The flexplate has failed twice since a dealer-performed transmission replacement in June 2024, once in August 2025 and again in August 2026. A flexplate failure causes sudden loss of the ability to move the vehicle under power, which is a safety hazard, particularly while merging, accelerating onto a highway, or in traffic. A Ford service bulletin covering the closely related Escape with the same engine and transaxle architecture documents that flexplate cracking is linked to engine-to-transaxle alignment issues that arise whenever the engine and transmission are separated, which is exactly the service performed during a transmission replacement. This report is filed because the recurrence of an identical failure mode, on a known-affected platform, without resolution by the servicing dealer, appears to reflect an unresolved and potentially widespread mechanical defect rather than an isolated incident."

The engine experienced a sudden failure that caused the vehicle to become completely inoperable. Prior to the failure, the vehicle exhibited intermittent hesitation, rough idling, and loss of power during acceleration. These symptoms began several weeks before the complete failure. No warning lamps were illuminated at first, but shortly before the breakdown, the vehicle displayed a check engine light and a “Service Engine Soon” message. While driving, the vehicle abruptly lost power and became unsafe to operate. I was forced to pull over immediately, which created a safety risk to myself and other drivers due to the sudden deceleration and inability to maintain speed in traffic. The vehicle would not restart afterward. A repair facility has confirmed that the engine has suffered a major internal failure and requires replacement. The estimated cost of repair starts upwards of $5,000. The vehicle is currently non‑drivable and remains available for inspection. The issue has been reproduced and confirmed by an independent service center. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives. However, I have found multiple similar complaints from other owners reporting the same type of powertrain failure, suggesting a potential pattern or defect affecting safety.

severe coolant intrusion issue where coolant leaks into the cylinders, causing rough running, overheating, and potential engine failure. Very unsafe especially due to the fact the their were recalls on the 2017-2019 but not for 2016 models and ford refuses to replace the engine.ford reproduced a better block however my 2016 isn't covered. There signs of rough idling and a engine code for a failed coil . Replaced all coils and spark plugs just for the car to still ride rough with no warning signs. Ford knows themselves that there was a flaw in the making the recall for the 2017-2019(21N12) . This is a major issue considering several reviews or reddit consumers have encountered the exact issue at 100k miles and less. Even when I took my car to bill estes they only found a missfire due to coil. Nothing specified a coolant intrusion.

I was driving my car one day and as I was coming down the road the car jerked and picked up speed like on its on and I had to really stop abruptly to stop the car. I thought that was strange, I didn't have any warning lights or any indication that anything was going on I just thought it was weird, then like 2 days later as was driving the car would do this random jerk but again no lights on any indication anything was wrong so I went to work car seemed fine, got on the high way to go pick up my grand kid after work and when I signaled to change lanes attempted to accelerate to pass, the car just hummed and my speed started to diminish and I immediately coasted to nearest exit because I could not accelerate more than 15 mies, I coasted to a strip mall turned the car off waited til I thought the car settled for a moment and attempted to start the car and when I put the car in gear it jerked but as I tried to accelerate it just hummed and no speed overc15 . So I left the car got it towed the next day to another shop because any ford dealerships I called to see if I could tow for diagnostic test said they had extensive waiting period like one had weeks before they could look at the car. So I had it towed to another car lot who had a good business rating to be told that it seems to look like something had came loose from the transmission. That i may have to get it replaced, this car starts right up, didn't give me any indication of a problem and doesn't even have a hundred thousand miles on it. Where did ford drop the ball on this car? Im very disappointed , I attempted to drive the car around the corner one day trying to explain to my neighbor the weird behavior of the car and the transmission piece I was told that cane loose, and the car ran fine for about 20 minutes even picking up speed and everything then the moment I got back on my block its like it got warm and then we had to push it back on the driveway it went back to not being able to accelerate.again

The contact owns a 2016 Ford Fusion. The contact initially received notification of NHTSA Campaign Number: 22V413000 (Power Train). The vehicle was taken to the dealer, and the recall repair was performed. Recently, the contact stated that upon starting the vehicle, the gear shifter failed to shift out of park(P), with the gear shift indicator on the instrument panel indicating that the vehicle was in neutral(N). Due to the failure, the vehicle was towed to the dealer, and it was determined that the failure was caused by a defective shifter cable bushing, and the contact was provided an estimate for the repair. The vehicle was repaired. The manufacturer was notified of the failure but offered no assistance. The failure mileage was 225,678.

Upon inspection, the contact discovered that the transmission shifter cable bushing had detached. The local dealer was contacted and arrived at the residence and replaced the transmission shifter cable bushing and the vehicle was repaired. The contact was informed that the VIN was not included in NHTSA Campaign Number: 22V413000 (POWER TRAIN). The contact stated that the vehicle had experienced the failure listed in the recall. The manufacturer was not notified of the failure. The failure mileage was 125,000.

The shifter cable bushing failed after being replaced under recall 7 years ago. When leaving a parking spot and trying to shift into drive my shift lever started to move freely and my digital PRNDL display was all white letters (usually the selected gear is lit up orange). No warnings displayed on my dash. I sensed something was wrong so I applied the parking brake and shut off the engine. I’m thankful this happened when I was leaving a parking spot and not when I was parking my car and getting out (potentially causing the car to roll away with my kids running behind the car on my hilly driveway). This is a known problem that Ford has issued multiple recalls for in the past. My car was covered by one early recall in 2018 (I will upload the Carfax service history showing date and mileage of 2018 recall repair). An orange plastic shifter bushing was installed under recall in 2018. This bushing disintegrated eventually, leading to the problem I experienced a few days ago. I will upload pictures of the orange plastic debris scattered on top of my transmission. (these pictures include a temporary repair that was made after the failure occurred- shifter was reattached with zip ties in order to reposition the vehicle for service). Ford has since developed updated repair parts (a yellow plastic bushing and a plastic cover to keep contaminants off the bushing) for later recalls that affected different trims/configurations of the Fusion. I had to go to the dealer and buy those updated parts myself, which I installed so that I could continue driving my car to work (I will include pictures of repairs). I feel like Ford should be upgrading the earlier recalled vehicles with these latest and greatest parts. My car was fixed under recall and then experienced the very failure the recall was supposed to address. My car has about 245,000 miles on it. The initial recall repair was performed at about 88,000 miles. After about 7 years and 156,000 miles the recall repair failed.

Ford Motor Company (Ford) is recalling certain 2015-2018 Focus and 2013-2016 Fusion vehicles previously repaired incorrectly under recall numbers 18V169 and 18V845. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

Consequence & remedy

Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Remedy: Dealers will update the software, free of charge. Additionally, dealers will inspect Focus vehicles and replace the clutch assembly as necessary, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 5, 2026. Additional letters will be sent once the remedy is available. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 26S43. Ford Motor Company (Ford) is recalling certain 2015-2019 Flex, 2015 Explorer, Lincoln MKT, Lincoln MKZ, 2015-2016 C-Max, Escape, Taurus, 2016 Fusion, 2018-2019 Taurus, 2019 Lincoln MKT, Fiesta, and 2020 Mustang vehicles. The rearview camera may display a distorted, intermittent, or blank image when the vehicle is in reverse.

Consequence & remedy

Consequence: A rearview camera that fails to properly display an image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will inspect and replace the rearview camera as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters notifying owners of the safety risk were mailed on October 25, 2025. Additional letters will be mailed once the final remedy is available on each phase. A component inside the door latches may break, which can make it difficult to latch or possibly allow the doors to open while driving.

Consequence & remedy

Consequence: Vehicle doors opening unexpectedly while driving can increase the risk of a crash or injury.

A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Ford Motor Company (Ford) is recalling certain 2013-2016 Ford Fusion and Lincoln MKZ, 2015-2016 Ford Edge, and 2016 Lincoln MKX vehicles originally sold, or currently registered in Connecticut, Delaware, the District of Columbia, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, and Wisconsin. Road salt or other contaminants may corrode the electric power steering gear motor attachment bolts.

Consequence & remedy

Consequence: If the bolts corrode, the steering gear motor may detach from the gear housing resulting in a loss of power steering assist, requiring a greater steering effort, especially at lower speeds, and increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the steering gear motor bolts and apply a wax sealer. If any of the steering gear motor attachment bolts are broken or missing, a new steering gear will be installed in the vehicle. These repairs will be performed free of charge. Ford Motor Company (Ford) is recalling certain 2013-2016 Fusion vehicles equipped with 2.5 liter engines. The bushing that attaches the transmission shifter cable to the transmission may degrade over time and cause the bushing to detach from the transmission. The condition could allow the driver to move the shift lever to Park and remove the ignition key, while the transmission may not be in Park, with no warning message or audible chime.

Consequence & remedy

Consequence: If the vehicle is exited without the transmission being in Park and without the parking brake being applied, the vehicle may unexpectedly move, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the shifter cable bushing, free of charge. The recall began September 10, 2019. Owners may contact Ford customer service at 1-866-436-7332. In the event of a crash, heat from the deployment of a seat belt pretensioner may cause a front seat belt anchor cable to fail.

Consequence & remedy

Consequence: If the driver or front passenger seatbelt cables cannot properly restrain the front seat occupants, they would be at an increased risk of injury.

Ford Motor Company (Ford) is recalling certain model year 2016 Ford Fusion vehicles manufactured September 3, 2015, to September 13, 2015, and 2016 Lincoln MKZ vehicles manufactured September 3, 2015, to September 12, 2015. The affected vehicles have a fuel tank that may have insufficent wall thickness. As a result, the fuel tanks may crack in the event of a crash and thus the vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 301, "Fuel System Integrity."

Consequence & remedy

Consequence: A fuel tank that cracks may leak fuel, increasing the risk of a fire.

On August 8, 2022, the Office of Defects Investigation (ODI) opened this investigation based on 50 Vehicle Owner Questionnaires (VOQs) alleging front brake jounce hose ruptures on model year (MY) 2013-2018 Ford Fusion and Lincoln MKZ vehicles. A ruptured front brake hose results in a brake fluid leak. As a result, the driver may experience an increase in pedal travel and a reduction in the rate of deceleration, increasing the risk of a crash. It should be noted that the subject vehicles identified fell outside the scope of NHTSA Safety Recall 20V-469, which addressed front brake hoses rupturing on certain MY 2015-2018 Ford Edge and 2016-2018 Lincoln MKX vehicles. The result causes a brake fluid leak, lengthening the distance needing to stop the vehicle, increasing the risk of a crash.On March 10, 2023, Ford Motor Company (Ford) submitted to NHTSA Safety Recall 23V-162 (Ford Recall 23S12) to address front brake jounce hoses that may rupture. Ford found the rupture occurs from a localized failure of the internal reinforcement braid due to cyclic fatigue through tensile/bending and torsional inputs during suspension and steering articulations. The supplier for the front brake jounce hoses is Hitachi. This recall affects the subject vehicles equipped with non-hybrid engines and the suspect front brake jounce hoses (a total of 1,280,726 vehicles). The recall covers subject vehicles built from February 3, 2012 to July 19, 2017 when the suspect hoses were used in vehicle production. The remedy is for dealerships to replace both front brake jounce hoses with new hoses that have a revised braid material at no cost. The vehicles equipped with hybrid engines are excluded from the recall because the internal hose pressure is much lower due to the regenerative braking provided by the hybrid brake system. No failures of brake hoses on hybrid vehicles have been reported to ODI or Ford.Ford's actions address ODIs concerns at this time. Hybrid vehicles will be monitored for related failures. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210.
