FRONT LEFT HEADLIGHT STOPPED WORKING AT NIGHT ON A 2006 TOYOTA PRIUS. IT NEVER WORKED AGAIN. THIS IS A SAFETY ISSUE. I UNDERSTAND THIS IS A COMMON PROBLEM ON THE 2006 PRIUS. HAVE TO HAVE IT REPLACED AT A COST OF $325 AT DEALERSHIP! THERE SHOULD BE A RECALL. *TR

MY 2006 TOYOTA PRIUS HAS 52,000 MILES ON IT. THE DRIVER'S SIDE HEADLIGHT WILL TURN ON WHEN I SWITCH THEM ON, THEN WILL GO OUT A FEW MINUTES INTO MY TRIP, AND WHEN I TURN THE LIGHTS OFF THEN ON AGAIN IT COMES BACK ON. I HAVE NOTICED THIS PROBLEM WITH BOTH HEADLIGHTS, ALTHOUGH IT IS MORE COMMON WITH THE DRIVER'S SIDE LIGHT. THIS OCCURS ALMOST EVERY TIME I DRIVE. BASED ON WHAT I'VE READ, THIS APPEARS TO BE A MANUFACTURER'S DEFECT. *TR
I HAVE A 2006 PRIUS. THE DRIVER SIDE HEAD LIGHT GOES OUT NOW AND THEN AS I AM DRIVING. IT IS UNPREDICTABLE. IF I TURN THE LIGHTS OFF AND THEN BACK ON, THE LIGHT WORKS AGAIN. IT HAS BEEN GOING ON FOR 2 MONTHS. NOW THE OTHER LIGHT IS DOING THE SAME THING. *TR

I OWN A 2006 TOYOTA PRIUS. FOR THE LAST 6 MONTHS THE DRIVER SIDE FRONT HEADLIGHT INTERMITTENTLY GOES OUT. IF WE SHUT OFF OUR HEADLIGHTS AND THEN BACK ON, IT WILL RE-LIGHT FOR AN INDETERMINATE AMOUNT OF TIME AND THEN THE PROBLEM REPEATS ITSELF. I'VE BEEN STOPPED TWICE BY THE POLICE AND GIVEN WARNINGS. I'BE TAKEN IT TO THE DEALERSHIP THREE TIMES NOW TO HAVE IT FIXED. THE FIRST TIME, THEY SAID THEY COULDN'T FIND ANYTHING WRONG. THE SECOND TIME, THEY COULD MAKE IT HAPPEN, BUT BY BANGING THE FRONT OF THE HOOD COULD GET THE HEADLIGHT TO ILLUMINATE, AND SUGGESTED I JUST LIVE WITH THE ISSUE, AS REPLACING THE HEADLIGHT WOULD BE "EXPENSIVE" (NO AMOUNT MENTIONED). TODAY I BROUGHT THE CAR IN FOR ITS REGULAR MAINTENANCE WORK, AS THE LIGHT ON THE DASHBOARD SUGGESTED IT WAS TIME. THE DEALERSHIP IS IRA TOYOTA OF MANCHESTER NH 03101. THEIR # IS: 603-624-1800. MY COMPLAINT IS THAT WHEN THEY CHANGED THE BULB THEY CHARGED $157.82 FOR THE LIGHTBULB AND ANOTHER $180 FOR LABOR TO REPLACE IT. I'VE NEVER HEARD OF SUCH COSTS TO REPLACE A HEADLIGHT. I WENT ON THE WWW TONIGHT TO SEE IF MY SITUATION WAS UNUSUAL AND FOUND THAT MANY CONSUMERS ARE HAVING THIS SAME ISSUE WITH THEIR 2006 PRIUS. IT WAS RECOMMENDED THAT WE FILE A COMPLAINT WITH YOU....HERE I AM. THE WEBSITE WITH THE INFORMATION REGARDING THIS COMMON INCIDENT IS: WWW.HYBRIDCARS.COM/FORUM/REPLACING-HEADLIGHT-BULB--T910 PLEASE LOOK INTO THIS. THANK YOU.

I HAD REPORTED IT TO THE TOYOTA DEALER AND THEY SAID NOTHING WAS WRONG AND THERE HAD NOT BEEN ANY SIMILAR PROBLEM REPORTED. I WENT BACK AGAIN THE NEXT WEEK WITH THE SAME COMPLAINT. THE SUPERVISOR OF MAINTENANCE SAID HE WOULD LOOK INTO IT. FINALLY, THE LAST TIME I TOLD THEM THAT IF I END UP IN A FATALITY D/T THIS PROBLEM WITH THE HEADLIGHTS, SOMEONE IN TOYOTA WILL BE IN DEEP TROUBLE. THAT IS WHEN THE SUPERVISOR TOOK CARE OF THE PROBLEM THE NEXT DAY. THE CAR WAS STILL UNDER WARRANTY. SAME THING HAPPENED NEXT YEAR 2008. I HAD TO HAVE THE TWO FRONT HEADLIGHTS REPLACED AND THIS TIME OUT OF MY POCKET WHICH IS VERY RIDICULOUSLY EXPENSIVE. *TR

2006 TOYOTA PRIUS WITH HID LIGHTS. AT 30,000 MILES, IN DECEMBER 2007, THE PASSENGER SIDE LIGHT BEGAN TO TURN OFF BY ITSELF. IT WOULD TURN BACK ON IF WE TURNED THE LIGHTS COMPLETELY OFF, THEN ON AGAIN (DANGEROUS AT NIGHT, OF COURSE). AT 35,000 MILES, THE DEALERSHIP ESTIMATED $350 TO REPLACE THE BULB, BUT THEN REPLACED BULB UNDER WARRANTY. AT 50,000 MILES THE DRIVERS SIDE HEADLIGHT BEGAN TO DO THE SAME THING. WE COULD NOT AFFORD TO PAY FOR THE REPLACEMENT SO HAVE BEEN TURNING OFF AND ON TO REMEDY. LAST WEEK THE PASSENGERS SIDE HEADLIGHT STARTED DOING IT AGAIN, EVEN THOUGH IT IS ONLY 1 YEAR OLD. THERE ARE TIMES WE HAVE ONE HEADLIGHT, AND TIMES WE HAVE NO HEADLIGHTS OTHER THAN THE FOG LIGHTS. WE HAVE BEEN STOPPED BY POLICE AND GIVEN A WARNING ABOUT A HEADLIGHT BEING OUT. THIS IS A DANGEROUS SITUATION AND WE THINK THAT IT CANNOT BE THE HID LAMPS THEMSELVES, AS THEY COME BACK ON AGAIN WHEN WE RECYCLE THE SWITCH. IT MUST BE A COMPUTER OR RELAY. WE HAVE DISCOVERED THAT THERE ARE MANY OTHER PRIUS OWNERS WITH THE SAME EXACT ISSUE: SEE THE FORUM AT HTTP://WWW.HYBRIDCARS.COM/FORUMS/REPLACING-HEADLIGHT-BULB-T910.HTML . *TR
2006 TOYOTA PRIUS HEADLAMP GOES OUT INTERMITTENTLY, TURNING LIGHTS OFF THEN BACK ON TEMPORARILY FIXES PROBLEM. SINCE PROBLEM IS INTERMITTENT IT IS IMPOSSIBLE TO KNOW IF FRONT LIGHTS ARE WORKING. *TR
THE HID HEADLAMPS, FLICKER, GO-OUT AT WILL, SOMETIMES ITS JUST ONE HEAD LIGHT OTHER TIME ITS BOTH AT THE SAME TIME!!!! TOYOTA HAS FIXED IT BUT AFTER MAYBE A MONTH IT WENT BACK TO DO THE SAME EXACT THING! THIS IS DANGEROUS!! AND APPARENTLY HAPPENS VERY VERY OFTEN TO ALOT OF OTHER 2006 TO 2009 TOYOTA OWNERS. *TR
2006 TOYOTA PRIUS FRONT DRIVER SIDE HID HEADLIGHT TURNS OFF. IF I TURN THE LIGHTS OFF AND THEN BACK ON, THE HEADLIGHT COMES BACK ON AND FUNCTIONS PERFECTLY. THIS HAPPENS AT RANDOM OCCASIONS (DAY, NIGHT, SMOOTH ROADS, DRY, WET CONDITIONS), CHECKED CONNECTION AND ALL CONNECTIONS ARE FINE. *TR

The Office of Defects Investigation (ODI) received a January 2013 defect petition requesting that it open an investigation on Model Year (MY) 2004-2009 Toyota Prius vehicles to address a possible manufacturing safety defect related to the steering column intermediate shaft.During the defect petition evaluation, ODI: 1.Reviewed the petition and its enclosures; 2.Reviewed the subject steering system layout; 3.Examined the Petition Vehicle?s history, including its repair history; 4.Reviewed and followed-up potentially related consumer complaints; 5.Assessed Safety Recalls 06V-188 and 12V-537; and, 6.Requested and examined complaint, claim, field report, service history, and warranty information from Toyota.ODI's review revealed that the petition vehicle had been recalled (Recall 06V188) to remedy a defect in the system identified in the petition with the remedy taking place prior to the failure that prompted the petition. Circumstances point to incorrect reassembly of the petitioner's vehicle by those who performed the recall remedy.ODI examined the issues raised by this petition to identify any related safety defect trend in the general subject vehicle population. That review identified a low rate of related consumer complaints (twenty-five over a eight year period from a population of over 600,000 vehicles) and no pattern that would identify either a safety defect not already covered by the two existing steering safety recalls by Toyota or a systematic problem with either recall remedy.Based on this evaluation, ODI does not believe that additional investigation will lead to a finding that a defect related to motor vehicle safety exists in the subject vehicles. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. The denial of this petition does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take further action if warranted by the circumstances. For more information, see the Federal Register notice in the public file for this action (https://www.nhtsa.gov/recalls#vehicle, select NHTSA ID, enter: DP13001)The ODI reports cited above can be reviewed on-line (https://www.nhtsa.gov/recalls#vehicle) under the following NHTSA ID numbers:10407520, 10418710, 10429551, 10473654, 10478195, 10478053, 10437229, 10489425, 10510722, 10639984, 10648277, 10499778, 10712626, 10723196, 10787530, 10808124, 10838803, 11071646, 11098144, 11099507, 11102659, 11160767, 11192607, 11287550, 11287606

ODI OPENED THIS INVESTIGATION AFTER RECEIVING REPORTS THAT BOTH HEADLIGHTS WOULD GO OUT, OR EXTINGUISH WHILE DRIVING.THE SUBJECT VEHICLES ARE EQUIPPED WITH HIGH INTENSITY DISCHARGE (HID) HEADLAMPS THAT OPERATE ON INDEPENDENT LAMPS AND ECU'S (ELECTRONIC CONTROL UNITS) FOR EACH HEADLIGHT, MEANING THAT EACH HEADLIGHT RUNS INDEPENDENTLY OF THE OTHER, AND A FAILURE IN ONE LAMP OR ECU DOES NOT NECESSARILY RESULT IN A FAILURE OF THE OTHER.DURING THE INVESTIGATION, A RANDOM SAMPLE OF FORTY CONSUMERS WAS CONTACTED BY ODI IN A TELEPHONE SURVEY TO VERIFY THEIR EXPERIENCES.THE CONSUMERS STATED IN COMPLAINTS TO TOYOTA THAT BOTH HEADLIGHTS FAILED AT THE SAME TIME.ODI DISCOVERED THROUGH TALKING WITH THESE COMPLAINANTS THAT IN FACT ONE HEADLIGHT WOULD BEGIN TO FLICKER AND THEN CUT OFF.THE OTHER HEADLIGHT WOULD STILL BE OPERATIONAL.IN A FEW CASES WHERE NO ACTION WAS TAKEN, SOME COMPLAINANTS STATED THAT AFTER A FEW MONTHS BOTH HEADLAMPS WOULD BEGIN TO FAIL RESULTING IN NO HEADLIGHTS; HOWEVER NONE REPORTED THAT BOTH LAMPS FAILED SIMULTANEOUSLY.COMPLAINANTS ALSO STATED THAT THEY COULD TOGGLE THE HEADLAMP SWITCH ON AND OFF TO TEMPORARILY RESTORE LIGHTING WHEN THIS CONDITION OCCURRED, EVEN IN CASES WERE BOTH HEADLAMPS WERE EXTINGUISHED.THE RESULTS OF THIS SURVEY ARE LARGELY CONSISTENT WITH ODI'S ASSESSMENT OF THE REPORTS AND WARRANTY DATA.TOYOTA SUBMITTED IN RESPONSE TO ODI'S INFORMATION REQUEST LETTER THAT, GENERALLY SPEAKING, ONLY ONE LAMP FAILED AT A TIME, AND THAT TOGGLING THE SWITCH RESTORED THE LAMP.TOYOTA REQUESTED A MEETING WITH ODI AND ON AUGUST 6, 2009, PRESENTED THEIR ASSESSMENT OF HEADLAMP FAILURE.TOYOTA CONFIRMED THAT A HEADLAMP COULD EXTINGUISH WHILE DRIVING DUE TO A BULB FAILURE, AND THAT IT COULD BE TEMPORARILY RESTORED BY TURNING THE HEADLIGHT SWITCH ON AND OFF, AND THAT SIMULTANEOUS FAILURE OF BOTH HEADLAMPS WAS RARE AND UNLIKELY.TOYOTA ALSO BRIEFLY DISCUSSED THEIR INTENTION TO CONDUCT A CUSTOMER SERVICE CAMPAIGN TO ADDRESS CONSUMER DISSATISFACTION WITH HID BULB FAILURES.SINCE THERE IS STILL ONE HEADLIGHT OPERATING WHEN THE OTHER ONE CUTS OFF, THERE IS STILL LIGHTING AVAILABLE TO GET THE VEHICLE TO A REPAIR FACILITY.THUS, A SAFETY DEFECT TREND HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.
