When purchased "As Is", an airbag deployment was disclosed but not that the front passenger side airbag had not been replaced. In December of 2025, an SRS Airbag warning appeared. Upon inspection, the mechanic discovered a "resistor" had been inserted in the SRS cirucuity to avoid disclosure of the absence of a replacement airbag. The replacement airbag was installed at my cost of $1,578.09 +8% Ohio Sales Tax. I contacted the selling dealer, Toyota of Bedford (OH), in early January 2026 but have not received a response. The other dealer disclosed in the Carfax (likely responsible for the omission of the airbag when the vehicle was repaired in July of 2022) is Suburban Toyota of Troy, MI.

The contact stated that approximately four months ago, her eldest son was entering the vehicle, he became stuck between the door and the door frame. The contact stated that on a later date while her youngest son was entering the vehicle, the close button was depressed on the outside of the vehicle, and without any mechanism to prevent premature closure, his arm became stuck between the door frame and the door while the contact was inside the residence retrieving additional items to take the children to school. The contact stated that her older daughter was able to remove him from the vehicle. The contact stated that her son sustained injuries to his arm and had marks all over his arm where it was stuck between the door and door frame. There was no medical attention provided but an ice pack was placed on the part of the arm that was stuck in the door. The contact was concerned about the safety risk posed by the failure and the lack of a sensor or mechanism to prevent the door from closing while a person was entering or leaving the vehicle. There were no warning lights illuminated during the failure. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The contact stated while driving 80 MPH, the rear passenger's side tire had a blowout. The contact stated she drove across the road and pulled to the side of the road. The contact stated that AAA assisted in replacing the tire with a spare tire. The vehicle was taken to Walmart the following morning, where the spare tire was replaced. Additionally, the contact stated while driving at an undisclosed speed, the TPMS warning light was illuminated. The vehicle was taken to Discount Tires, where it was determined that there was a hole in the rear driver's side tire. The tire was replaced. The contact stated that the front passenger's tire was also replaced at Walmart due to a pinhole. The dealer was notified of the failure. The tire manufacturer was not notified of the failure. The contact owns a 2022 Toyota Sienna Hybrid. The contact stated while driving 40 MPH, a rock was kicked up and struck the passenger’s side of the windshield near the middle of the windshield. The contact owns a modified wheelchair accessible vehicle. The vehicle was equipped with multiple sensors and a wheelchair ramp for the contacts husband. The contact stated that she first took the vehicle to a glass shop to repair the windshield. The windshield was replaced with a manufacturer OEM windshield. The glass shop could not calibrate the windshield and stated that they did not have the specs for the calibration. The contact drove the vehicle to a dealer. The dealer also could not calibrate the windshield and referred the contact to the vehicle modification company. The vehicle had not been properly repaired. The contact stated the vehicle modification manufacturer was in Arizona however, the contact lives in Virginia. The contact called the modification company who advised that if the contact could locate a dealer or glass repair shop they could walk the dealer or shop through the process; however, the contact stated that there was no dealer or shop nearby. The manufacturer had been informed of the failure. The failure mileage was approximately 6,000.

Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
Consequence & remedy
Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.
Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."
Consequence & remedy
Consequence: An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.
Remedy: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
Consequence & remedy
Consequence: Driving with a deactivated stability control system increases the risk of a crash.
Remedy: Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Sienna vehicles equipped with seating for eight passengers. An incorrect seat belt webbing guide may have been installed on the outboard second-row seat belt assemblies.
Consequence & remedy
Consequence: During seat belt pre-tensioner deployment, the incorrect webbing guide may damage the seat belt webbing, causing it to tear and increasing the risk of injury during a crash.
Remedy: Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
