The contact owns a 2012 Toyota Prius V. The contact stated that while her husband was driving at an undisclosed speed into the dealer's parking lot, the brakes failed to stop the vehicle immediately. The brakes, ABS, and several other unknown warning lights were illuminated. The contact's husband then shifted the transmission into neutral (N) and coasted until the vehicle came to a complete stop. The dealer diagnosed the vehicle with DTC codes: C1252, C1253, and C1256, and determined that the brake booster and ABS pump needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 166,000.

The contact stated that several unknown warning lights were illuminated with the brake pedal going to the floorboard while being depressed. The contact stated that the failure also occurred while starting the vehicle. The contact stated that the failure persisted. The vehicle was taken to an independent mechanic where it was diagnosed and determined that the brake booster and the brake booster vacuum pump needed to be replaced. The vehicle had not been repaired. The manufacturer had been informed of the failure. The contact called the manufacturer and was informed that the failure was related to Customer Support Program: 20TE07 - Brake Booster and Brake Booster Pump Assemblies (Service Brakes); however, the vehicle was not covered under warranty. The contact owns a 2012 Toyota Prius V. The contact stated while driving approximately 40 MPH, the brake pedal was depressed with excessive force; however, the brakes failed to respond as needed. The contact stated that five unknown warning lights were illuminated. The contact then stated that the failure was reoccurring while driving at various speeds. The vehicle was taken to the dealer where an outside technician diagnosed the failure as five unknown fault codes related to the brakes. The contact stated that the brake booster and actuator needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and the contact was informed that the VIN was not included in a recall. The failure mileage was approximately 211,000.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2014 Toyota Prius and 2012-2014 Toyota Prius V vehicles. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.
Consequence & remedy
Consequence: If the hybrid system shuts down unexpectedly, it can increase the risk of a crash.
Remedy: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

A component within the hybrid inverter assembly may overheat, causing the hybrid system to reduce output power allowing the vehicle to only drive a short distance. In some circumstances, the hybrid system may shut down causing the vehicle to stop while being driven.
Consequence & remedy
Consequence: The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.
