AS LEAVING MY HOME AND GOING NO MORE THAN 15 MPH BECAUSE I WAS STILL IN A RESIDENTIAL NEIGHBORHOOD, I NOTICED THAT "BATTERY SAVER MODE" CAME ON THE DASH. WITHIN SECONDS, I NOTICED THE RADIO TURNED OFF AND THE CAR WAS SLOWING DOWN. I IMMEDIATELY TURNED AROUND AND AS I WAS RETURNING HOME (WHICH WOULD HAVE TAKEN ABOUT 60 SECONDS), THE CAR COMPLETED STOPPED. I STARTED TO NOTICE A BURNING SMELL, SO I TRIED TO PUT THE CAR IN PARK BUT IT WOULD NOT GO. I TRIED TO TURN THE CAR OFF, BUT NO SUCCESS. AS I WAS SMELLING THE BURNING SMELL, I DECIDED TO JUST OPEN THE CAR DOOR TO GET OUT AND IT WAS AT THAT TIME THE CAR JUST TURNED OFF. I CALLED THE DEALER AND DESCRIBED WHAT HAPPENED. THEY ADVISED THAT IT WAS SOMETHING TO DO WITH ALTERNATOR AND WOULD CHECK FOR RECALL. THEY CHECKED MY VIN AND ADVISED I WAS NOT INCLUDED IN RECALL, BUT OTHER 2012 CHRYSLER 300S HAD BEEN INCLUDED. AS I RESEARCHED ONLINE, MANY CONSUMERS WERE COMPLAINING ABOUT THIS SAME PROBLEM AND BEING TOLD THEY WERE NOT IN RECALL. I READ THE SYMPTOMS OF THE PROBLEM AS THE RECALL WAS NOTED AND I EXPERIENCED THE SAME THING. IN ADDITION, I WAS AFRAID THE CAR WAS GOING TO CATCH ON FIRE BY THE SMELL THAT WAS COMING IN THE CAR. HOPEFULLY, THIS PROBLEM CAN BE ADDRESSED.

ON 12/22/16, I USED MY CAR FOR WORK AND LATER CHRISTMAS SHOPPING WITH NO PROBLEM. AT 9:00 PM, MY HUSBAND, GRANDDAUGHTER AND I WERE ON THE FREEWAY TRAVELING ABOUT 60-65 MPH WHEN THE BATTERY SAVE MODE LIGHT CAME ON. I SMELLED SMOKE SO MY HUSBAND MERGED OVER TO THE FAR RIGHT, STILL TRAVELING ABOUT 50-55 MPH. THE CAR BEGAN TO SHUT DOWN, FIRST THE RADIO, THEN INSIDE PANEL LIGHTS, HEADLIGHTS, AIRBAG WARNING LIGHTS, BRAKE LIGHTS, WINDSHIELD WIPERS UP, AND CAR SLAMMED INTO PARK WHILE MOVING. FELT LIKE WE WERE REAR ENDED. VERY SCARY. LUCKILY, WE WERE ABLE TO COAST DOWNHILL OFF THE SIDE OF THE FREEWAY, BARELY MAKING IT OFF, WITH THE BACKEND OF THE CAR IN JEOPARDY OF BEING HIT. THE THREE OF US WERE STRANDED THERE ON THE SIDE OF THE ROAD UNTIL 1:30 AM, IN A NOT SO LIT AREA, UNTIL AAA ARRIVED. NONE OF US KNEW HOW TO PUT THE CAR IN NEUTRAL SO IT HAD TO BE FORCED ON AND OFF THE FLAT BED. THE NEXT DAY, I NOTIFIED CHRYSLER AND FOUND OUT THERE WAS A RECALL ON THE ALTERNATOR FOR THIS SAME MAKE, MODEL AND YEAR. MY CARE # IS 30764006. HOWEVER, I WAS TOLD THAT MY CAR WAS NOT COVERED BECAUSE IT HAD A 180 AMP ALTERNATOR INSTEAD OF 160 AMP. I WAS TOLD BY [XXX] TO HAVE MY CAR TOWED TO MOTOR VILLAGE LA FOR AN OFFICIAL DIAGNOSIS AND THAT SHE (CHRYSLER) WOULD TRY TO HELP ME GET IT APPROVED TO BE FIXED UNDER THE RECALL. MOTOR VILLAGE QUOTED ME $1200 TO FIX AND DENIED THE RECALL CLAIM. MY FAMILY AND I ALMOST LOST OUR LIVES THAT EVENING, AND I WASTED TIME WITH CHRYSLER RECALL. I HAD TO GET THE CAR TOWED ELSEWHERE BECAUSE I COULD NOT AFFORD $1200 FOR REPAIR. FURTHERMORE, WHEN I CALLED BACK, I WAS NOT ABLE TO SPEAK WITH [XXX] AND OF COURSE, THE STORY HAD CHANGED. THEY NO LONGER OFFERED HELP, ONLY APOLOGIES. AS A RESULT, I LOST TIME OFF WORK AND WAS WITHOUT A CAR FOR A WEEK. I AM REQUESTING REIMBURSEMENT FOR MY $130.00 DIAGNOSTIC CHARGE PAID TO MOTOR VILLAGE. THANK YOU. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *TR

EA21002 · Desiccated Air Bag Inflator Rupture

Opened Sep 17, 2021 · No close date supplied

Status: open (inferred from source dates) · Air Bags:frontal:driver Side:inflator Module; Air Bags:frontal:passenger Side:inflator Module

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Air Bags:frontal:driver Side:inflator Module

EA16002 · Vehicle Rollaway, Engine On

Opened Feb 3, 2016 · Closed Jun 24, 2016

Status: closed (inferred from source dates) · Power Train:automatic Transmission:gear Position Indication (prndl)

On April 22, 2016, Fiat Chrysler Automotive US LLC, (FCA) submitted a Defect Information Report (DIR) to NHTSA regarding a defect that could result in unattended vehicle rollaway with the engine running in approximately 811,586 model year (MY) 2012 to 2014 Dodge Charger and Chrysler 300 vehicles and MY 2014 to 2015 Jeep Grand Cherokee vehicles equipped with 8-speed transmissions and a monostable gear selector (NHTSA Recall #16V-240, FCA Recall #S27). According to FCA's DIR, The existing strategies built into these vehicles to deter drivers from exiting the vehicle after failing to put the transmission into PARK have not stopped some from doing so. Drivers erroneously concluding that their vehicle's transmission is in the PARK position may be struck by the vehicle and injured if they attempt to get out of the vehicle while the engine is running and the parking brake is not engaged. The vehicles recalled by FCA are equipped with Monostable electronic (shift-by-wire) gearshift assemblies supplied by ZF Group (see Figure 1).The subject Monostable shifter has a single neutral position that it snaps back to when the driver releases the shift knob.Although the Monostable gearshift has the familiar appearance of a conventional console mechanical gearshift assembly, it has an unfamiliar movement that does not provide the tactile or visual feedback that drivers are accustomed to receiving from conventional shifters.Consequently, the driver must take additional time to verify that the desired gear position was achieved by checking the PRNDL display on the shift knob or the Electronic Vehicle Information Center (EVIC) display.
