With the cruise control set to a speed of around 60mph and with no other vehicles around, the truck accelerates (uncommanded) to some value over 71mph. This occurs randomly but is repeatable and has only started occurring within the past week. A definite safety concern.

When traveling on a divided highway in the left lane with no vehicle ahead of me, as I am passing a vehicle in the right lane, just when I am about even with that vehicle beside me my truck applies the brakes firmly and unexpectedly. There is no associated warning message on the instrument panel display or audio alert like there is when it goes into collision avoidance. When it happened the first time I was very startled and worried it saw something that required braking but as it happens almost every time I pass someone I am getting used to it.

On certain occasions, the transmission will make a banging noise while causing the truck to jerk forward when the vehicle has to upshift such as taking off from an intersection but rarely jerks when the vehicle is downshifting. Also, while using the adaptive cruise control system, the vehicle will travel close to the right or middle lane as if it is looking for the pavement striping and then move back over to the center of the lane. With the adaptive cruise system, when the vehicle is supposed to be in the center of the lane, the system slightly turns the the wheel as if it is having trouble staying in the center of the lane. The vehicle also has a CSP for unintended tailgate opening. i have an appointment set for November 18th but have been trying to get in touch with the service advisor that was assigned to me multiple times and was told he would receive an email to contact me each time but he has not contacted me. The shifting/jerking problem has occurred only three times and the issue with the adaptive cruise system is every time it is turned on while traveling a multi lane highway that has a broken striped line on both sides of the vehicle. Since this is my only vehicle, if the vehicle has to be kept at the dealer, I would need a loaner vehicle to travel to and from work while my vehicle is being repaired. I only use my truck on paved roads. Date is most recent incident of transmission issue.

I was driving with my wife and our one month old baby on I-25 in Albuquerque on a hot day (truck screen indicated 106 F) when suddenly the AdvanceTrac traction control, forward pre-collision braking, adaptive cruise control, and ABS system warnings all came on the dashboard. At the same time the brake pedal went limp (no braking action for the first ~50% of the pedal press). I slowed the truck by coasting for over a mile and picked an off-ramp that I expected to be empty before getting off the highway. Upon exiting the highway, I pressed the brake pedal all the way down and at 90-95% of the pedal compression the brakes finally engaged but locked up the rear wheels leading us to skip and skid across two lanes. We were able to come to a stop in the middle of the ramp and then gently moved over to the side of the exit, again locking up the rear wheels when we came to a stop. Upon turning the truck off and turning it back on again everything went back to normal suggesting that it was some subsystem that caused the electronic brake booster and ABS to become unresponsive. After a week at the local Ford dealer, the service technicians were able to find the error code C05CF. This error code appears to be officially only triggered by "Brake Master Cylinder Piston Position Sensor "B" -This DTC sets due to incomplete or improper module programming procedures." Which suggests this problem may have been lingering for over 30k miles of use. Ford technicians updated the traction control module firmware, body control module firmware, and then reproduced the issue leading them to perform an ABS system calibration which they claim fixed the issue. It is claimed that no component replacement was necessary, only software updates and calibration. Full dealer notes attached as .pdf It's crazy to think a software bug caused our brake system to almost entirely fail on the highway, particularly with an error code that should only have been able to been generated at the factory.

On June 17, 2022 while driving to work on highway in Nee York Stare, I put the adaptive cruise control on so I could adjust my drivers side rear view mirror. The truck suddenly lunged to the left striking a guard rail and overturned several times. Despite being a direct high speed impact to the guard rail and overturning , the steering airbag did not deploy. See police report attached. Due to this , I sustained major internal tissue damage including: a broken sternum, punctured lung bruised aorta, 4 fractures to the vertebrae in both my upper and lower spine and 4 broken ribs in my upper rib cage. I spent a total of 11 nights in the hospital including 4 nights in the ICU. I have not been cleared to return to work and instill under medical care. I understand there was a airbag/steering column recall on other F150s but I strongly suspect that the reveal needs to be expanded to mite vehicles such as mine. All the Doctors that treated me said my injuries were the direct result of the steering wheel airbag not deploying. The state trooper that reined me from my vehicle , as well as one of the witnesses both told me that my steering airbag did not deeply. I also have pictures as well.

The dealer is Sill-Terhar Motors in Broomfield, CO. 4. The passenger and drivers side rear modules were replaced by the dealership, but the issues still persist. 5. The system failure messages did appear at the time of failure. They started appearing approximately a week after we purchased the truck. First warning is logged in the FordPass app on June 13, 2021 I get three warnings pretty consistently when the system fails. So far the FordPass app has over100 error messages regarding my issue. The warning messages all show up at the same time. They are usually: 1. Blind Spot System Fault 2. Cross Traffic System Fault 3. Reverse Brake Assist Not Available See Manual (even though it IS turned on) 4. Hill Start Assist Error occurs as well 5. Sometimes when I am in reverse and these go off and I then brake with the foot pedal, the electronic emergency brake engaged without any manual intervention. First 3 errors have happened while reversing AND while driving in normal city street and highway speeds. Taking into the dealer to have it looked at again on Nov. 4, 2021. Issues started 06/13/2021 and continue to present (as of this writing 10/25/2021).

EA23002 · Loss Of Motive Power

Opened Sep 29, 2023 · Closed Oct 31, 2024

Status: closed (inferred from source dates) · Engine

On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family.  If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. In view of the recall action being taken by Ford, ODI is closing this Engineering Analysis. To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.
