I am filing a safety complaint regarding a dealership-provided loaner vehicle that had an open safety recall at the time it was issued to me. On Tuesday 1/20, I received a loaner vehicle from Beaver Toyota in Saint Augustine, Florida while my personal vehicle is being held due to a recall. The loaner was a 2025 Toyota Sienna. I was not informed of any safety recalls at the time the vehicle was provided. Late that evening, I discovered that the loaner vehicle had an active, unresolved safety recall listed as open through NHTSA. The recall was active while the vehicle was in my possession. After discovering the recall, I attempted to contact dealership management multiple times regarding this urgent safety issue. On 1/21, I contacted the General Manager through the dealership’s website. I also called the dealership on Wednesday at 1:37 PM and left a voicemail for the General Manager. I did not receive a response. On 1/22 at 12:24 PM, I called again and left another voicemail. I then followed up with an email requesting a return call due to the safety concern. As of this evening 1/22, I have received no response. Providing a loaner vehicle with an open safety recall, without disclosure, placed me and other occupants at unnecessary risk. I am requesting that NHTSA review this matter for recall compliance and appropriate corrective action.

I am filing a complaint regarding a safety recall on my 2025 Toyota Sienna. My vehicle is subject to an active safety recall that affects the second-row seating area. Due to this recall, the second row cannot be safely used. This makes the vehicle unsuitable for normal family use. I have a family of six, and the vehicle was purchased specifically for family transportation. Despite repeated attempts, the dealer has been unable to provide a timely repair appointment. I have been informed that the proposed recall repair may involve cutting and welding the vehicle floor, which would constitute a major structural modification to a brand-new vehicle. I am extremely concerned that such a repair would negatively impact the safety, integrity, and value of the vehicle. At this time, the vehicle remains unrepaired, and I am effectively deprived of full and safe use of the car due to a safety-related defect. I am requesting that this issue be reviewed, as the recall remedy is delayed and the proposed repair raises serious safety and quality concerns. Thank you for your attention to this matter.

I am submitting this complaint regarding an unresolved safety recall on my leased Toyota vehicle and the manufacturer’s failure to provide a timely or reasonable remedy, resulting in ongoing child passenger safety risks. Toyota’s recall instructs that no one sit in or use a child restraint in the second-row seats while the vehicle is moving until the remedy is performed. This effectively prohibits use of the safest and recommended seating position for children. The vehicle is used daily for highway driving to transport my family, including three children aged [XXX] , [XXX] , and [XXX] old. Because of Toyota’s seating restriction, my [XXX] toddler must now sit in the third row. This prevents the driver from easily monitoring or comforting the child and has caused driver distraction during highway travel when the child becomes distressed, creating a foreseeable safety risk. Toyota states it is “preparing the remedy” but has provided no repair timeline. After multiple contacts and escalation, I was told it could take up to 60 days with no interim solution. Toyota suggested contacting dealerships for a loaner but advised that an equivalent vehicle was unlikely, availability was uncertain, costs might need to be paid upfront with later reimbursement, and the duration would be open-ended. This is not a reasonable interim safety solution while continuing to pay full lease obligations. I was also informed that returning the vehicle would require paying approximately $15,000 in negative equity, which I believe is unreasonable given that the safety defect and usage restrictions are due to a manufacturer recall. Toyota has not provided a repair timeline, a reasonable interim transportation solution, or a penalty-free way to exit the lease. I request that NHTSA review whether Toyota is meeting its obligations under federal safety recall requirements. In October, 2025 I learned of a serious safety recall for improperly welded seat rails during manufacturing of a 2025 Toyota Sienna. I was not officially communicated of this safety recall by mail until December, 2025. I purchased this vehicle specifically to safely transport children and elderly parents, requiring full functionality of the captains seats and rear seats. While a repair/fix is being evaluated, Toyota provided the option to rent a vehicle and leave my new Toyota Sienna sitting on the dealer lot. However, no equivalent Toyota Siennas are available for long term rental at local dealerships, forcing us into highly inconvenient options for transport. Toyota was negligent in selling a new vehicle with improperly welded seat rails. There was a lapse in manufacturing quality control that is evident. Why were customers not notified until December of 2025, when these vehicles were sold nearly 6 months prior? After communications in December, 2025, there have been zero follow up correspondence from Toyota as of January 20, 2026.

Toyota issued a recall on some 2025 Toyota Sienna’s. Our app notified us in October and we immediately took it to the dealership for a rental, as we were advised not to use the second row seats. We have to use those because we have 3 kids in car seats. The interim notice was not even mailed out until December. We were told they would start rolling out the fix for this issue in late December or January. It is now January, and there is no update on the recall and nobody can get any information from Toyota. Our car has been sitting for over three months at the dealership, undrivable. They are not offering any compensation for people affected by this recall. Since it has been out of service for that long, it should should be a lemon law qualification. We did not purchase a brand new car for it to sit there unusable. There does not seem to be any urgency on Toyota‘s part for this to be fixed. We have no updates or information and this does not seem to be a priority for them. There are 55,000 vehicles affected. This is a large number of people, and somebody needs to put some real pressure on Toyota to fix the problem. This is unacceptable and somebody needs to look into this further. Please pass this along and do something about it because this is not fair to those of us who in good faith are making our payments every month on a car that we are not able to use. It’s basically sitting there rotting while Toyota takes their sweet time to figure out how to fix it. I am extremely unhappy and am looking for some type of recourse. And I know I am not the only one as I am seeing posts about it on online daily, from unhappy customers. This recall is due to a safety issue, if it’s so unsafe, why aren’t they fixing it?

They stated the second row is not safe in a high speed collision because they could eject. I’ve been using a rental for 3 months with no end insight. They stated they would have a timeline by 12/6 but that has yet to happen. My car is sitting at the dealership out in the elements for the unforeseeable future.

I received a notification on my Toyota App in mid-Oct advising there was a recall due to the second-row seat being improperly welded. At that time, I was advised a repair was in work and to avoid using those seats. However, I have four children and require all the seats in my Sienna so I have been forced to continue to use the seats, despite the warning. In December, we called our dealership to inquire if the repair was ready or if they had a loaner vehicle we could use until the repair was ready since we were not supposed to be using those seats. The answer was "no repair is ready" and there was no loaner they could provide for the foreseeable future. At this point, I feel like I am driving an unsafe car, continually forced to put my toddler and infant into seats that could become disconnected at any time because in almost 4 months Toyota still can't seem to figure out a repair for seat welds. Toyota either needs to get a repair done ASAP, provide us with a loaner, or pay for my family to rent an appropriate vehicle to support our needs until such a time as a remedy has been enacted.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Consequence & remedy

Consequence: Improperly welded seat rails can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will replace the second-row seat rails, free of charge. Owner letters were mailed May 15, 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB12 and 25TA12.

Consequence & remedy

Consequence: An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.

Remedy: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.
