WHILE DRIVING FROM MARYLAND TO MAINE FOR A VACATION, I NOTICED MY TRANSMISSION WAS SLIPPING AND GRINDING GEARS FREQUENTLY IN THE RANGE OF 20-35 MPH. I NOTICED IT HAPPENING MORE FREQUENTLY DURING SLOWDOWN FOR MAKING A TURN AND THEN ACCELERATION TO RESUME DRIVING. I DID NOT HAVE ANY ACCIDENTS, BUT THIS HAS BEEN HAPPENING MORE FREQUENTLY. I HAD PREVIOUSLY REPORTED THE PROBLEM, WHEN IT HAPPENED LESS FREQUENTLY, TO MY DEALERSHIP (DARCARS OF FREDERICK) AND THEY TOLD ME THAT 1)THEY COULD NOT DUPLICATE THE SOUND DURING THEIR TEST RUN AND 2) IF SUCH A THING HAD HAPPENED, MY ECU WOULD HAVE MADE A NOTE OF IT AND RECORDED IT. SINCE THERE WAS NO RECORD OF IT HAPPENING, IT WAS NOTHING. HOWEVER, UPON READING SEVERAL COMPLAINTS ABOUT THE RAV4 02-03 MODELS, REGARDING TRANSMISSION PROBLEMS, I FIND THAT THEIR ISSUES OF HANGING BEFORE SLIPPING INTO GEAR, GRINDING GEAR AND NOT FINDING THE GEAR AT ALL ARE THE SAME ONES I HAVE BEEN EXPERIENCING IN MY '06 RAV4. COULD IT BE THAT MY ECU IS BAD? YES, BUT MY DEALER WON'T ACKNOWLEDGE THAT POSSIBILITY. THEY CHECK MY FLOOR MATS, BUT NOT THE ECU FOR FAULTY ISSUES. THIS IS MY PRIMARY VEHICLE AND I TRANSPORT MY DAUGHTER IN IT ALL OF THE TIME. I AM WORRIED ONE DAY I AM GOING TO BE DRIVING AND GET INTO A SERIOUS ACCIDENT BECAUSE OF THIS ISSUE. I HAVE ALL OF MY MAINTENANCE RECORDS AND RECENTLY HAD THE TRANSMISSION FLUSHED AND FILLED COMPLETELY. I USED OEM FLUID AND AM ON SCHEDULE WITH ALL OF MY MAINTENANCE. *TR

FOR THE SECOND TIME THIS YEAR (I ALSO DOCUMENTED THE SAME PROBLEM 2/24/13), WE EXPERIENCED UNEXPECTED ACCELERATION AND THE CAR BUCKING. WE WERE BACKING OUT OF A DRIVEWAY. MY HUSBAND BACKED UP AND REALIZED HE NEEDED TO STRAIGHTEN OUT ON THE CURVED DRIVEWAY SO PUT IT IN DRIVE FOR A FEW FEET AND THEN WHEN HE PUT IT IN REVERSE, THE ENGINE STARTED REVVING UP TO A MAXIMUM RPM'S. HE IS 200 POUNDS AND HAD TO HOLD HIS FOOT ON THE BREAK TO KEEP IT FROM MOVING. HE PUT IT IN NEUTRAL AND TURNED THE CAR OFF. MY 20 YEAR OLD SON WAS IN THE CAR WITH US AND GOT OUT AND COULD SMELL THE ENGINE FROM REVVING UP SO HIGH. MY BROTHER AND SISTER IN LAW CAME RUNNING OUT OF THE HOUSE BECAUSE THEY COULD HEAR THE ENGINE REVVING UP AND KNEW SOMETHING WAS VERY WRONG. LAST TIME THIS HAPPENED, WE BROUGHT IT TO THE DEALERSHIP TWICE AND THEY COULDN'T REPLICATE THE ISSUE. I AM NOT SAFE IN MY OWN CAR. TOYOTA HAS GOT TO TAKE RESPONSIBILITY FOR THIS AND FIX THE ISSUE! IT IS A HUGE PROBLEM. HAD I BEEN DRIVING, NO TELLING WHAT WOULD HAVE HAPPENED AS I WOULD HAVE BEEN CAUGHT TOTALLY OFF GUARD AND I'M NOT SURE I WOULD HAVE HAD RESPONDED APPROPRIATELY UNDER PRESSURE. THIS IS UNACCEPTABLE!!!!! I AM AFRAID TO DRIVE MY CAR!!! *JS

I TOOK MY 2006 RAV4 INTO THE LEITH TOYOTA DEALERSHIP IN RALEIGH, NC FOR THE ACCELERATOR PEDAL RECALL. SERVICE MANAGER CALLS ME AND SAID THAT THERE IS A BANGING VIBRATION AND NOISE COMING FROM THE STEERING COLUMN WHEN THE STEERING WHEEL IS TURNED. (THIS DID NOT EXIST BEFORE.) THE MANAGER SAID THERE IS A TECHNICAL SERVICE BULLETIN ON THE MANUFACTURERS DEFECTIVE DESIGN AND THAT FAILURE TO CORRECT THE PROBLEM COULD RESULT IN AN ACCIDENT. THIS REPAIR IS NEARLY $400 AND IS NOT COVERED UNDER ANY RECALL NOR MY NOW EXPIRED WARRANTY. WHEN I NEXT DROVE THE RAV4 THE BANGING WAS SO STRONG THAT THE FRONT SEAT PASSENGER COULD FEEL IT. I CALL TOYOTA CUSTOMER CARE PROGRAM, WHO CONFIRMED THE TECHNICAL SERVICE BULLETIN BUT SAID THAT THE PROBLEM IS NOT A SAFETY ISSUE AND THAT I SHOULD CONTACT THE DEALERSHIP TO INQUIRE ABOUT THEM PAYING FOR THE REPAIR. THE DEALERSHIP SAID THAT THE REPAIR IS NOT COVERED AND THAT TOYOTA CUSTOMER CARE WOULD CONTACT ME AS TO WHAT FURTHER WOULD BE DONE. AFTER WAITING MORE THAN 60 DAYS FOR TOYOTA TO CALL ME, I CALLED CUSTOMER CARE AGAIN ONLY TO BE TOLD THAT NOTHING MORE WOULD BE DONE. I ASKED TO SPEAK TO A SUPERVISOR, BUT NONE WAS AVAILABLE. I WAS TOLD THAT A SUPERVISOR WOULD CALL ME BACK WITHIN 24 HOURS. 30 HOURS LATER TOYOTA CUSTOMER CARE LEFT A PHONE MESSAGE FOR ME TO CALL THEM BACK. I HAVE ALSO HAD ELECTRICAL PROBLEMS THAT HAVE CAUSED THIS RAV4 TO LOOSE POWER AND STOP WHILE TRAVELING DOWN THE ROAD AND THE BATTERY GOES DEAD ABOUT EVERY TWO MONTHS. *TR

Aug 12, 2016

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2011 Toyota RAV4 vehicles manufactured October 31, 2005, to September 7, 2010, and 2010 Lexus HS250h vehicles manufactured July 6, 2009, to August 26, 2010. The affected vehicles have rear suspension arms (rear tie rods) with adjusting lock nuts that may have been improperly tightened after servicing. As a result of being loose, the arm may have thread damage and may rust, possibly leading to the failure of the arm and an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners and dealers will replace both rear suspension arms and encapsulate the locknuts, free of charge. The recall began on September 30, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0V for Toyota vehicles and GLK for Lexus vehicles. Note: This recall supersedes recall 13V-383, in which some vehicles were only inspected, sealed, and did not have both arms replaced. The affected vehicles were previously recalled under 13V-383, but did not have both rear suspension arms replaced at that time. Those vehicles need to have both arms replaced as part of this recall remedy.

Oct 22, 2015

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Consequence & remedy

Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.

Apr 9, 2014

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.

Consequence & remedy

Consequence: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.

Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. General Motors sent notices to the Pontiac Vibe owners on September 15, 2014, that a remedy is available. The recall began on October 27, 2014. Owners may contact Toyota at 1-800-331-4331. Owners may contact General Motors at 1-800-762-2737.

Aug 28, 2013

Toyota is recalling certain model year 2006 through 2011 RAV4 and 2010 Lexus HS250h passenger vehicles manufactured from October 2005 through September 2010. If the nuts for adjusting the rear wheel alignment are improperly tightened during service, the rear suspension arm (rear tie rod) may develop unwanted movement and rust leading to thread damage and eventual failure. Failure of the rear tie rod will cause an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners, and dealers will inspect the tie rods and replace any that have rust or corrosion on the alignment adjustment threads. All of the rear tie rods will then be encased in an epoxy to prevent future environmental damage. Due to limitations in parts, the recall remedy will be launched in phases from September 2013 through mid-April 2014. All owners will receive notification of the recall, with most receiving second notifications when parts are available in their area. IMPORTANT: This recall supersedes Toyota and Lexus recall 12V-373. All vehicles that have been inspected and remedied under the previous recall need to be re-inspected and have the appropriate remedy applied.

PE12013 · Rear Suspension Arm Assembly Failure

Opened Jun 4, 2012 · Closed Sep 13, 2012

Status: closed (inferred from source dates) · Suspension; Suspension:rear

In a letter dated August 1, 2012 Toyota Motor Engineering & Manufacturing North America, Inc. (Toyota) submitted a Defect Information Report (NHTSA Recall No. 12V-373) to the NHTSA identifying a safety defect regarding possible separation of the Rear Suspension Arm No. 1 Assembly (arm) at the threaded portion (shaft and turn buckle) of the component in MY 2006 through MY 2011 Toyota RAV4 vehicles built from October 2005 through September 2010 and in MY 2010 Lexus HS250h vehicles manufactured from July 2009 through August 2010.According to Toyota, if the adjustment nuts for the rear wheel alignment are improperly tightened when alignment is performed in service, backlash may develop at the threaded portion of the arm, followed by rust formation. The threads may then wear causing the arm to separate, which may result in a loss of vehicle control. In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and consumer complaint data submitted to Toyota, ODI identified 131 non-duplicative reports alleging failure of the subject rear suspension arm (Note:some of the complaints and related service records refer to the arm as a rear tie rod).Six of the complainants allege the failure resulted in some loss of vehicle control. For purposes of this analysis, loss of control events involve alleged complete or partial departure from the intended lane of travel. An additional four complaints reported difficulty maintaining control of the vehicle when the arm failed.Recalled vehicles will be inspected for proper torque of the adjustment nuts and replaced as necessary. In addition, Toyota will take several actions to reduce the potential for improper torque in future alignment procedures. This includes:(1) providing proper tightening sequence and torque specification information to two major wheel alignment equipment manufacturers, who would disseminate the information to alignment shops using their equipment; (2) installation of a clip on each arm with an instructional label showing proper tightening sequence and torque specification (the clip must be removed to perform a wheel alignment); (3) installation of additional labels on the front and back vertical surfaces of the Rear Suspension Arm No. 2 assemblies; and (4) providing an instruction booklet to owners to place in the glove box. This investigation is closed.
