AS I WAS DRIVING HOME, THE CHECK ENGINE, THE TRAC AND VSC LIGHTS ALL CAME ON AT THE SAME TIME. I ALSO HEARD A HISSING NOISE WHEN I PRESSED DOWN ON THE BRAKES. I WENT TO A MECHANIC TO SEE WHAT THE PROBLEM MIGHT BE, BUT IT WAS LATE IN THE DAY AND EVERYONE WAS CLOSING. HE TOLD ME IT SOUNDED LIKE A VACUUM LEAK IN THE BRAKE BOOSTER AND TO BRING IT BACK FIRST THING MONDAY (HE SEEMED TO THINK IT'D BE OKAY TO DRIVE OVER THE WEEKEND). ON THE DRIVE HOME THE BRAKE LIGHT CAME ON AND THERE WERE A FEW TIMES WHERE I WAS AFRAID I WOULDN'T BE ABLE TO STOP THE CAR AT ALL. IT WAS QUITE SCARY! I WAS PRESSING THE BRAKE PEDAL AS HARD AS I COULD AND THEN IT FINALLY GRABBED, THE CAR JERKED, AND THE SEAT BELTS TIGHTENED UP AROUND ME. I WORKED TODAY (SATURDAY) AND THE SAME THING HAPPENED TO AND FROM WORK, ONLY NOW I AM USING THE EMERGENCY BRAKE TO SLOW THE CAR AND I PUT IT IN PARK AT STOP LIGHTS. MY NEXT TRIP WILL BE STRAIGHT TO A MECHANIC! IT SEEMS IT IS NOT COMMON FOR BRAKE BOOSTERS TO FAIL AND I AM LEARNING THAT OTHER TOYOTA RAV4 OWNERS HAVE HAD THE SAME PROBLEM, SEVERAL ENDING IN ACCIDENTS. THIS SHOULD DEFINITELY BE ADDRESSED BY TOYOTA, AS IT IS A HUGE SAFELY ISSUE, AND POSSIBLY BE CAUSE FOR A RECALL. *TR

I WAS TRAVELING AT APPROXIMATELY 45 MPH AND APPLIED THE BRAKES. A HISSING SOUND COULD BE HEARD AND THERE WAS A SIGNIFICANT LOSS OF STOPPING POWER. I HAD TO APPLY AN EXCESSIVE AMOUNT OF PRESSURE TO THE PEDAL TO STOP THE VEHICLE. AND IT ALSO TOOK AN EXCESSIVE DISTANCE TO GET IT STOPPED.. THE CHECK ENGINE LIGHT ILLUMINATED ALONG WITH THE STABILITY CONTROL AND OTHER DASHBOARD WARNING LIGHTS. I CONTINUED TO DRIVE CAUTIOUSLY TO GET HOME. THE LIGHTS THEN WENT AWAY. AND THE CAR PERFORMED SOMEWHAT NORMALLY. THEY CAME BACK ON, A DAY LATER. AND SO DID THE SYMPTOMS, BUT ONLY SPORADICALLY. THE BRAKE BOOSTER HAS A LEAK. THERE ARE MANY OTHERS WITH THE SAME PROBLEM ALL OVER THE INTERNET. SPECIFICALLY, HERE: HTTP://WWW.RAV4WORLD.COM/FORUMS/99-4-3-MECHANICAL/91243-WHAT-CAUSES-BRAKE-BOOSTER-FAILURE.HTML I THINK GIVEN THE HISTORY OF SUDDEN ACCELERATION AND OTHER ISSUES WITH THIS MODEL OF VEHICLE A RECALL SHOULD BE DONE. THIS IS A $900+ REPAIR, AND FOR A CAR WITH A LITTLE OVER 50K MILES IS UNACCEPTABLE AND ABOVE ALL, DANGEROUS. *TR

BRAKES WERE THERE (PEDAL WAS FIRM AND DID NOT GO TO FLOOR) BUT ACCELERATOR SEEM TO OVERRIDE THE BRAKING SYSTEM. PROCEEDED TO STAND ON BRAKES TO ATTEMPT TO STOP AS THERE WERE THREE LANES OF STOPPED TRAFFIC AND NO WHERE ELSE TO GO. SHIFTED CAR INTO PARK TO COME TO AN ABRUPT STOP TO AVOID HITTING 8+ CARS AND PUSHING THEM INTO A VERY LARGE INTERSECTION. CAR HAD APPROXIMATELY 46000 MILES ON IT AND HAD NEW BRAKES INSTALLED 8 MONTHS PRIOR. CAR HAD BEEN SERVICED 1 MONTH PRIOR FOR GENERAL SERVICE POINT CHECKS. TOOK CAR TO DEALERSHIP SAME DAY OF THIS INCIDENT - THEY COULD NOT FIND NOTHING WRONG WITH THE CAR. OFFERED TO SERVICE BASED ON A PREVIOUS RECALL TO REMOVE THE CURRENT FLOORMAT AND ADJUST ACCELERATOR. ONE MONTH PRIOR TO THIS INCIDENT THE RADIO STOPPED WORKING FOR A COUPLE OF DAYS THEN STARTED WORKING INTERMITTENTLY SOMETIMES DISPLAYING GARBLED TEXT ON THE RADIO DISPLAY. *TR

Aug 12, 2016

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2011 Toyota RAV4 vehicles manufactured October 31, 2005, to September 7, 2010, and 2010 Lexus HS250h vehicles manufactured July 6, 2009, to August 26, 2010. The affected vehicles have rear suspension arms (rear tie rods) with adjusting lock nuts that may have been improperly tightened after servicing. As a result of being loose, the arm may have thread damage and may rust, possibly leading to the failure of the arm and an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners and dealers will replace both rear suspension arms and encapsulate the locknuts, free of charge. The recall began on September 30, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0V for Toyota vehicles and GLK for Lexus vehicles. Note: This recall supersedes recall 13V-383, in which some vehicles were only inspected, sealed, and did not have both arms replaced. The affected vehicles were previously recalled under 13V-383, but did not have both rear suspension arms replaced at that time. Those vehicles need to have both arms replaced as part of this recall remedy.

Apr 9, 2014

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.

Consequence & remedy

Consequence: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.

Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. General Motors sent notices to the Pontiac Vibe owners on September 15, 2014, that a remedy is available. The recall began on October 27, 2014. Owners may contact Toyota at 1-800-331-4331. Owners may contact General Motors at 1-800-762-2737.

Aug 28, 2013

Toyota is recalling certain model year 2006 through 2011 RAV4 and 2010 Lexus HS250h passenger vehicles manufactured from October 2005 through September 2010. If the nuts for adjusting the rear wheel alignment are improperly tightened during service, the rear suspension arm (rear tie rod) may develop unwanted movement and rust leading to thread damage and eventual failure. Failure of the rear tie rod will cause an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners, and dealers will inspect the tie rods and replace any that have rust or corrosion on the alignment adjustment threads. All of the rear tie rods will then be encased in an epoxy to prevent future environmental damage. Due to limitations in parts, the recall remedy will be launched in phases from September 2013 through mid-April 2014. All owners will receive notification of the recall, with most receiving second notifications when parts are available in their area. IMPORTANT: This recall supersedes Toyota and Lexus recall 12V-373. All vehicles that have been inspected and remedied under the previous recall need to be re-inspected and have the appropriate remedy applied.

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

Status: closed (inferred from source dates) · Suspension; Suspension:rear

In a letter dated August 1, 2012 Toyota Motor Engineering & Manufacturing North America, Inc. (Toyota) submitted a Defect Information Report (NHTSA Recall No. 12V-373) to the NHTSA identifying a safety defect regarding possible separation of the Rear Suspension Arm No. 1 Assembly (arm) at the threaded portion (shaft and turn buckle) of the component in MY 2006 through MY 2011 Toyota RAV4 vehicles built from October 2005 through September 2010 and in MY 2010 Lexus HS250h vehicles manufactured from July 2009 through August 2010.According to Toyota, if the adjustment nuts for the rear wheel alignment are improperly tightened when alignment is performed in service, backlash may develop at the threaded portion of the arm, followed by rust formation. The threads may then wear causing the arm to separate, which may result in a loss of vehicle control. In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and consumer complaint data submitted to Toyota, ODI identified 131 non-duplicative reports alleging failure of the subject rear suspension arm (Note:some of the complaints and related service records refer to the arm as a rear tie rod).Six of the complainants allege the failure resulted in some loss of vehicle control. For purposes of this analysis, loss of control events involve alleged complete or partial departure from the intended lane of travel. An additional four complaints reported difficulty maintaining control of the vehicle when the arm failed.Recalled vehicles will be inspected for proper torque of the adjustment nuts and replaced as necessary. In addition, Toyota will take several actions to reduce the potential for improper torque in future alignment procedures. This includes:(1) providing proper tightening sequence and torque specification information to two major wheel alignment equipment manufacturers, who would disseminate the information to alignment shops using their equipment; (2) installation of a clip on each arm with an instructional label showing proper tightening sequence and torque specification (the clip must be removed to perform a wheel alignment); (3) installation of additional labels on the front and back vertical surfaces of the Rear Suspension Arm No. 2 assemblies; and (4) providing an instruction booklet to owners to place in the glove box. This investigation is closed.
