I have been contacted multiple times by dealerships and Chrysler that my vehicle needs the Z11 (NHTSA 22V-077) recall performed and that it is urgent for me and my family's safety. I have been trying to get the Z11 recall performed since February 2023. I have contacted 3 local dealerships to do the job but scheduling was difficult and there is no loaner cars available. The first dealer (Autonation CDJR of Katy Texas) who has done all my previous recalls just wanted me to bring the car in to sit for 3 days while not performing the recall. The second was inexperienced and unqualified. After a week delay, I took my car into the third dealer (Big Star Dodge in Webster Texas) on 3/13/2023, but after two days they told me they were having trouble with equipment and gave me the vehicle back on 3/15/2023. They rescheduled for 3/27/2023 and I took it in again. The first attempt to perform the recall procedure failed as a mistake was made. The recall procedure should take 2 days but it has been there for a week. The recall procedure was written poorly by Chrysler/FCA/Stellantis and confuses the technicians so the test is not performed as it should be and has to be repeated multiple times. The extra manipulation and time at the dealership increases the probability that my vehicle will be messed up or damaged. Chrysler/FCA/Stellantis bears the responsibility for the delays and poor performance of the recall causing the unnecessary loss of my vehicle's use and delays the recall performance, increasing my risk described by the recall. Performance of the recall has not been timely or done correctly yet.

I was driving with my family about 30mph and for no reason, the vehicle lost power and I coasted and braked to a stop on the side of the road. Nothing I did would bring any response from the vehicle while it was coasting. A short time later while stopped, I was able to turn the vehicle off and then was able to turn it back on. This happened about a year ago and I thought it was strange. It has not re-occurred. I did not seek out any resolution until I found out about the 03A recall (NHTSA 23V-010). That recall describes what happened to me. My vehicle is currently undergoing multiple attempts to perform the Z11 recall (NHTSA 22V-077) so I do not currently have access to my vehicle.

I paid 9k for a new unit 9/30/21 and after 6 bmonths began to hear the same noise. I notified and dropped off vehical on 3 seperate occasions the last 12/30/22 and was told (normal operation) The transmission failed in exact same way on 2/2/23 with only 25k miles transmission and 182k on vehicle and currently problem is not resovled as the chrysler dealer I towed it to refused to repair under warrenty and Ihad to two to croton pouint auto park chrysler dealer where it currently is and problem is as of yet not resolved. Leading up to the failure the vehicle would roll in excess of 4 inches on inclines before park engaged (unless steep enough auto brake engaged) I have a feeling that this problem is widespread (chrysler has the transmission on restriction) and would like to know if other people are having this issue as it is quite dangerous seeing as the car could roll loose from park.

Pacifica Hybrid Limited found completely dead in our driveway and unresponsive to the remote. I manually unlocked the driver door, no usual dash indications, opened the hood and found the 12v battery system at 2.9v. It would not start & no responses or indications. There had been no warnings or indications of any problems prior to this. The car prevented my 6 A charger from charging the battery while it was connected to the car. I jump started the car with a known good battery, removed the jump, and allowed the engine to charge the car battery some. I decided to take no chances and replace the 12v battery. I turned the car to the OFF state via the dash push button. The car was never moved out of P park. I left it to go purchase a new 12v battery. I returned 2 hours later with a new battery to find the car in the street and having knocked over my neighbors 700-800 lb brick mail box. A police report was filed, pictures taken, and I jump started the car with the new battery. It started normally and I backed it up under its own power into my driveway where I replaced the 12v battery without incident. The car has operated normally with no warnings or indications of a problem before and since the incident. The car on its own and while OFF, unlocked the transmission park pawl allowing the car to roll 80 feet down a very mild slope and across the street to crash into my neighbor’s mail box when the 12v battery failed. There was no occupant, and no key fob was present. My quick search has found at least 3 other incidents with this same model, where the 12v battery failed and the car, on its own, unlocked the transmission allowing it to roll away. Of the 4 incidents I am aware of, 3 resulted in collisions and property damage. 2108 Hybrid Pacifica stalled a couple of times on freeway on our recent trip . Here are the details : About 2 hours after driving on freeway. - then, dashboard showed few errors and eventually showing "service charging system" - then displaying "shift to P to start" and "D" symbol is flashing. - after this, pacifica van lost power ( pushing accelerating do nothing ) but steering, brake seems OK and I had to steer the car to freeway shoulder and then turn off the car and start again after a few minutes and can start the car again but "engine check light" is on for at least 24 hours after. - After starting again, then a half a hour later, it happened again with same symptoms as above. The car has about 40,000 miles and running in gas mode only. These are very dangerous situation and luckily this time, I had time to steer the Pacifica to a complete stop.

While traveling at 75+ mph on a multi-lane highway the vehicle shut off displaying warnings "Shift to P to Start" "Service Charging System" "Sliding Door Open" "Forward Collision Warning Unavailable" and large red turtle icons. With the van shut off the only option available was to coast to side of the road/shoulder while trying to avoid other high speed vehicles and while the other vehicles attempted to dodge me. The vehicle has been at the dealer 5 times. Various codes are always available but they are unable to recreate the problem. The first visit the 12v battery was replaced. The most recent visit (the van is still at the dealer) the dealership and the manufacturer tore apart the van and checked parts for 5 weeks and could not identify anything wrong. I have pictures and lists of conditions, codes and problems. There is no warning when this happens, the vehicle merely chimes and lets the driver know that it's no longer on.

While driving the vehicle in rush hour traffic the engine slowed and would not accelerate when the gas pedal was applied. The light for the parking break went on along with the battery light. The vehicle appeared to be breaking on it’s own. A message was displayed though I do not recall what it was. Maybe a message to park? I attempted to switch to an outer lane and coast toward the side of the road until the vehicle stopped. I was partially blocking one of the two lanes in a no parking area. I turned the vehicle off and several times attempted to restart it. It would not initially shift into park. When it finally did I repeatedly attempted to start it. The display worked though the vehicle would not shift into drive, reverse, or neutral. The light for each gear flashed. I attempted for some time and at one point it did go into neutral. I attempted to restart and put it in drive then it would not go into neutral. I contacted the local Chrysler dealer who gave me the number for roadside assistance. I contacted the police to notify them as the vehicle was disabled in the road partially in a driving lane. I contacted Chrysler as this has happened previously. I have repeatedly had it serviced for similar problems with it unexpectedly turning off or not starting.

My husband, who was driving, pressed the gas pedal to try and regain power. At that point the check engine light came on, followed by a red turtle image accompanied by the words "vehicle speed may be limited". My husband was able to coast us to the shoulder of the highway before the vehicle stopped moving completely. We had no warning at all that anything was wrong. I had just had the vehicle in for an oil change a few days prior to the trip and they did a basic inspection- all was fine. I am so thankful the road was not crowded and that no 18 wheelers were too close. This could have ended very differently and we were very scared. After waiting by the side of the highway for nearly an hour, we were towed to a nearby dealer. They diagnosed transmission failure.

Chrysler (FCA US, LLC) is recalling certain 2017-2018 Pacifica Plug-In Hybrid Electric (PHEV) vehicles that received a software update remedy under NHTSA recall number 22V-077. The battery pack may cause a vehicle fire when parked, even with the ignition in the "Off" position.
Consequence & remedy
Consequence: A vehicle fire increases the risk of injury.
Remedy: Owners are advised not to charge their vehicles, and to park outside and away from structures, until they are repaired. Dealers will update the high voltage battery pack control module (BPCM) software. In addition, dealers will inspect and if necessary, replace the battery pack assembly. Repairs will be performed free of charge. Interim notification letters, notifying owners of the safety risk, were mailed August 1, 2024. Owner notification letters were mailed December 12, 2024. Owners may contact FCA US LLC customer service at 1-800-853-1403. FCA US LLC's number for this recall is 73B. Vehicles previously repaired under recall 22V-077 will need to have the new remedy completed.

Remedy: FCA will conduct a voluntary safety recall on affected vehicles to increase the torque and install a waterproof gasket to the 12v isolator post, the post will also be inspected for signs of contamination. In the event the post is compromised, the 12v isolator post and associated cables will be replaced. Until the final remedy is available, customers should bring their vehicles to dealers for an interim inspection of this joint. The recall remedy is currently under development. FCA US LLC will notify owners, and dealers will perform an interim inspection of this joint. If the inspection indicates the joint is compromised, owners will receive a loaner vehicle, until a final remedy is developed. Interim notifications are expected to begin around June 22, 2020. Chrysler (FCA US LLC) is recalling certain 2018 Chrysler Pacifica and Pacifica hybrid vehicles. The right front lower control arm may separate from the steering knuckle.
Consequence & remedy
Consequence: A control arm separation could increase the risk of a crash.
Remedy: Chrysler will notify owners, and dealers will inspect the right front knuckle and lower control arm for damage, replacing the components as necessary. The right front lower control arm knuckle pinch bolt will be replaced with a larger-diameter pinch bolt. These repairs will be performed free of charge. The recall began April 26, 2019. Chrysler (FCA US LLC) is recalling certain 2018 Jeep Renegade, Compass and Grand Cherokee, RAM 1500 and Promaster, Fiat 500x, Dodge Journey, Challenger, Charger and Durango and Chrysler 300x vehicles, 2017-2018 Jeep Wrangler, Dodge Grand Caravan and Chrysler Town and Country vehicles and 2018-2019 Jeep Cherokee and 2018 Chrysler Pacifica and Pacifica Hybrid vehicles. The powertrain control module may be equipped with a voltage regulator chip in the circuit board that may fail, causing a stall or a no start condition.
Consequence & remedy
Consequence: A vehicle stall can increase the risk of a crash.

On October 3, 2024, the Office of Defects Investigations (ODI) opened PE24028 to investigate alleged failures of Enel X Way's JuiceBox Level 2 residential EV chargers. The allegations were described as thermal incidents and electrical malfunctions, which could trip the circuit breakers in Juicebox owners’ homes and disrupt electric vehicle charge cycles. On October 2, 2024, Enel X Way posted on its website that Enel X Way North America would cease operations in the United States and Canada on October 11, 2024. The closure would potentially impact the charging app and supporting software. ODI sent Enel X Way an Information Request (IR) letter on October 21, 2024. Enel X Way provided an incomplete response to the IR on December 9, 2024. Enel X Way later provided a complete response to the IR on January 24, 2025. ODI reviewed Enel X Way’s IR submissions to determine whether further action is required. Based on ODI’s analysis, there was no clear trend in the cause of the failures described in customer complaints and warranty reports. Additionally, there have been no recent reports alleging safety issues. The available information does not warrant further action at this time. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high voltage (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery fires, and to increase monitoring of the manufacturer root cause investigation. On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy.
