I was driving my 2024 CX90 PHEV when a warning about an electrical leakage in the hybrid system was detected. I brought it home and parked in my garage. I called the dealership and they told me to bring it in. When I started to leave, the car would not shift out of park and bricked up. A towing service had to drag it out of my garage. My concern is that there is no limp mode or any way to switch to the gasoline engine when this happened. Had there been a fire in the battery, I would not have been able to get it out of my garage, putting our house and children at risk. Also, had it locked up on a highway, an accident could easily happen. After the dealership dragged it off of the rollback, they couldn’t even get the car to move. They told me that they would have to order special tools to be able to get the car to move. Along with the steering recall that worked for about a month, the steering rack had to be replaced, and the failure of the electrical system, I feel like this car is unsafe to drive. Thank you.

On [XXX], I was heading home from Brooklyn. It was a rainy day. The vehicle was functioning fine when all of a sudden the sign on the dashboard said "Iactive sense deactivated" which is something that always happens when it's raining. It had happened the Thursday prior to the accident when heading to the city and many other times. I had just crossed the George Washington Bridge, on [XXX] , going the speed limit or less, when the the iactive sense sign came up. About 1000 feet after I saw the sign and repeating it to my daughter the vehicle tried to get into the right lane, I was in the left lane of three lanes. I knew it was the iactive sense because i had felt it before many times. I braked and controlled the vehicle back into the left lane. said, "I can see the lane" The vehicle then sped on, moved right, then sped all the way right into the guardrail, all the while I tried to brake and turn left to avoid hitting the guardrail but the wheel was locked and the brake did not work. My right arm got hit by the air bag deploying. After hitting the guardrail we span all the way back into the left lane with the vehicle facing the opposite direction facing on coming traffic. I then hit the hazard lights a few times out of panic, then I had my daughter crawl to me (she was sitting behind me) in the front so we can get out because I thought there was smoke. Turns out it was the airbags' powder, according to the fire department. Another lady who was from the NYPD stopped to see what happened and I explained to her what happened and then the NJ police showed up and she left. When the police got there, I told him what happened. He repeated, "you lost control" at which I said "no the vehicle took control" From my observation the vehicle was like was angry and confused about where to stay and hitting something was the only way it would stop. When driving at highway speed (>60mph), the driver side mirror appears to be vibrating, and when attempting to focus on images in the mirror, the images appear blurred. Over time, the blurring vision starts to make the driver feel dizzy when on long (3+ hrs) road trips. This was initially noticed/experienced on a road trip from Illinois to Norfolk, Virginia, and again on two more trips to Washington DC from Norfolk VA. Needless to say, the issue is persistent every time at highway speeds. The issue was reported to a local dealership in Chesapeake, VA, on October 2nd, 2024. Initially, the dealer said they could not replicate the issue. However, upon going on the test driver with a "senior mechanic," the issue was acknowledged. Dealer also picked a random Mazda CX 90 inline (non PHEV) from the lot, and the issue was again observed on test drive, with a more pronounced vibration in both mirrors. The dealer however declined to address or report the issue. Instead, the dealer maintained that the issue was unique to CX90s, so it is normal in this model. On October 9th, this issue was reported to Mazda USA and was assigned case number [XXX]. Bottom line: this issue may be "normal" in Mazda CX 90s, but that is not normal driving conditions, and poses a safety risk to drivers taking (long) road trips in this vehicle. Mazda CX 90 PHEV. It also causes a buzzing/humming in other appliances and causes led lights in the house to flicker. This is with a 100amp breaker panel and a new upgraded 200amp breaker panel. May cause electrical issues up to electrical fire in other areas of the home. Scheduled diagnostic test in vehicle 10/1/24, but other owners report same issue on Mazda CX 90 online forums.

I was driving in EV mode and was behind another vehicle waiting to turn out of a plaza. The exit is on a slope which is important to mention. The car in front of me began to turn and as I was releasing the brake to move up the hill, the car engine kicked on, the RPM’s went up extremely high as if the accelerator was being pressed all the way to the floor, the car jolted forward on its own and hit the car in front of me. I kept trying to brake, but the car continued to accelerate on its own into the car in front of me a second time. I understand that when on a slope driving in EV mode, the cars engine will kick in to allow the car enough momentum to make it up the hill. However, in this case, the engine kicked in, but the momentum was extreme to the point that when I released the brake to roll forward the car shot forward so fast there was no avoiding a collision. This is extremely faulty and unsafe! My child and I suffered injuries as a result of striking the other car twice. We’re lucky we were not hit by multiple other cars as we were propelled into the intersection. I’m terrified to drive this car with my children. I contacted the dealership that instructed me to have the collision repairs completed first and then they would check out the car out.

The contact stated that on several occasions while driving at various speeds, the message "Vehicle Failure Detected - Hybrid System Malfunction", "Possibility of Restricted Power Output", "I-Activsense System Malfunction", "Dynamic Stability Control Malfunction", and "Push button Start System Malfunction" were displayed. Additionally, the I-Activsense warning light was illuminated. The vehicle was taken to a dealer where it was diagnosed that an unknown software needed to be updated. The vehicle was repaired but the failure reoccurred. The vehicle was taken back to the same dealer where it was diagnosed that the connector to the battery needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 3,960.

July 11, 2024 – Odometer – 7,604, Gas – Full, Electric – 19 Miles range - From work to Home, Receive the following errors “Engine Malfunction, Have Vehicle Inspected” and “Hybrid System Malfunction, Have Vehicle Inspected” warning lights come on. Cars behavior – Air conditioning motor running but not cooling air, Loss of power and car very sluggish (have to be careful at intersections), seems to maintain speed even when taking foot of gas and requires extra breaking, seems to be clunky when transmission upshifts/downshifts at times. When leaving work, did not seem to like being in EV mode even though fully charged (Chargers at work) and would switch itself out of that move and say unavailable. Outside air temperature 90 degree (Air Conditioning not working, very uncomfortable). This defect is dangerous and I serious concerns driving this car such as unpredictable loss of power when entering traffic and hesitation / unpredictability when hitting the accelerator. In addition, with the car maintain speed while not touching the gas pedal and requiring extra breaking is a scary. June 13 – Issue cleared and wanting lights were gone before I was able to take it in for service. July 25th – Recurrence of the same issue - Odometer – 8,162, Gas – Full, Electric – 14 Miles range - From work to Home, Receive the following errors “Engine Malfunction, Have Vehicle Inspected” and “Hybrid System Malfunction, Have Vehicle Inspected” warning lights come on.
