The contact owns a 2025 Toyota Sienna Hybrid. The contact received notification of NHTSA Campaign Number: 25V668000 (Seats); however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that while attempting to adjust the position of the second-row seat forward or backwards, there was a lot of resistance, requiring an excessive amount of force to move the seat. Occasionally, the contact had to physically push the seat with their feet. The dealer was contacted and informed that the part to do the recall repair was not yet available. The vehicle remained with the dealer, awaiting parts for the recall repair. The manufacturer was made aware of the issue; however, the manufacturer failed to follow up on with the contact. The failure mileage was approximately 18,000.

I am reporting an unreasonable delay in the remedy for NHTSA Recall 25V-668 (Toyota Recall 25TA12). My 2025 Toyota Sienna has been subject to Interim Notice 25TB12 for approximately 170 days. Per Toyota’s official safety instructions, I have been directed to ensure that 'no one sit in or use a child restraint in any second-row seat while the vehicle is in motion.' Despite this recall being active since October 2025, Toyota has failed to provide the necessary replacement seat rails or a firm repair date. This defect involves structural weld failures that pose a high risk of seat detachment in a collision. By rendering the primary passenger area of this minivan unusable for nearly 6 months, the manufacturer has caused a substantial loss of utility and safety. I am requesting NHTSA investigate the lack of remedy availability and Toyota’s failure to support owners who cannot safely use the vehicle for its intended multi-passenger purpose.

It is now March 26 and there is still no fix or timeframe for a fix. This issue put my family and anyone else who sat in the second row at severe risk of injury or worse if an accident occurred.

Toyota has issued a recall for the 2025 Sienna Mini Vans: The subject vehicles contain second-row seats that are mounted on seat rails that may have been improperly welded. If these seats are occupied during certain high-speed collisions, the seat may lose structural integrity, increasing the risk of injury. This recall was initiated in October 2025, not notifying customers until December of 2025. I was riding around with my young children in these unsafe seats for two months before notified. The dealership is giving me a loaner but it is not large enough or adequete for my family. Toyota continues to list "remedy not yet available" when I go to check on the status. The problem has not been reproduced because they are just not to be sat it which in turn means, I cannot use the vehicle. My vehicle has not been inspected by the dealer or service center as there is no remedy. There wer not any warning lamps that appeared due to this issue.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Sienna Hybrid vehicles. The second-row seat rails may be welded improperly.

Consequence & remedy

Consequence: Improperly welded seat rails can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will replace the second-row seat rails, free of charge. Owner letters were mailed May 15, 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB12 and 25TA12.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Consequence & remedy

Consequence: An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.

Remedy: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.
