Support Program for paint peeling on vehicles equipped with Blizzard Pearl or Super White paint. During the active coverage period, only certain visibly affected panels were refinished, while other factory-painted panels were left untreated. Since the program has expired, the previously untreated panels are now exhibiting the same paint peeling and failure that the program was intended to address. The paint deterioration developed gradually over time and was not the result of external damage. No warning lamps or messages were present prior to the failure.

In 2018, my 2010 Toyota Corolla was taken to the dealership for the Takata Airbag Recall. After the repair the airbag warning light would intermittently display and go away only to come back later. The light was never consistent and was always randomly on so in turn could be off some times during the start up of the vehicle. In 2021, I took the vehicle to the dealership in which at the time the light was still intermittent. The dealership did not present any diagnostic and instead had reset the code during that time. The light would continue to still intermittently show afterwards. Currently the airbag warning light has been solid. Today 1/9/2026, the dealership has indicated they would not remediate this situation under workmanship warranty work despite previously resetting the code. The dealership insists the customer should pay for this remediation work. I have currently declined the work to seek external advise. The dealership work invoice states the code B1806 for the front passenger side squib circuit, this continued error puts passengers at risk in a car accident. Upon request of the previous dealership service history, the customer noted that there is a missing gap for service records on the dealership side. Most importantly the customer stated concern for the 2021 requested remediation was not documented. These appointments are still visible via the customer's service appointment reminder emails.

These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Consequence & remedy
Consequence: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
Remedy: Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Consequence & remedy
Consequence: If damaged, the copper strand heating element may short circuit, increasing the risk of a fire.
Remedy: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.
Consequence & remedy
Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.
Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. The affected vehicles may experience compression of the seat cushion which may damage the seat heater wiring.
Consequence & remedy
Consequence: Damage to the seat heater wiring could cause the wires to short, increasing the risk of the seat burning and causing personal injury to the occupant.
Remedy: SET will notify owners, and dealers will disconnect the heaters with copper strand heating elements and refund the purchase price of the seat heater accessory, free of charge. The recall is expected to began in January 2015. Owners may contact SET customer service at 1-866-405-4226.

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.
Consequence & remedy
Consequence: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.
Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."

Consequence: An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). On February 18, 2010, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE10-008 to investigate complaints of steering wander or drift in model year (MY) 2009 and 2010 Toyota Corolla and Matrix vehicles.The complaint descriptions indicated dissatisfaction with the on-center steering feel in the subject vehicles. Some drivers (~25%) related the feel to driving in a strong crosswind and others indicated that the effort and attention required to maintain heading increased driver fatigue on long trips.These complaints all relate to the system operating as designed and do not involve failure of any steering system components.The subject vehicles are equipped with an electronic power steering system that was introduced in the MY 2009 vehicles.The power assist, including on-center steering feel, is controlled electronically and is part of the system calibration.Toyota responded to steering feel concerns from some customers by issuing a technical service bulletin (TSB-0140-10 STEERING ON-CENTER FEEL) that released an optional EPS electronic control unit that has a re-calibrated on-center feel.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency will continue to monitor complaints and other information relating to the alleged defect in the subject vehicles and take further action in the future if warranted by the circumstances.For further details see the attached VRTC and ODI Closing Reports.
