STARTED THE CAR AND THE "BRAKE" WARNING LIGHT IMMEDIATELY CAME ON. ALSO, IN THE MULTI-INFORMATION DISPLAY AT THE CENTER OF THE SPEEDOMETER, THE MESSAGE "CHECK VSC SYSTEM" APPEARED. I THOUGHT MAYBE MY PARKING BRAKE WAS CAUSING A PROBLEM, SO I ENGAGED AND DISENGAGED THAT. THEN I TRIED TO DRIVE THE CAR. THE BRAKE PEDAL BEHAVES AS IF THERE IS NO BRAKE-ASSIST. I JAM IT TO THE FLOOR TO GET MINIMAL BRAKING. I DROVE SLOWLY AND CAREFULLY HOME IN THIS CONDITION. IN SUBSEQUENT TRIPS (OVER THE LAST TWO DAYS) I HAVE ONLY SEEN THIS AGAIN ONE MORE TIME. WHEN THE SYSTEM DOES NOT REPORT THE PROBLEM, THE BRAKES WORK FINE. AND IN THAT ONE CASE WHERE THE PROBLEM RE-APPEARED, I WAS ABLE TO TURN THE CAR OFF AND BACK ON AGAIN A FEW TIMES, UNTIL THE PROBLEM DISAPPEARED. I HAVE NOT SEEN THE PROBLEM BEGIN WHILE I AM DRIVING -- ONLY WHEN I AM STARTING THE CAR. HOWEVER, I'VE NOW SEEN OTHERS DESCRIBE THE PROBLEM AS BEGINNING DURING DRIVING, SO I WILL BE EXTREMELY CAREFUL AND GET THE CAR TO THE DEALER ASAP. *JS

WE WERE ON A TRIP FROM NY TO ATLANTA, GA LAST THANKSGIVING AND SOMEWHERE IN NORTH CAROLINA, THE CAR EMITTED A WARNING PING, AND A NUMBER OF LIGHTS ON THE DASH, INCLUDING BRAKES, CHECK ENGINE, VCS AND A YELLOW TRIANGLE, ILLUMINATED. A MESSAGE ON THE DISPLAY READ "CHECK ABS". AT THE SAME TIME THE BRAKES BECAME EXTREMELY DIFFICULT TO ACTUATE AND I FELT AS THOUGH I HAD TO PUT ALL MY EFFORT INTO ACTIVATING THE CAR'S BRAKES. WE PULLED INTO A REST STOP AND AS WE BEGAN TO SLOW, THE LIGHTS WENT OUT. JUST BEFORE WE GOT TO A PARKING SPOT HOWEVER, THE SAME THING OCCURRED. WE LET THE CAR SIT FOR A WHILE BEFORE ATTEMPTING TO MOVE TO TURN IT ON AGAIN, AT WHICH POINT, THINGS SEEMED NORMAL. IT HAPPENED ONCE OR TWICE AGAIN ON THE TRIP THERE, BUT DIDN'T OCCUR AT ALL ON THE WAY BACK NOR THROUGHOUT 2014... UNTIL NOW. RECENTLY THE SAME THING HAS BEGUN HAPPENING WITH MORE REGULARITY. WE TOOK THE CAR IN FOR A DIAGNOSIS TODAY. WHILE RESEARCHING THE ISSUE, I NOTICED THAT THIS HAS BEEN REPORTED PREVIOUSLY, SO I DECIDED TO ADD MY VOICE TO THE MASSES. *TR

I WAS DRIVING ABOUT 35 MPH WHEN ALL OF MY BRAKE, SLIP INDICATOR AND ABS WARNING LIGHTS CAME ON. I IMMEDIATELY PULLED OVER AND LOOKED IT UP IN MY OWNER'S MANUAL. THERE, IT TOLD ME TO TAKE THE CAR TO THE TOYOTA DEALER. I WAS 40 MILES FROM HOME SO I PROCEEDED TO GO BACK TO THE HOSPITAL TO PICK UP MY BOYFRIEND. I GOT LESS THAN A MILE DOWN THE ROAD WHEN THE MASTER WARNING LIGHT AND AN ALARM CAME ON. AGAIN I PULLED OVER AND TURNED OFF THE CAR. THE MANUAL TOLD ME THAT I NEEDED TO HAVE IT TOWED TO THE DEALER IMMEDIATELY. AT THE DEALERSHIP I FOUND THAT THERE WAS A "LIMITED SERVICE CAMPAIGN" ON A FILTER THAT CONNECTS THE CHAMBERS IN THE BRAKE RESERVOIR. IT WAS REPLACED AND I DIDN'T HAVE ANOTHER ISSUE UNTIL 11/26/14. AGAIN, ALL THE WARNING LIGHTS AND ALARM CAME ON AND I HAD TO HAVE IT TOWED TO THE DEALER. THERE, I WAS TOLD THAT THE BRAKE ACTUATOR HAD FAILED AND IT WOULD BE OVER $1500 TO REPLACE. LUCKILY, I DID SOME RESEARCH AND FOUND THIS IS ALSO BEING COVERED BY TOYOTA UNDER A "WARRANTY ENHANCEMENT" PROGRAM. *JS

"BRAKES" WARNING LIGHT AND "ABS" WARNING LIGHT WAS ILLUMINATED WITH OTHERS WHICH I DID NOT TAKE NOTE OF. I TURNED THE VEHICLE OFF AND CALLED FOR A TOW TO THE TOYOTA DEALER. DRIVING ON THE ROAD, EVERYTHING WAS NORMAL, THEN MY ENTIRE DASHBOARD LIGHTS UP WITH ALL THE WARNING SIGNS. THE INTERNAL CAR DISPLAYS VSC MAINTENANCE REQUIRED, AND I'M THINKING I DIDN'T DO ANYTHING. I JUST GOT THE CAR THIS YEAR. THERE SHOULDN'T BE A PROBLEM LIKE THIS. THE BRAKES WERE VERY HARD TO PRESS DOWN ON TO EVEN BRAKE. I FEE LIKE THE BRAKES JUST STOPPED WORKING OR THE BRAKE ITSELF WOULD LET ME BRAKE AS NORMAL. LITTLE SHOCKED BECAUSE I'D HAD CRASH INTO THE DRIVER IN FRONT OF ME. I STARTED TO DRIVE VERY SLOW. THEN THE LIGHTS AND WARNING SIGNS WENT AWAY, AND MY BRAKES WERE BACK TO NORMAL. I FELT LIKE IT WAS JUST A COMPUTER GLITCH AND STARTED TO DRIVE NORMALLY AGAIN. AFTER I DID, THE CAR AGAIN WENT OFF WITH THE WARNING SIGNS ON THE DASHBOARD AND THE VSC DISPLAY WAS ON AGAIN STATING MAINTENANCE WAS REQUIRED ONE AGAIN. SO I STARTED TO DRIVE SLOW ONCE AGAIN. WENT TO A REPAIR SHOP THEY SAID MY BRAKES WERE GOOD, AND THAT THE CAR PROBABLY NEEDED TO GET IT CHECK BECAUSE OF THE HYBRID SYSTEM. THIS ONLY HAPPEN DURING THE SUMMER. DURING THE WINTER IT GOES OFF, TO WHICH I STILL FELT LIKE IT WAS A COMPUTER GLITCH. WENT TO THE DEALER RECENTLY AND THEY SAID THAT MY ACCUMULATOR NEEDS TO BE REPLACE. I ASKED HOW DOES SOMETHING LIKE THIS OCCUR. THE MECHANIC STATED SOMEWHERE IN THE LINES OF IT'S A MAN MADE PART SO IT'S CAN HAPPEN FROM TIME TO TIME. BUT IN MY OPINION, IF I DID EVERYTHING LIKE I WAS SUPPOSE TO DO, THIS SHOULD NOT HAPPEN. THE TOTAL REPAIR FOR THE PART IS $1892! THAT'S WITH LABOR INCLUDED! SERIOUSLY, WHY DOES A PART THAT IS SO IMPORTANT TO THE CAR THAT EXPENSIVE?! AND WHY DOESN'T TOYOTA REPORT ON THIS TECHNICAL ISSUE FOR THEIR CARS? *TR

15V689000 · Visibility:power Window Devices And Controls

Oct 22, 2015

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Consequence & remedy

Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.

12V491000 · Visibility:power Window Devices And Controls

Oct 10, 2012

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

09V388000 · Vehicle Speed Control:accelerator Pedal

Oct 5, 2009

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation. After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first. In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles. The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims. The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness. In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident. This preliminary evaluation is closed. The closing of this investigation does not constitute a finding that a safety-related defect does not exist. For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
