I MANAGED TO BRING THE CAR TO TOYOTA DEALER SERVICES ON THAT DAY. THEY COULD NOT FIND ANYTHING WRONG. THEY RECORDED THAT C1202 STORED IN ECU, LOW BRAKE FLUID WARNING, ABS PUMP MOTOR OPERATION SUSPE TOPPED OFF BRAKE FLUID. I STARTED HEARING A CREAKING NOISE IN MARCH 2014 WHEN I STEPPED ON THE BRAKE. I BROUGHT THE CAR TO THE TOYOTA DEALER SERVICES ON APRIL 2, 2014. INITIALLY THEY COULD NOT FIND ANYTHING WRONG, HOWEVER, I INSISTED ON TESTING IT FURTHER. EVENTUALLY, THEY FOUND THAT THERE WAS A PROBLEM WITH AN ACTUATOR IN THE BRAKING SYSTEM. I HAD TO REPLACE THE ACTUATOR IN THE BRAKING SYSTEM AND WAS NOT COVERED BY A WARRANTY. I HAD TO PAY OVER $3300 INCLUDING TAX. AFTER UNDERSTANDING HOW AN ACTUATOR WORKS IN BRAKING SYSTEM, I BELIEVE THAT THE INCIDENT ON NOVEMBER 25, 2013 MUST BE CAUSED BY AN ACTUATOR ISSUE. I URGE TOYOTA TO RECALL THE ACTUATOR IN BRAKING SYSTEM FOR THE FOLLOWING REASONS. FIRST, EVEN TRAINED TECHNICIANS AT TOYOTA DEALER COULD NOT FIND AN ISSUE EASILY. THEY FINALLY FOUND IT AFTER THEY FAILED TWICE IN OUR CASE (IT TOOK NEARLY TWO DAYS). THOSE TECHNICIANS SHOULD BE INFORMED, SO THEY CAN DEAL WITH CUSTOMERS LIKE US QUICKLY AND PROPERLY. SECOND, A SIGN OF ACTUATOR PROBLEM CAN BE EASILY NEGLECTED. A CREAKING NOISE, WHICH CAUSED US TO BRING A CAR TO THE DEALER WAS PRETTY SUBTLE. THIRD A BRAKE FAILURE MAY LEAD FATAL CAR ACCIDENTS. I WAS VERY SCARED WHEN THE BRAKE ALMOST FAILED TO WORK. THIS MAY BE A LUCKY CASE SINCE I DID NOT HAVE A CAR ACCIDENT. *TT

APRIL 5, 2014' BRAKE ACTUATOR MALFUNCTION. THE VISUAL OF THIS PROBLEM OCCURRED ON THREE SEPARATE OCCASIONS WHILE DRIVING AT VARYING SPEEDS. ON EACH OCCASION THE EMERGENCY BRAKE LIGHT CAME ON AND WITHIN A FEW MINUTES MANY OF THE WARNING/EMERGENCY LIGHTS (ENGINE CHECK, ABS, VCS, BRAKE, ETC.) ON THE DASHBOARD CAME ON. THE BRAKES THEN BECAME VERY SPONGY TO THE POINT I HAD TO PRESS VERY HARD TO GET THE CAMRY TO STOP. IT COULD HAVE BEEN DISASTROUS IF I HAD HAD TO STOP TO AVOID A CRASH. AFTER THE FIRST OCCURRENCE, I TOOK THE CAMRY TO TEJAS TOYOTA TO HAVE IT CHECKED. THE DIAGNOSTIC INFORMATION FROM THE COMPUTER INDICATED NOTHING WAS WRONG. I WAS INSTRUCTED BY THE TOYOTA SERVICE MANAGER TO BRING IT IN WHILE IT WAS HAPPENING, IF POSSIBLE, SO THEY COULD SEE WHAT WAS GOING ON. ON APRIL 5TH, THE PROBLEM OCCURRED AGAIN AND WE DROVE IT DIRECTLY TO TEJAS. THE PROBLEM WAS STILL OCCURRING WHEN THEY CONNECTED THEIR COMPUTER TO CHECK THE VEHICLES ONBOARD COMPUTER. THEIR COMPUTER DIAGNOSED THE BRAKE ACTUATOR ASSEMBLY WAS FAULTY AND NEEDED TO BE REPLACED, AND DUE TO THE FAULTY ACTUATOR, THE BRAKE PADS ON THE BOTH FRONT AND REAR HAD TO BE REPLACED AND THE BRAKING SYSTEM FLUSHED. THE COST OF THE ACTUATOR WITH LABOR TO INSTALL WAS OVER $3,500, TOTAL BILL ALMOST $4,200. TEJAS PERSONNEL DID A GREAT JOB REPAIRING THE PROBLEM AND GETTING THE VEHICLE BACK TO ME. I CALLED THE TOYOTA MAIN CUSTOMER SERVICE NUMBER AND SPOKE WITH ONE OF THEIR REPRESENTATIVES, [XXX]. HE CREATED CASE # 1404141818 AND I WAS INSTRUCTED TO FAX (NOT EMAIL BUT FAX) A COPY OF THE INVOICE I RECEIVED FROM TEJAS TO HIM. WHICH I DID. HE SPOKE TO THE HIGHER UPS IN TOYOTA AND CALLED ME BACK TODAY (4/16). TOYOTA MANAGEMENT DENIED IT WAS THEIR RESPONSIBILITY TO PAY THE REPAIR COSTS BECAUSE THE VEHICLE WAS PAST THE INITIAL WARRANTY PERIOD. I BELIEVE TOYOTA IS RESPONSIBLE DUE TO FAULTY DESIGN OF THEIR ACTUATOR. HE TOLD ME ITS NOT THE FIRST TIME A CUSTOMER HAS CAME IN WITH THIS PROBLEM ON THE 2007 AND 2008. I GUESS THEY CHANGE SOMETHING ON ABS PUMPS ON THE 2009 AND NEWER VEHICLES THAT'S WHY THEY DON'T HAVE THIS PROBLEM. I JUST HOPE THE NHTSA LOOKS INTO THIS DEFECT PART BEFORE SOMEBODY GETS HURT OR KILL. ITS A SERIOUS PROBLEM ESPECIALLY WHEN YOUR DEALING WITH VEHICLE BRAKES NOT FUNCTIONING THE WAY THERE SUPPOSE TO THAT WE ALL DEPEND ON. THE REASON WHY TOYOTA HAVEN'T DONE A VOLUNTARY RECALL IS BECAUSE OF THE COST OF THE ABS ACTUATOR IT'S EXPANSIVE AT AROUND 2,500 TO 3,000 THEY KNOW THEY HAVE A PROBLEM IN THERE HANDS. Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Consequence & remedy

Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation.After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first.In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles.The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims.The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness.In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding that a safety-related defect does not exist.For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
