The contact owns a 2021 GMC Yukon. The contact stated that while driving at 70 MPH with family members in the vehicle, the transmission independently shifted into neutral. No warning lights illuminated. The contact was able to steer the vehicle into a residential driveway, where the vehicle failed to restart. The contact requested towing services; however, the initial tow driver failed to arrive after nearly three hours, requiring the contact to call a second tow company. The vehicle was towed to a dealer, where it was diagnosed with engine failure, and it was determined that the engine needed replacement. The vehicle was not repaired, but replacement parts were ordered. The contact associated the failure with NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling). The contact stated that in January 2026, the vehicle was taken to the dealer for the recall repair, but the contact was informed that the vehicle had passed the recall inspection. The manufacturer was notified of the failure, and a case was opened. The manufacturer offered a 90% discount on the repair costs; however, the contact declined the offer and requested a full refund for both towing service fees. The failure mileage was approximately 151,017.

1. What component or system failed or malfunctioned. The Fuel Pump Module failed unexpectedly while the vehicle was being driven. Failure caused the engine to stall, resulting in a complete loss of power and mobility. Unsure if part is available for inspection. 2021 GMC Yukon have a recall N252516900, but our VIN is not included in this. Can our VIN be added to this recall please? 2. How was your safety or the safety of others put at risk? The vehicle abruptly stalled while being driven through a parking lot, causing a complete loss of power and leaving the vehicle disabled in an active traffic area. Because the vehicle could not be shifted out of PARK, it became an obstruction, creating a hazard for other drivers navigating the parking lot and increasing the risk of a collision. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. After the vehicle was towed to a local mechanic on a flatbed truck, the dealer diagnosed and confirmed that the Fuel Pump Module had failed, which caused the vehicle to stall and lose power. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by a local repair shot, which diagnosed the failed Fuel Pump Module. The vehicle was also evaluated during the towing process. To my knowledge, the manufacturer, police, or insurance representatives have not separately inspected the vehicle or component. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Prior to the failure, there were no significant symptoms indicating an imminent fuel system failure. When the incident occurred, the vehicle abruptly stalled, lost all power, and displayed a message indicating that the vehicle was "not in a condition to shift," preventing it from being shifted out of PARK. This message first appeared immediately following event.

I am writing to report my 2021 GMC Yukon Denali. This vehicle recently suffered a catastrophic failure of its 6.2L V8 engine while traveling on the freeway with my family inside, creating a severe and terrifying safety hazard. This failure occurred after my vehicle was supposedly remedied under GM’s official safety recall for the known L87 engine defect (Recall N252494001). As the National Highway Traffic Safety Administration's (NHTSA) recently escalated Engineering Analysis reflects, the recall inspection and 0W-40 oil remedy are failing to prevent these engines from destroying themselves. My experience is direct evidence of this continued safety defect. GM has since now fully replaced my engine however only reimbursed me 30% of what it cost to tow to their dealership. I am filing this complaint regarding repeated catastrophic engine failures in my 2021 GMC Yukon Denali equipped with the 6.2L L87 V8. I purchased this vehicle as a GM Certified Pre-Owned vehicle. At approximately 7,000 miles, the engine developed a knocking noise and was determined to require complete replacement. I brought the vehicle to the dealership on May 19, 2025, and it remained there until September 17, 2025—nearly four months—while the replacement engine was obtained and installed. Since receiving the replacement engine, I have driven approximately 6,000 miles. On August 4, 2026, I was informed by the dealership that the replacement engine has also failed and requires another complete engine replacement. This means my vehicle has now experienced two major engine failures at extremely low mileage, including failure of the replacement engine. I understand that the 6.2L L87 engine has been subject to a GM recall involving potential engine failure and loss of propulsion. I have now learned that NHTSA has escalated its investigation into continued failures involving these engines, including failures occurring after the recall remedy. My experience appears directly relevant to that investigation. I regularly transport my children in this vehicle and am extremely concerned about the possibility of an engine failure occurring while traveling at highway speeds or with my family in the vehicle. I feel fortunate that the previous failures did not occur under those circumstances. I have contacted GM and requested that they repurchase or replace my vehicle. GM denied my request and has refused to provide me with a reason for the denial. I am asking NHTSA to document my complaint and investigate whether the existing recall remedy is adequately addressing the underlying safety defect, particularly given that my replacement engine failed after only approximately 6,000 miles.I can provide repair orders, recall documentation, mileage records, and correspondence with GMC

As I was driving, the vehicle's air conditioning suddenly began blowing warm/hot air. Moments later, the check engine light and all electronic service stability lights illuminated on the dashboard. Simultaneously, the vehicle suffered a complete loss of electronic brake assist, resulting in an extremely hard brake pedal and a severe reduction in braking power. The vehicle also displayed a "No Drive Mode Available" message. Third-party diagnostic scanning revealed network power codes U3018, U3505, P00FF, and P0700, indicating a total loss of power to the Body Control Module (BCM). This is the second life-threatening incident with this vehicle I have experienced following a previous highway engine stall due to engine recall. The vehicle was towed to a GM dealership for diagnostic and emergency electrical repairs.

It now needs the engine replaced and does not fall under the recall vin numbers acknowledged for the same issue.

On July 28, 2026, while traveling on vacation with my family, our vehicle experienced a catastrophic engine failure. There were warning lights displayed for 30 seconds prior to the engine failure. On July 23, 2026, one day before taking the vehicle to the dealership, the following warning indicators appeared on the dashboard and then disappeared: “Service ESC,” “Service Traction Control,” and a flashing engine light. We took the vehicle to the dealership on July 24, 2026, because we wanted to be proactive and ensure that the vehicle was safe to operate. The dealership inspected the vehicle but reported that no diagnostic trouble codes were identified and no issue was found. We picked the vehicle up from the dealership on the evening of July 27, 2026. On July 28, 2026, while traveling on the highway approximately 2.5 hours from home, the same warning messages appeared on the dashboard again. Because we were traveling at highway speeds and vehicle speed management was becoming an issue, I immediately exited the highway and stopped in a parking area. I placed the vehicle in park, at which point the engine began idling extremely rough. I could feel the vehicle shaking while sitting in the driver's seat. I exited the vehicle to open the hood, but before I was able to do so, I noticed gray smoke coming from the exhaust. We located a GMC dealership approximately two miles away. Because of the vehicle's condition, we drove there slowly on the frontage road at approximately 15 mph and dropped the vehicle off for diagnostic inspection. The GMC dealership subsequently diagnosed the vehicle with catastrophic engine failure and recommended that the engine be replaced. The vehicle remains at the dealership and is available for inspection.We made a reasonable effort to have the vehicle inspected before our trip because we were concerned about the warning indicators and wanted to ensure that the vehicle was safe to travel.

The connecting rod and/or crankshaft engine components may have manufacturing defects that can lead to engine damage and engine failure.

Consequence & remedy

Consequence: Engine failure increases the risk of a crash.

Remedy: Dealers will inspect the engine, and as necessary, repair or replace the engine. For vehicles that pass inspection, dealers will add higher viscosity oil, install 6 oil fill cap, replace the oil filter, and update the owner's manual. Repairs will be performed free of charge. General Motors, LLC (GM) is recalling certain 2021 Yukon, Yukon XL, Chevrolet Tahoe, Chevrolet Suburban, Cadillac Escalade, and Cadillac Escalade ESV vehicles. The daytime running lights (DRLs) may not deactivate when the headlights are on. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, "Lamps, Reflective Devices, and Associated Equipment."

Consequence & remedy

Consequence: Daytime Running Lights that do not deactivate as intended may cause a glare, reducing visibility and increasing the risk of a crash.

Remedy: Dealers will provide an over-the-air (OTA) update for the Body Control Module software, free of charge. Owner notification letters were mailed December 15, 2022. General Motors, LLC (M) is recalling certain 2021-2022 Cadillac Escalade, Escalade ESV, Chevrolet Suburban, Tahoe, GMC Yukon, and Yukon XL vehicles equipped with third-row seats. The rivet that retains the buckle to the mounting bracket in the left or right side third-row seat belt buckle assembly may have been improperly formed.

Consequence & remedy

Consequence: A seat belt with a deformed rivet may not properly restrain an occupant, increasing the risk of injury during a crash.

Remedy: Dealers will inspect the rivet head formation on both the left and right side third-row seat belt buckle assemblies and replace seat belt buckle assemblies, as necessary, free of charge. The communications gateway module incorrectly processes a loss of communication with the sensing diagnostic module (SDM), causing the air bag malfunction indicator light to inconsistently illuminate.

Consequence: The air bag malfunction indicator light may fail to warn the driver of a problem with the air bag, increasing the risk of injury.

Remedy: The communications gateway module software will be updated by a dealer, or through an over-the-air (OTA) update, free of charge. Owner notification letters were mailed on July 1, 2021. On April 9, 2021, the Office of Defects Investigations (ODI) opened PE21-010 to investigate instances of air bag malfunction indicator light (MIL) illumination and potential for frontal air bag non-deployment in several vehicles manufactured by General Motors (GM). The vehicles included model year (MY) 2020 through 2021 CT4 and CT5 passenger cars, Sierra and Silverado trucks (1500/2500/3500 models), XT4 and MY 2021 Escalade, Suburban, Tahoe, Yukon SUVs.Technical Service Bulletin (TSB) 21-NA-005, issued in March 2021, addresses air bag MIL illumination with diagnostic trouble codes (DTCs) B0001-1B or B0012-0D. It cites rust particles in the connection terminal interface of the driver air bag inflator housing as the cause of the air bag MIL illumination. Illumination of the air bag MIL under these circumstances may result in a non-deployment of the frontal driver air bag during a frontal collision and increased risk of injury to the driver.GM stated a correlation exists between production delays due to the pandemic shutdown and premature corrosion initiation on the surface of the air bag inflator housing, which, in some cases, caused rust development during a seven (7) month period of vehicle production. In the rare instance when a rust particle dislodges from the driver air bag inflator housing and enters the air bag connector prior to assembly, an intermittent resistance spike at the air bag connector may result. The condition generates excessive resistance in the air bag electrical system when the steering wheel horn cover is manipulated or the steering wheel is turned. This excessive resistance is momentary. A crash commanding air bag deployment would need to occur exactly when the momentary excessive resistance occurs. Thus, there is a very low likelihood the hazard will present a risk during a crash of sufficient severity to warrant a commanded deployment. This failure mechanism occurs early in life and results in an air bag MIL illumination that corresponds to a DTC warning the driver that the vehicle requires service. The vehicle's air bag remains functional while the MIL is illuminated, and the DTC is present.Once the connection of the driver air bag module to the clockspring is made at the plant, or at the dealership for service, there is no longer an opportunity for rust migration into the connection point. The connector is sealed and has gold terminals, eliminating the expectation of fretting corrosion. All subject vehicles passed standard corrosion validation testing for extensive corrosion with no issue of air bag non-deployment.There is a low rate of occurrence: 1.41 incidents per thousand vehicles (IPTV) for all related occurrences, and 0.04 IPTV for occurrences with a mention of visible rust and/or corrosion on the air bag inflator housing. The average occurrence mileage for the subject vehicles is approximately 2,100. Every occurrence related to the alleged defect experienced an air bag MIL illumination and there are no verified allegations of air bag non-deployment within all data sources.To date, there have been no reported fires, crashes, injuries, or fatalities related to this issue. In view of the low rate, and the nature of early-life failure, the high detectability of failures, and low potential hazard to drivers, this Preliminary Evaluation (PE) is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.
