Subject: Steering Rack Failure Reimbursement Request, VIN [XXX] To Whom It May Concern, I am requesting review and reimbursement for an electronic steering rack replacement on my 2021 Ford truck, VIN [XXX]. The electronic steering fault first appeared at start-up and when turning the wheel to full lock. Initially, I could pull over, shut the truck off, restart it, and the warning would clear. After a battery replacement, battery monitoring system reset, and multiple steering relearns, the steering failed completely. When the failure occurred, steering became extremely difficult. After hitting a bump, the steering wheel jerked suddenly and caused pain to my hand and thumb. This also created a serious safety risk for me and others on the road due to the reduced ability to steer. The dealership replaced the electronic steering rack at a cost of over $2,000. The truck had only about 40,000 miles at the time. I later found Ford Recall 21S28, which appears to cover certain trucks built between XXX. Based on my VIN and build date, my truck appears to fall within that range, but the dealership advised my VIN was not included. Given the low mileage, safety risk, and apparent build-date match with Recall 21S28, I request that Ford and NHTSA review my vehicle’s eligibility under this recall or any related program and reimburse me for the repair. Upon speaking to AutoFair Ford of Haverhill service they informed me Ford did not include my truck in the recall despite it matching year and model and they would not be addressing the issue for me. The contact owns a 2021 Ford F-150. The contact stated that while parking the vehicle, the vehicle unexpectedly lost power steering functionality. The contact opened and closed the driver's side door to reset the vehicle, and the power steering function was restored; however, the failure recurred while driving. The vehicle was towed to an independent mechanic, where it was diagnosed with a PSCM internal failure and steering mechanism ball retainer failure. The mechanic determined that the failure was associated with NHTSA Campaign Number: 21V407000 (Steering) and referred the contact to the dealer for assistance. The vehicle was towed to the dealer, and the issue was confirmed after a diagnostic test, but the contact was informed that parts were not yet available. The vehicle was not repaired. The manufacturer was notified of the failure, and a case was filed. The failure mileage was approximately 79,000.

The EPAS (Electric Power Assisted Steering) module experienced an internal sensor failure, causing a sudden and complete loss of power steering assist. The vehicle is available for inspection upon request and the EPAS module has been retained. The loss of power steering occurred without warning while the vehicle was in motion, requiring significantly greater physical force to maintain control of the steering wheel. This creates a serious risk of losing vehicle control, particularly at higher speeds, during lane changes, or in emergency maneuver situations. Because the failure occurs randomly and without any prior indication, there is no opportunity to safely prepare or pull over before it happens, putting both the driver and other motorists at risk. Yes. The issue has been acknowledged by multiple Ford dealerships and independent mechanics, all of whom confirmed this is a known and recurring problem with this vehicle’s EPAS system. Numerous online automotive forums also document other owners experiencing the identical failure, suggesting this is a widespread issue rather than an isolated incident. The vehicle was inspected by an authorized Ford dealership, which confirmed the EPAS module failure and acknowledged familiarity with the issue. No warning lights, dashboard messages, unusual sounds, or handling changes occurred before the failure. The power steering loss happened suddenly and without any prior symptoms, leaving no opportunity to anticipate or react to the malfunction before it occurred.

After a day of heavy rain, the vehicle stalled at a stop light and would not restart. The steering wheel and transmission locked, preventing the vehicle from being placed in neutral, creating a safety hazard as the vehicle was immobile in traffic. No warning lamps appeared prior to the failure. The vehicle required a tow. The dealer confirmed water had intruded through the passenger side cowl/firewall seam due to inadequate factory sealing, tracking down and pooling directly onto the Body Control Module (BCM) mounted on the passenger side firewall. The BCM failed due to water damage, causing PATS desynchronization and the no-start condition. The BCM failure subsequently shorted out the starter motor. Dealer replaced the BCM, repaired the leak, and replaced the starter. Total repair cost approximately $3,200. Failed components are available for inspection at the dealership. This identical failure; passenger side cowl water intrusion leading directly to BCM damage — is documented across numerous 2021+ F-150 owner forums, indicating a widespread factory sealing defect at the cowl/firewall/inner fender seam junction.

The contact was informed that the rack and pinion needed to be replaced. The vehicle was not repaired. The contact was informed that the parts were in back order. The contact referenced an unknown recall; however, the VIN was not associated with a recall related to the failure. The manufacturer was not made aware of the failure. September 2025, we were on a vacation when we noticed that the column gear shifter on our 2021 Ford F150 seemed a bit "loose" when shifting. No issues getting into the appropriate gear…but a weird loose feeling. My husband pulled off the leather boot on the column (not sure if this is what it is called, I believe it keeps dust out) and found the problem. Two missing bolts! Earlier that year, I had found a bolt in the floor of the truck, and, unable to find where it came from (or did it even belong to the truck? I did not know) I put it in a ziploc bag, into the glove box. Turns out that bolt fit. So we had one bolt, and the shifting felt okay. But we still needed one bolt, so I did some internet sleuthing. I found the part (W790628S900) and ordered it, and we installed it when we got home. We theorize that the “ziploc bolt” came loose early in 2025, and the second bolt (never found) came loose on our trip. We know that up until this problem, we had never, ever, removed that dust cover, so we were not responsible for the two bolts coming loose. Components: that "failed" would be the two bolts on the column gear shifter (under the dust cover) Safety: we were very worried that without the two bolts, we would not be able to shift at some point! Confirmed: no, because we were camping on vacation, and did the troubleshooting ourselves Inspected: we will ask our mechanic when we next take the truck in Warning: no, just that we noticed that the column gear shifter was "loose" (again, likely that this was after the 2nd bolt came out).

A software error may cause the rearview camera image to be obstructed by the infotainment home screen menu when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: An obstructed rearview camera image can reduce the driver's rear visibility, increasing the risk of a crash.
Remedy: The accessory protocol interface module (APIM) software will be updated by a dealer or through an over-the-air (OTA) update, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 7, 2026. Additional letters will be sent once the remedy is available, anticipated in September 2026. Consequence & remedy
Consequence: An engine stall increases the risk of a crash.
Remedy: Dealers will replace the low pressure fuel pump on super duty vehicles and update the powertrain control module (PCM) software on all other affected vehicles, free of charge. On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles.
