A SHACKLE ON THE LEAF SPRING FAILED WHILE IN MOTION. FAILURE WAS FOUND DURING ROUTINE PHYSICAL INSPECTION. FURTHER FAILURE OF THE THE ATTACHMENT POINTS COULD HAVE CAUSED THE LOSS OF ONE OR MORE AXLES.

DRIVERS SIDE REAR LEAF SPRING HANGER SHEARED OFF FROM FRAME WHILE UNDERWAY. I HAD A WELDER TELL ME THAT THE WELDS LOOKED LIKE THEY HAD "UNDERCUT" .
