ON MY WAY TO WORK ONE MORNING, I WAS DRIVING ON A THREE-LANE HIGHWAY, GOING ROUGHLY 73 MPH WHEN SUDDENLY, AS I WAS ATTEMPTING TO MAINTAIN MY CURRENT SPEED 'CONSISTENT WITH TRAFFIC FLOW 'MY VEHICLE WOULD NOT ENABLE ME TO ACCELERATE. DURING MY INABILITY TO ACCELERATE, MY VEHICLES ELECTRICAL SYSTEM SURGED, ILLUMINATING MY CHECK ENGINE LIGHT, TRACTION CONTROL LIGHT, AND CABIN LIGHTS. AS A RESULT, IN ORDER TO REMAIN UNINJURED, I CROSSED THREE LANES OF MODERATELY CONGESTED TRAFFIC, AND TURNED MY VEHICLE OFF FOR APPROXIMATELY 5-7 MINS. UPON STARTING MY CAR, I WAS ABLE TO ACCELERATE; HOWEVER, BOTH LIGHTS WERE LIT ON MY DASHBOARD. AFTER DRIVING 130 PLUS MILES, I IMMEDIATELY DROVE TO THE DEALERSHIP I PURCHASED MY VEHICLE FROM'THEY CONDUCTED TESTS; RESULTING IN A COMPUTER GLITCH. THE DEALERSHIP ADDRESSED THIS BY CLEARING THE TRANSMISSION CONTROL MODULE. THIS ANSWER WAS VERY UNSATISFACTORY TO ME AS I PURCHASED THIS VEHICLE BRAND NEW IN JUNE 2014 WITH LESS THAN 100 MILES. THEREFORE, I CONTACTED THE MANUFACTURE AND SOUGHT ASSISTANCE. SINCE, I HAVE BEEN ASKED TO MAKE AN APPOINTMENT WITH 2 DIFFERENT DEALERSHIPS, TOTALING 4 SERVICE VISITS 'ALL INCLUSIVE. OVER ALL, I AM EXTREMELY UPSET AND UNSATISFIED. BUYING A NEW VEHICLE WITH TRANSMISSION ISSUES AND OR COMPUTER ISSUES IS NOT IDEAL OR EXPECTED. NOW, I AM SLIGHTLY PARANOID EVERY TIME I DRIVE MY VEHICLE 'HOPING I WILL BE ALIVE TO RETELL BOTH THE DEALERSHIP AND MANUFACTURE THIS PROBLEM HAS BEEN DUPLICATED. *TR

I WAS DRIVING ON A 4 LANE HIGHWAY SLOWING DOWN FOR A LIGHT THAT HAD GONE FROM RED TO GREEN. AS I ACCELERATED TO GO THROUGH THE LIGHT MY VEHICLE SUDDENLY STOPPED AND WOULD NOT MOVE. IT WAS DEAD IN THE WATER TYPE OF STOP. I PUT ON MY FLASHERS AND TRIED TO START THE ENGINE AGAIN. IT WOULD NOT START. I PUT ON THE BRAKE AND PRESSED THE START BUTTON AND OPENED THE DRIVER DOOR. I WAS ABLE TO START THE ENGINE BUT THE CAR WOULD NOT MOVE. AFTER DOING THIS TWICE I WAS ABLE TO START THE CAR AND MOVE TO THE CENTER OF THE HIGHWAY. A POLICE OFFICER STOPPED TO CHECK ON ME AND ASKED IF I COULD MOVE TO A SAFER PLACE. I WAS ABLE TO MOVE THE VEHICLE TO A PARKING LOT. THE POLICE OFFICER MADE SURE THAT I WAS OK AND LEFT. I THEN CALLED JEEP FOR A TOW. WHAT I SAW WHEN THIS HAPPENED WAS THE NO SKID LIGHT, THE SERVICE TRANSMISSION, AND SEVERAL LIGHTS ON THE LEFT SIDE OF THE SCREEN. IT SHOWED THE BRAKE WAS ON BUT IT WAS NOT ON. THE ENGINE LIGHT WAS ON AND STAYED ON. I HAVE NOT HEARD ANYTHING FROM THE DEALERSHIP AT THIS TIME. IF I HAD BEEN ON A NORMAL WORK DAY I WOULD HAVE BEEN ON THE EXPRESSWAY. THERE IS NO DOUBT THAT SOMETHING FAR MORE SERIOUS WOULD HAVE HAPPENED TO ME. I AM NOT SURE THAT I CAN DRIVE THIS VEHICLE AGAIN. *TR

AS SOON AS IT GOT INTO THE SAND, IT DROPPED TO 2ND GEAR AND RPMS SHOT UP TO JUST OVER 3000. CAR WOULD NOT SHIFT OUT OF SECOND. TEMP WENT UP TO 230. LATER ON ALL COMPUTERS IN CAR STARTED TO FLASH LIKE WHITE NOISE. BROUGHT IT BACK TO THE DEALER AND WAS GIVEN A VARIETY OF EXCUSES. BAD ABS SENSOR, LOOSE FUSE ETC. WENT TO PICK THE CAR UP FROM THE DEALER AFTER HOURS WHEN THEY WERE CLOSED. AS SOON AS I TURNED IT ON, FLASHING WHITE NOISE ON ALL THE SCREENS.I ALSO TOOK IT ON THE BEACH A DAY OR TWO LATER, AND AGAIN CAR WOULDN'T SHIFT OUT OF SECOND GEAR ON THE BEACH. THE CAR NOW BEGAN TO SHIFT VERY ERRATICALLY. IT WOULD UP SHIFT VERY FAST ACCELERATE WHEN MY FOOT WAS NOT ON THE GAS. WHEN COMING TO A STOP IT WOULD DOWN SHIFT SO HARD. WHEN I BROUGHT THE CAR BACK TO THE DEALER, (NOW WITH 700 MILES ON IT) THEY TOLD ME THEY WERE REPLACING THE ENTIRE TRANSMISSION. THEY HAVE HAD THE CAR FOR 3 WEEKS TOTAL. FINALLY PICKED UP THE CAR LAST WEEK.WHEN I GOT INTO THE CAR THE ODOMETER READY 6000 MILES! I DROPPED IT OFF WITH 700.I TOLD HIM FOR THE 3RD TIME THAT I DID NOT WANT IT. I HAVE ALSO BEEN DEALING WITH A JEEP RESOLUTION TEAM FROM CHRYSLER.ITS BEEN A WEEK, THE CAR IS SHIFTING MORE ERRATIC THAN IT HAS SINCE THE NEW TRANSMISSION. THE SUNROOF LEAKS WATER, THE WINDSHIELD IS CONSTANTLY FOGGED AND LOOKS LIKE THERE IS A CLEAR COAT THAT MAY HAVE GOTTEN ON THE CAR THAT MAKES IT IMPOSSIBLE TO SEE WHEN DRIVING. ALSO, WHEN I TURN THE FLOOR HEAT ON, THE FRONT DEFROSTER COMES ON.CHRYSLER CONTACTED ME ON FRIDAY, AND MY JEEP HAS OFFICIALLY BEEN DEEMED A LEMON, AND I AM ENTITLED TO A REPLACEMENT. I DON'T WANT A REPLACEMENT. I WANT A FULL REFUND AND TO HEAD OVER TO THE TOYOTA DEALER. *TR

CAR DOES NOT JERK LEFT WHEN LANE SENSE IS SESING THE LANE PROPERLY (GREEN INDICATION LIGHT). #2-PARK SENSE FLASHES ON THE SCREEN 5-6 TIMES WHILE I?M DRIVING. IT THINKS I?M PARKING WHILE DRIVING THE CAR OR SLOWING THE CAR DOWN INTO A TURN LANE. IT LITERALLY DISPLAYS THE DIAGRAM OF CAR WITH YELLOW BARS SHOWING HOW CLOSE I AM TO SOMETHING WHILE DRIVING OR WAITING IN A TURN LANE, HOWEVER, THERE WERE NO CARS OR OBSTACLES AHEAD OF ME, NOR AM I PARKING. AT ONE POINT I WAS PULLING THE CAR LEFT INTO THE LEFT HAND LANE TO TURN INTO MY NEIGHBORHOOD. I WAS NOT AT A FULL STOP, BUT ALMOST, IN THE LEFT-HAND TURN LANE. PARK SENSE FLASHED 5 TIMES ON AND OFF EACH TIME A CAR PASSED ME IN THE OTHER DIRECTION. #3-LANE SENSE CANNOT SENSE THE LANE 90% OF THE TIME ON MAIN STREETS, ONLY HIGHWAYS. *TR

IMMEDIATELY THEREFORE, A SERIES OF ICONS APPEARED ON THE DASHBOARD IN THIS ORDER: 1) RED LIGHTENING BOLT FOR THE ETC, 2) FRONT END COLLISION LIGHT LIKE I HAD A WRECK, 3) ENGINE LIGHT, 4) TRACTION LIGHT. THE POWER STEERING APPEARED VERY STIFF. CAR WAS STILL RUNNING. DROVE CAR OUT OF PARKING SPACE AND IT WOULD NOT ACCELERATE; IT MOVED AT A VERY SLOW PACE LIKE IT WAS STUCK IN MOLASSES OR QUICK SAND. PLACED CAR IN PARK AND TURNED OFF IGNITION. WHILE SHIFTING FROM D TO P, THE SHIFT WAS SO VIOLENT IT SHOOK THE ENTIRE CAR AND I COULD HEAR GRINDING. LET CAR REST FOR 60-90 SECONDS. TURNED CAR BACK ON AND ONLY THE ENGINE LIGHT REMAINED LIT. POWER STEERING WAS NOT STIFF. DROVE CAR TO DEALERSHIP THE NEXT MORNING. *TR

SHE WENT ON TO EXPLAIN TO ME THAT HER VEHICLE WAS "ACTING" UP AGAIN WITH ALL THE SAME SYMPTOMS AS LAST TIME (FIRST INCIDENT HAS ALREADY BEEN SUBMITTED UNDER SAME INFORMATION). SHE LOST A SIGNIFICANT AMOUNT OF POWER WHILE TRAVELING ON A BUSY HIGHWAY WHERE THE SPEED POSTED IS 65MPH. WHEN THIS HAPPENED SHE PULLED OFF TO SHOULDER AND TOOK PHOTOGRAPHS WITH HER IPHONE TO KEEP FOR OUR RECORDS AND TO WHAT HER DASH WAS DISPLAYING. SHE STOPPED WHILE ON THE SHOULDER AND PUT THE VEHICLE IN PARK AND TURNED OFF THE IGNITION. AFTER SOME TIME SHE TURNED THE IGNITION ON AND PUT THE VEHICLE IN DRIVE WHEN SHE NOTICED THE VEHICLE "JERKED" AND MADE A LOUD CLUNKY NOISE. THIS IS THE SECOND INCIDENT. THE FIRST INCIDENT DEALERSHIP WAS UNABLE TO FIND ANY PROBLEMS UNDER THE DIAGNOSTICS THEY RAN. *TR

UPON FURTHER EXPLANATION SHE INFORMED ME WHILE ACCELERATING TO THE PROPER SPEED FOR THE POSTED SPEED LIMIT OF 55MPH THE VEHICLE HAD LOST POWER AND THE "SERVICE TRANSMISSION" LIGHT ILLUMINATED. SHE THEN PULLED OFF THE BUSY ROAD TO A SAFE A SPOT AND PROCEEDED TO CONTACT OUR INSURANCE AND THE LOCAL JEEP/CHRYSLER DEALERSHIP, AS WE ARE CURRENTLY UNDER WARRANTY. ONCE I ARRIVED TO MY WIFE'S LOCATION WE WAITED FOR THE TOW TRUCK AND UPON HIS ARRIVAL HE ASKED IF THE VEHICLE COULD BE DRIVEN, AT THIS TIME I TURNED ON THE VEHICLE AND PLACED IT IN DRIVE, THE "SERVICE TRANSMISSION" LIGHT WAS STILL ON. I WAS ABLE TO DRIVE IN APPROXIMATELY 3 MILES TO THE DEALERSHIP ALL AWHILE BEING FOLLOWED BY THE TOW TRUCK AND WITHOUT INCIDENT. *TR

20V343000 · Power Train:automatic Transmission; Power Train:driveline:differential Unit

Jun 11, 2020

Chrysler (FCA US LLC) is recalling certain 2014-2017 Jeep Cherokee vehicles equipped with a two-speed Power Transfer Unit (PTU). Relative movement in the Power Transfer Unit (PTU) between the differential input splines and the transmission output shaft may cause some input spline teeth to wear off, which may eventually cause a loss of engagement between the transmission and the differential inside the PTU. If this occurs, power cannot be transferred between the front wheels and the transmission which results in a loss of drive while the vehicle is in motion and a loss of the Park function while stationary.

Consequence & remedy

Consequence: A loss of drive can cause a vehicle crash without prior warning. A loss of the Park function can cause unintended vehicle rollaway which can increase the risk of a crash or injury.

Remedy: FCA US LLC will notify owners, and dealers will perform a software update that will maintain vehicle propulsion by engaging rear wheel drive and prevent rolling in Park by activating the electronic parking brake if a failed input spline occurs. This recall began July 14, 2020. Owners may contact FCA USA LLC customer service at 1-800-853-1403. FCA's number for this recall is W47.

Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. A list of the affected trailer models is available at: https://static.nhtsa.gov/odi/rcl/2017/RMISC-17V824-0103.pdf. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. PE22009 · Electronic Parking Brake Water Ingress

Opened Jul 22, 2022 · Closed Oct 29, 2023

Status: closed (inferred from source dates) · Parking Brake:electrical; Parking Brake:electrical:control Module

The Office of Defects Investigation (ODI) opened investigation PE22-009 after having received 80 complaints on model years 2014-2020 Jeep Cherokees alleging the Electronic Parking Brake (EPB) inadvertently activated while the vehicle was in motion.During this investigation, the subject vehicles were updated to include only Model Years (MY) 2014-2016, while MY 2017-2020 were considered peers.Based on related repair invoices, ODI believed the EPB module was damaged by water leaking into the vehicle. When the EPB inadvertently activates, it can cause a stalling condition bringing the vehicle to a stop. ODI was aware of recall 15V-393 that addresses water ingress affecting the power liftgate module on nearly the same population of vehicles subject of this investigation. The liftgate module can potentially catch fire as a result of the water damage. Both the liftgate and EPB modules are located in a similar area of the vehicle.  On May 11, 2016, the manufacturer released a Technical Service Bulleting (ODI #10191415, FCA #08-060-16) that instructs dealers on replacing the EPB module and inspecting the electrical connector for corrosion if a customer describes having the EPB not releasing.  This Preliminary Evaluation was opened to assess the scope, frequency, and safety-related consequences of the alleged defect. On September 8th, 2022, ODI sent an information request letter to Fiat Chrysler US LLC (FCA) requesting pertinent information on the subject 2014-2016 Jeep Cherokee and Peer 2017-2020 Jeep Cherokee vehicles. FCA Provided its full response on October 27th, 2022 after ODI granted a one-week extension.  ODI has reviewed the information FCA provided as well as new reports submitted to NHTSA’s Vehicle Owner Questionnaire database.  Based on a review of all available information, ODI has identified evidence supporting a total of 1,655 subject vehicles that likely experienced a failure of the subject EPB.  Within the subject vehicle complaints, ODI identified the 2014 Model Year (MY) Jeep Cherokee as having the highest rate of failure, while also being the oldest group of vehicles in this investigation.  The 2014 Jeep Cherokees that likely experienced a failure of the subject EPB represent an incident rate of approximately 0.58%.  The average Time In Service (TIS) at failure was 2.6 years and the oldest failure occurred at 8.5 years in service.  The 2014 population of vehicles is at 9 years in service.  All other MY Jeep Cherokee vehicles (both subject and peer) within this investigation had significantly lower failure rates than the 2014 MY Jeep Cherokee. ODI has reviewed all the information available at this time, and identified four allegations of a crash, three allegations of injuries, and one allegation of a fire. Two of the crash allegations alleged injuries, and the alleged injuries were minor severity. All of the alleged injuries appeared to be consistent with hard braking and did not need medical attention.  There was no supporting information that demonstrated a fire had occurred on the vehicle. In its response to ODI, FCA provided its assessment of the alleged defect in the subject vehicles, stating that it does not believe the alleged defect poses an unreasonable risk to motor vehicle safety.  FCA states the majority of incidents involving the EPB inadvertently activating occurred while the vehicle was stationary and that a small number of incidents occurred while the vehicle was backing up or at low speeds. This is consistent with ODI’s observations. Based on the analysis conducted by ODI, this investigation will be closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. NHTSA reserves the right take additional actions if warranted by future circumstances. To review the ODI reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

EA21002 · Desiccated Air Bag Inflator Rupture

Opened Sep 17, 2021 · No close date supplied

Status: open (inferred from source dates) · Air Bags:frontal:driver Side:inflator Module; Air Bags:frontal:passenger Side:inflator Module

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Air Bags:frontal:passenger Side:inflator Module

On July 23, 2015, Fiat Chrysler Automobiles (FCA) launched Safety Recall 15V-461 to remedy security vulnerabilities in approximately 1.4 million model year (MY) 2013 through 2015 vehicles equipped with Uconnect head units (HU) 8.4A (RA3 radio) and 8.4AN (RA4 radio) manufactured by Harman International. On July 24, 2015, the Office of Defects Investigation (ODI) opened Recall Query, RQ 15-004, to investigate HU security vulnerabilities and remedy effectiveness in the recalled population and to determine whether similar units have been supplied for use in other FCA vehicles. In an August 11, 2015 letter, FCA submitted a second Part 573 safety recall report expanding the scope of the Uconnect RA4 model radio to include additional 7,810 MY 2015 Jeep Renegade vehicles manufactured from September 18, 2014 through June 25, 2015 (Recall 15V-508).Scope analysis indicated that Uconnect radios installed in FCA vehicles not included in recalls 15V-461 or 15V-508 (subject recalls) are not equipped with built-in cellular access or short range wireless communication features and, thus, do not contain the security vulnerabilities addressed by the subject recalls. SUBJECT VEHICLES: MY2014 through 2015 Dodge Durango, Jeep Grand Cherokee and Jeep Cherokee sport utility vehicles; MY2013 through 2015 Ram 1500, 2500, 3500 and 4500/5500 pickup trucks; MY2013 through 2015 Dodge Viper vehicles; and MY2015 Chrysler 200, 300, Jeep Renegade, Dodge Charger and Challenger vehicles.According to FCA, long and short range wireless vulnerabilities identified in the recalled vehicles could allow unauthorized third-party access to, and manipulation of, networked vehicle control systems.Successful exploitation of the vulnerabilities, coupled with reverse engineering of networked microprocessor control modules, could result in unauthorized manipulation of vehicle control systems. This unauthorized manipulation of vehicle controls and systems could expose the driver, vehicle occupants or other highway users to an increased risk of injury.FCA and its network provider, Sprint, conducted a nationwide campaign to block access to a radio communications port that was unintentionally left open.On July 27, 2015, short range wireless vulnerabilities were also blocked. Finally, third party security evaluation and regression testing identified vulnerabilities that were either remedied by Sprint or through updates to the FCA Uconnect software.ODI identified a total of 30 complaints or field reports on unique vehicles submitted by FCA (29) or received by NHTSA (1) alleging incidents of theft from a vehicle or anomalous performance that the owner alleged were caused by, or may have been caused by, remote hacking.Twenty-six (87%) of these reports were submitted after a magazine article was published on July 21, 2015, describing the remote hacking of an FCA vehicle by researchers who were able to affect the operation of various vehicle control systems, including the service brakes, steering, throttle and ignition. Most of the complaints involved vehicle systems that were not safety critical (e.g., complaints related to radio, navigation system, or air-conditioning control) and did not affect vehicle control.Three complaints reported engine stalls. One owner reported sudden unintended acceleration allegedly related to hacking.None of the complaints or field reports reviewed involved the steering and braking vehicle control effects demonstrated by the research hackers prior to the recall. There were no confirmed incidents of hacking in any of the records reviewed by ODI.The remedies completed by Sprint and FCA appear to have eliminated vulnerabilities that mi
