VEHICLE HAD A SLIGHT MISFIRING WHEN CLIMBING INCLINE. WE REPLACED TWO SPARK PLUGS. MISFIRING CALMED DOWN. A COUPLE DAYS LATER MISFIRING RESUMED. HUSBAND WAS DRIVING DAUGHTER TO DAYCARE WHEN CAR HAD MULTIPLE DTS CODES APPEARED (STABILITY TRACK, CHECK ENGINE, REDUCED POWER) WITH CAR SUDDENLY STALLING WHILE ON A HIGHWAY LEAVING BARELY ENOUGH TIME TO COAST OFF THE ROAD AND OUT OF THE WAY OF TRAFFIC TRAVELING 60MPH. GMC REFUSED TO ASSIST US ON THIS ISSUE UNTIL WE FOUND ONLINE A TSB ABOUT THE TIMING CHAIN WHICH HAD BROKEN. DUE TO THE CONSTANT MISFIRING (DIRECT RESULT OF THE TIMING CHAIN) TWO OF MY CATALYTIC CONVERTER BECAME CLOGGED. THOSE HAVE NOW BEEN REPLACED OUT OF MY POCKET HOWEVER WHEN DRIVING VEHICLE HOME WHILE IT DOES RUN SMOOTHER IT NOW MISFIRES ALL THE TIME EVEN WHEN NOT GOING UP HILLS AND IDLING VERY LOW AND MISFIRING WHILE IDLING. I AM SCARED THAT WHILE DRIVING MY VEHICLE THIS MAY HAPPEN AGAIN BECAUSE THE ISSUE HAS BECOME WORSE. *JS

AS MY WIFE PULLED OUT OF THE DRIVEWAY, SHE HEARD A "CLUNK" NOISE, AND SHE HAD TO APPLY A LOT OF EFFORT TO STRAIGHTEN OUT THE VEHICLE. MY WIFE HAS STATED THAT THIS IS ALMOST A COMMON ISSUE WITH THE STEERING AND MANY TIMES SHE HAS TO USE A LOT OF FORCE TO STEER THE VEHICLE IN A STRAIGHT LINE. WE TOOK THE VEHICLE TO THE DEALERSHIP WHO STATED THAT THIS IS A COMMON OCCURRENCE AND THAT IT WOULD COST $400 TO REPAIR THIS PROBLEM. WE DECLINED THE REPAIR AND FELT THAT THIS WAS AS SAFETY ISSUE AND THAT IT SHOULD BE REPAIRED WITHOUT ANY COSTS TO US. WE HAVE FILED A COMPLAINT WITH THE GENERAL MOTORS COMPANY HOWEVER WE HAVE ONLY RECEIVED A FORM TYPE E-MAIL INDICATING THAT OUR COMPLAINT IS IN THEIR FILES. WE HAVE ALSO INFORMATION FROM OTHER WEB SITES THAT THIS IS A COMMON ISSUE, HOWEVER NOTHING HAS BEEN DONE AT A FEDERAL LEVEL. WE ONLY HAVE 35,000 MILES ON THIS VEHICLE AND NOW WE ARE REALLY AFRAID TO TAKE ANY KIND OF LONG HIGHWAY TRIPS FOR FEAR THAT THIS WOULD HAPPEN AT HIGHWAY SPEEDS AND THAT THIS MAY RESULT IN SERIOUS INJURY AND OR DEATH OF OURSELVES AND OUR LOVED ONES. PLEASE ADVISE AS TO WHAT OTHER ACTIONS WE CAN TAKE TO RESOLVE THIS SAFETY ISSUE. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) *JS

FIRST PROBLEM WAS THE TIMING CHAINS NEEDING TO BE REPLACED AT 18000 MILES. AFTER THAT REPAIR WAS MADE THE GAS MILEAGE WAS AVERAGING 11 MPG. IT WAS A ONE YEAR OLD CAR! AFTER THAT, IT WAS A MULTITUDE OF PROBLEMS. STEERING RACK/POWER STEERING REPLACED AND 'FIXED', ELECTRIC STABILITRAK LIGHTS GOING ON AND OFF, DVD PLAYER NOT PLAYING PROPERLY. ALL THINGS THE DEALER HAD SAID THEY FIXED (AFTER THEY WHISPERED THAT I SHOULD NOT HAVE BOUGHT AN AMERICAN CAR.) NOW THE TRANSMISSION IS ACTING UP. ROLLING DOWN AN INCLINE WHEN I WAS TRYING TO BACK OUT OF A PARKING SPOT. THE DEALER TOLD ME IT WAS FINE AND THEY COULD NOT DUPLICATE THE ISSUES I WAS HAVING. I'VE WRITTEN TWO LETTERS TO GM CORPORATE TO COMPLAIN ABOUT THE SUB-STANDARD VEHICLE. THEY WERE HELPLESS AND WILL NOT ASSIST IN ANY REPLACEMENT OF THE CAR. THEY EVEN TOLD ME THAT THIS WOULD BE A DIFFICULT CAR TO TRADE IN CONSIDERING THE PROBLEMS I HAVE HAD WITH IT. WHO IS KIDDING WHO? I WOULD NEVER RECOMMEND THIS CAR AND HATE TO CONTINUALLY PUT MY FAMILY IN IT .IT IS UNSAFE. PERIOD. *TR

I BELIEVE I HAVE SOME TYPE OF ELECTRICAL ISSUE. I STARTED TESTING DIFFERENT PARTS OF THE DASHBOARD LIGHTS SUCH AS THE DRIVER SEATBELT. I PURPOSELY DROVE SEVERAL MILES IN MY NEIGHBORHOOD WITH THE SEATBELT OVER ME, BUT NOT ENGAGED AND THE LIGHT DID NOT COME ON. MY AIRBAG SERVICE LIGHT CAME ONE FOR SERVICE, THEY SERVICED IT, IT CAME BACK ON, THEY REPLACED A SEATBELT COMPONENT, THE LIGHT WAS OFF, BUT CAME BACK ON, THEY DID ANOTHER REPLACEMENT AND THE LIGHT IS NOW OFF. MY FRONTEND WAS MAKING FUNNY NOISE WHEN MAKING TURNS, THEY REPLACED MY RACK AND PINION. MY SERVICE STABILITRAK LIGHT, SERVICE TRACTION CONTROL LIGHT CAME ONE, THEY FIXED IT, THEY CAME ON AGAIN, THEY KEPT MY VEHICLE FOR A DAY AND FIXED IT. THEY ARE BACK ON AGAIN ALONG WITH THE ABS LIGHT AND NOW I'M TOLD THAT I PROBABLY NEED A BREAK JOB. BREAKS WERE NOT MENTIONED THE OTHER TIMES. MY ELECTRONIC KEY PAD DETERIORATED AND NO LONGER WORKS, THEY SAID IT WAS USER HANDLING, MY OTHER KEY PAD IS NOT MALFUNCTIONING, YOU CAN SEE WERE THE PAD IS BEGINING TO SINK IN, BUT THEY SAID IT'S MY HANDLING. THE CASE IS CHIPPED AND THE CHIPPING HAS OCCURRED IN THE SAME PLACE ON BOTH KEY PADS. MY MIDDLE SEAT PASSENGER SIDE CASE CAME APART TWICE, EACH TIME THEY FIXED IT, IT'S NOW BROKEN, AND I WAS TOLD THEY DON'T REPAIR BROKEN PARTS. I AM STILL UNDER THE BUMPER TO BUMPER WARRANTY, BUT GMC WILL NOT REPLACE MY KEY PADS, AND THE SERVICE TECH SAYS HE WILL TRY REALLY HARD TO GET MY SEAT COVERED. THE TINT ON MY WINDOWS HAVE BEEN REPLACED THREE TIMES AND STILL IS NOT CORRECT. MY DVD PLAYER DOES NOT PLAY CORRECTLY AND HAS NOT BEEN FIXED SINCE IT'S BEEN IN MY VEHICLE. MY VEHICLE IS A 2009 ACADIA. I HAD A LEAK IN THE TRUNK, THEY FIXED THAT. AIR PRESSURE WILL NOT STAY CONSISTENT, WAS TOLD BY SERVICE ADVISOR TO OVER INFLATE TO COMPENSATE, WHICH IS DIFFERENT FROM WHAT THE OWNERS MANUAL STATES. TIRE PRESSURE ISSUES NEEDED SENSOR REPLACED AFTER SEVERAL TRIPS TO SERVICE DEPARTMENT. THE VEHICLE IS AN EXPENSIVE PIECE OF JUNK. *TR

GENERAL MOTORS IS RECALLING CERTAIN MODEL YEAR 2009-2010 CHEVROLET TRAVERSE, BUICK ENCLAVE, GMC ACADIA, AND SATURN OUTLOOK VEHICLES. THE SECOND ROW SEAT SIDE TRIM SHIELD RESTRICTS THE UPWARD ROTATION OF THE SAFETY BELT BUCKLE WHEN THE SEAT BACK IS RETURNED TO A SEATING POSITION AFTER BEING FOLDED FLAT. IF THE BUCKLE CONTACTS THE SEAT FRAME, ADDITIONAL EFFORT IS REQUIRED TO RETURN THE SEAT TO A SEATING POSITION. IF SUFFICIENT FORCE IS APPLIED, THE BUCKLE COVER COULD BE PUSHED DOWN THE STRAP, EXPOSING AND PARTIALLY DEPRESSING THE RED RELEASE BUTTON.

Consequence & remedy

Consequence: THE SAFETY BELT MAY NOT RESTRAIN THE OCCUPANT AS INTENDED DURING A CRASH, INCREASING THE RISK OF INJURY TO THE SEAT OCCUPANT.

Remedy: DEALERS WILL MODIFY THE SECOND ROW SEAT SIDE TRIM SHIELDS. IF A SAFETY BELT BUCKLE IS DAMAGED, DEALERS WILL REPLACE IT FREE OF CHARGE. THE SAFETY RECALL BEGAN ON AUGUST 18, 2010. OWNERS MAY CONTACT CHEVROLET AT 1-800-630-2438, BUICK AT 1-866-608-8080, GMC AT 1-866-996-9463, AND SATURN AT 1-800-972-8876, OR AT THE OWNER CENTER AT WWW.GMOWNERCENTER.COM.

Status: closed (inferred from source dates) · Seat Belts; Seat Belts:front; Seat Belts:front:anchorage

On August 31, 2020, the Office of Defects Investigations (ODI) opened Engineering Analysis (EA) EA20002, as an upgrade of Recall Query (RQ) RQ19003. This investigation, related to recall 14V-266, was opened to investigate instances of detachment of the flexible steel seat belt anchor cable attached to the outboard side of the front seats in vehicles manufactured by General Motors (GM). The subject vehicles included model year (MY) 2009 through 2014 GMC Acadia, Buick Enclave, and Chevrolet Traverse SUVs as well as MY 2009 through 2010 Saturn Outlook SUVs. These vehicles were previously repaired for this defect under recall 14V-266. The subject vehicle population is approximately 1.3 million vehicles. Special Coverage N192219480, issued in June 2019, addressed the subject defect in vehicles that previously received the recall remedy repair for subject recall 14V-266. The Special Coverage allows for component replacement for 12 years or 180,000 miles from the initial sale date, whichever comes first. Special Coverage N192219480 instructed service technicians to inspect the driver’s seat belt anchor cable for any damage to the protective sleeve and if damage was observed, the cable was replaced with a new sleeve as well as a new closeout piece that relocated the cable to mitigate the subject condition. In view of the Special Coverage conducted by General Motors, the high detectability of the subject failure, and the low rate of occurrence, this Engineering Analysis is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. To review the ODI reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov. Please see the attached document “EA20-002 Detailed Closing Summary” for more a detailed closing resume summary.

RQ19003 · Seat Belt Anchor Cable Failure

Opened May 28, 2019 · Closed Aug 31, 2020

Status: closed (inferred from source dates) · Seat Belts:front; Seat Belts:front:anchorage; Seat Belts:pretensioner

On May 19, 2014, General Motors LLC (GM) notified the Office of Defects Investigation (ODI) that it would conduct Safety Recall 14V266 to address fatigue and separation of the flexible steel cable that connects the front outboard seat belts to the seat-mounted belt pre-tensioner in 1.4 million model year (MY) 2009-2010 Saturn Outlook, 2009-2014 Buick Enclave, GMC Acadia and Chevrolet Traverse (Lambda Platform) vehicles. The remedy was to inspect for signs of damage and replace pre-tensioner anchor cables as needed. Re-work was conducted to relocate the pre-tensioner anchor cable out of the occupant entry path. ODI has subsequently received four (4) Vehicle Owners Questionnaires (VOQ) reporting anchor cable separations with no apparent warning in covered vehicles after the Recall 14V266 remedy had been applied.ODI opened RQ19-003 to assess the effectiveness of the actions GM took in Recall 14V266 and review the full warranty and complaint data related to seat belt anchor cable separations and replacements. ODI continues to receive occasional cable failure complaints both for the subject vehicles and other GM models and model years that have also been previously recalled addressing this condition.Although the cable separation complaint rate is very low, over the past three years ODI has received two (2) additional VOQ?s citing belt cable separations for Lambda platform vehicles outside the scope of recall 14V266, (11217092, 11118075) and complaints for other GM models covered in a number of similar Safety Recalls:* 15V269, 2011-12 Chevrolet Malibu (11181115, 11288168); * 16V209, 2014-15 Silverado 1500, Sierra 1500 (11254364, 10935183, 10953851, 11025376, 11282043, 11288701, 11309884); and * 16V518 2014-16 Caprice Police Pursuit, SS (10905644, 10990669, 11340893).ODI's review of GM's data confirms the low potential failure rate for the vehicles covered by recall 14V266. However, ODI believes that complainants may not recognize accumulating cable fatigue damage from the visual cues available in routine vehicle use.ODI is upgrading RQ19-003 to an Engineering Analysis to more closely assess seat belt conditions and strength levels in the field. This work will include the collection of seat belt samples from a wider range of recalled and peer vehicles for detailed analysis and testing. The ODI reports (VOQs) associated with this RQ19-003 can be viewed at NHTSA.gov under the following reference numbers: 11144704, 11076704, 11185619, 11151922, 11164011, 10967518, 11113486, 11115648

DP12003 · Loss Of Headlamp Illumination

Opened Jun 4, 2012 · Closed Jul 24, 2014

Status: closed (inferred from source dates) · Exterior Lighting:headlights

As of July 16, 2014, out of a population of 248,453 subject vehicles, NHTSA received 473 consumer complaints of inoperative head lamp. Many of these complaints indicated that the head lamp harness suffered damage from overheating. After reviewing the complaints, ODI found: -69% (328) alleged that a single head lamp was inoperative. -18% (86) alleged that both head lamps were inoperative but not at the same time. -9% (41) alleged that both head lamps were inoperative but the complaints did not indicate whether the failures had occurred at the same time. -4% (17) alleged that both head lamps were inoperative at the same time. -One additional complaint cited wire harness damage to both sides but did not specify an outage -No crashes or loss of vehicle control were reported. -Any alleged thermal damages were limited to melting of the head lamp harness and the head lamp housing. -A head lamp would intermittently fail to come on or flickers before becoming completely inoperative. For the seventeen complaints that alleged simultaneous failure of both head lamps while attempting to turn them on or while driving, the head lamp failures likely had occurred one at a time. The subject vehicle's head lamps are connected in a parallel circuit and each circuit is fused. Therefore, failure of one head lamp or its harness should not affect the other head lamp operation. Over the last 25 years, ODI has opened numerous defect investigations of the loss of headlamp illumination.Investigations that resulted in safety recalls involved simultaneous loss of illumination from both headlamps. NHTSA does not consider the loss of a single headlamp as presenting an unreasonable safety risk-such failures are readily detectable by the driver while allowing the vehicle to retain forward visibility and conspicuity from the remaining headlamp. There is typically enough time between the failure of the first headlamp and the second during which the vehicle operator can obtain the needed repairs. Based on the information currently available, NHTSA does not believe that the headlamp condition as alleged by the petitioner indicates the likelihood of a safety-related defect that would warrant a formal investigation. Therefore, in view of the need to allocate and prioritize NHTSA's limited resources to best accomplish the agency's safety mission, the petition is denied.Further specifics can be found in the Federal Register Notice ODI complaint ID numbers are referenced on the next page.

AQ10001 · Fleet Vehicle Recall Completion Audit

Opened Nov 18, 2010 · Closed Dec 28, 2015

Status: closed (inferred from source dates) · Unknown Or Other

On November 18, 2010, NHTSA's Recall Management Division (RMD) opened an audit query (AQ10-001) to investigate recall remedy completion by rental car companies on safety recall campaigns. It was expected to provide the agency an indication of how completely and how quickly rental car fleets perform necessary recall-related remedies on the vehicles owned and then rented/leased for use on the roadways.RMD queried three (3) major motor vehicle manufacturers (Ford, Chrysler, and General Motors) and requested data regarding completion figures for fifteen (15) discreet safety recalls.Analysis of this data demonstrated that rental car recalls completion was neither complete nor consistent. The rental car companies representing the largest market share in the industry were then queried as to their respective policies for the rental, lease, and sale of vehicles with open recalls. The data and responses from the manufacturers and the rental car companies can be found in the document repository by accessing the ?Associated Documents link above. In December 2015, Congress enacted the Fixing America's Surface Transportation (FAST) Act.One section of the Act prohibits rental car companies from renting, leasing, or selling motor vehicles subject to open safety recalls. With the passage of the FAST Act and its prohibition, there is no continuing need for the agency to investigate rental car recall completions.Accordingly, this investigation is closed. The agency reserves the right to open any future investigations, including audits of individual recalls, or groups of recalls, or rental car companies, to examine more closely safety recalls performance and completion issues in this area.
