Hello I am reporting a safety concern involving a 2022 Honda Odyssey EX-L that was involved in an accident and declared a total loss. During the incident, I was driving in snowy conditions at approximately 50 mph while entering a split lane. The vehicle began to skid, and shortly afterward I observed warning indicators activating. It appeared that vehicle safety systems, including stability or braking systems, engaged automatically. Over the next several seconds, I attempted to regain control by applying the brakes multiple times and making steering corrections. However, the vehicle did not respond as expected. Despite these inputs, the vehicle continued toward the shoulder and ultimately impacted a barrier. I was unable to regain effective steering control during this period. This raises concern about the behavior and response of vehicle safety systems, including ABS (Anti-lock Braking System), Vehicle Stability Assist (VSA), and related driver-assist features during loss-of-traction conditions. Additionally, I became aware of a recall affecting similar model years related to unintended or improper curtain airbag deployment due to system sensitivity. Given that my vehicle falls within this model range, I believe it is important to evaluate whether system behavior may have contributed to the incident. I am requesting that this incident be reviewed as a potential safety issue and considered alongside similar reports to determine whether further investigation is warranted.

I was at a 4 way stop on a quiet street, I stopped and then proceeded to move forward. As I was moving forward, in the middle of the intersection, the automatic emergency braking system activated which slammed on my brakes, involuntarily, in the middle of the intersection. There were no hazards in front of me that the vehicle would have detected as being a cause for braking. Thankfully, no one was coming through the intersection at the time from the perpendicular sides or else I would have been hit. The braking was so forceful that it caused my phone, that was sitting in my lap to be flung to the floor and was ripped from the charging cord with force. Additionally, to show the force, I had books sitting on the passenger seat that all got thrown to the floor with the impact of the braking system.

Vehicle Make/Model/Year: 2022 Honda Odyssey Date of Incident: 5-1-25 Mileage at Incident: 30,000 Summary of Incident: While my 2022 Honda Odyssey was fully stopped in traffic with my foot firmly on the brake pedal, the vehicle unexpectedly lurched forward and struck the car in front of me. Just before or during the lurch, the brake system warning light appeared on the dashboard. I did not have my foot on the accelerator at any time. The road was flat and weather conditions were normal. The Idle Stop system has not been functioning for several months, so the vehicle was operating in normal engine-on mode. Despite this, the car surged forward from a full stop, indicating a possible failure in the brake system, electronic throttle control, or a software/ECU malfunction. This was a dangerous, uncommanded movement of the vehicle while stopped, which resulted in a minor collision. Thankfully, no one was injured, but this poses a serious safety hazard, especially if it were to occur in crosswalks or at intersections. I will report the issue to Honda the dealership and requested a full diagnostic inspection. I am submitting this complaint because this incident could have caused serious injury or worse and may be part of a broader defect affecting brake or throttle systems on this model.

For approximately 1 year the collision mitigation braking system will intermittently activate even when no hazards or objects are present in front of the vehicle. This has happened regardless of speed or any other conditions and has even happened while accelerating. There are no lights or warnings that present before the system activates. This poses a hazard of losing control of the vehicle or potential causing a crash due to the unexpected and sudden braking. This problem is intermittent and does not happen every time the car is driving but has happened enough to bring the car in to the service center. The first time we were told they couldn't replicate the problem so they wouldn't look into it. The second time they acknowledged a know issue with their collision mitigation system and recommended the camera and radar be recalibrated at a cost of $850. For some reason this failure of the system is not covered under warranty. They also stated the calibration is not guaranteed to fix the problem.

Consequence: Air bags that deploy unexpectedly can increase the risk of injury.
Remedy: Dealers will reprogram the supplemental restraint system (SRS) electronic control unit (ECU) software or replace the ECU, as necessary, free of charge. Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
Consequence & remedy
Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Due to a faulty Media Oriented Systems Transport (MOST) communication coaxial cable connector, the rearview camera image may not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury.
Remedy: Dealers will replace the MOST cable harness and install a straightening cover over the vehicle cable connector, free of charge. Owner notification letters were mailed July 24, 2023. On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign.
