While driving on vacation approximately five hours away from home with my family, I suddenly heard a loud popping sound followed immediately by increased outside wind noise coming from the panoramic moonroof. Based on the sound, I suspected the moonroof glass had shattered. Because my family was in the vehicle, I intentionally did not open the interior sunshade, as I was concerned that broken glass could fall into the cabin and seriously injure my passengers. After safely pulling over, I inspected the roof from the outside and confirmed that the panoramic moonroof glass had completely shattered. During the drive back, the shattered glass panel completely detached from the vehicle and flew off. Fortunately, it landed in the grass and did not strike another vehicle or person, which could have resulted in a serious accident or injuries. Because this occurred over the weekend, I had no immediate access to repairs. To safely transport my family the five-hour drive back home, I had to temporarily cover the opening with a trash bag and tape to protect the vehicle’s interior and reduce exposure to the elements. After returning home, I contacted the dealership, provided photographs of the damage, and explained the circumstances. I was informed that the failure would not be covered under the vehicle’s warranty and that I would be responsible for the repair costs. I am extremely concerned that the moonroof shattered without any apparent impact from road debris or object. Had the interior sunshade been open at the time, pieces of broken glass could have fallen into the passenger compartment, potentially causing serious injuries to my family, including eye injuries or cuts. Additionally, when the glass panel detached from the vehicle, it could have struck another motorist or pedestrian, creating a significant safety hazard. I believe this incident represents a serious safety concern and request that this matter be thoroughly investigated and reconsidered for warranty coverage.

Beginning in May 2025, my 2025 Chevrolet Traverse began exhibiting a false “rear liftgate open” condition when the liftgate is fully closed. This defect triggers a continuous audible alarm that occurs before every drive and persists for the entire duration of the drive, stopping only when the vehicle is turned off. As a result of this defect, the rear windshield wiper and washer are disabled, eliminating rear visibility during rain and snow. The vehicle alarm system also cannot be armed, creating a security risk. The continuous alarm causes significant driver distraction, and the loss of rear visibility impairs safe operation of the vehicle, particularly in adverse weather. Minor children are frequent passengers, and the defect has occurred while they were in the vehicle. The vehicle has been presented to an authorized Chevrolet dealership on four separate occasions for the same issue. Multiple repair attempts, including diagnostics, software updates, component replacement, and latch assembly replacement, have failed to correct the defect. Because the alarm cannot be armed, the vehicle was left unsecured and was subsequently broken into, resulting in theft of personal property. General Motors has been notified and declined repurchase, directing me back to repeated diagnostics despite the unresolved safety-related defect. EA26006 · Electronic Brake Module Component Failure

On April 11, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24011 after receiving complaints concerning a loss of brake assist in 2023 MY Cadillac Lyriq vehicles. These complaints allege a hard brake pedal, followed by a “Brake System Failure” message at start up or while driving. GM shared records regarding several internal investigations for loss of brake assist due to alleged spindle fractures in the eBoost system. GM has stated that in the event of a spindle fracture, anti-lock brakes (ABS), stability control, and traction control remain functional until the vehicle comes to a complete stop. At this point, ABS, brake assist, stability control, and traction control functions are lost. The loss of these functions causes the vehicle to set one of three Diagnostic Trouble Codes (DTCs) based on failed performance criteria for the Electronic Brake Control Module (EBCM). These DTCs, in turn, activate several driver alerts: an audible chime, a visual “Service Brake System” message in the display, and multiple illuminated Malfunction Indicator Lights (MILs) for the ABS, Traction Control, and Brakes informing the driver of loss of functionality. The vehicle speed will also be limited to 70 KPH (43 MPH). Since opening PE24011, ODI has continued to receive reports of loss of brake assist. Some of these reports describe symptoms inconsistent with GM’s description of a spindle failure. These reports allege an immediate loss of brake assist functionality as opposed to the loss occurring after the vehicle comes to a complete stop. The sudden loss of brake assist during a braking event could result in extended braking distance, which increases the risk of a crash or injury. ODI believes further analysis is necessary to assess the potential for failures in GM’s eBoost system to resulting in safety-related outcomes. At this time, ODI is aware of the following vehicles that are equipped with this eBoost system: 2023-2026 MY Cadillac Lyriq, Chevrolet Colorado, and GMC Canyon; 2024-2026 MY Buick Enclave, Envision; Chevrolet Blazer EV, Equinox EV, Traverse; Cruise Origin; and GMC Acadia; 2025-2026 MY Cadillac Celestiq and Optiq. ODI is also aware of 2024-2026 MY Acura ZDX and 2024-2026 MY Honda Prologue vehicles that were built with this system. The Acura and Honda vehicles were manufactured by GM in a joint venture with Honda, and will be included in the Engineering Analysis as peer vehicles. ODI is opening this Engineering Analysis to further assess the safety-related consequences of a failed eBoost system. In addition, ODI is expanding the scope of the investigation to encompass all GM vehicles that share this eBoost system.
