Subject: Request for Review of Recall Remedy Effectiveness – Ford Recall 24S78 / NHTSA Recall 24V-957 Vehicle: 2020 Ford F-250 Super Duty 6.7L Power Stroke VIN: [XXX] To Whom It May Concern: I am submitting this letter to document a significant safety event involving my 2020 Ford F-250 Super Duty and to request that NHTSA consider whether the current recall remedy associated with Ford Recall 24S78 / NHTSA Recall 24V-957 adequately addresses vehicles that are already exhibiting symptoms of CP4 fuel pump deterioration. I am not submitting this complaint to seek financial compensation. My purpose is to provide information that may assist NHTSA in evaluating whether the existing remedy sufficiently protects motorists from the safety risks associated with CP4 pump failure. Background My vehicle's VIN has been confirmed as included in the affected population covered by the CP4 fuel pump recall. Prior to the ultimate failure, the vehicle exhibited symptoms that, in hindsight, may have been indicators of fuel-system deterioration. In February 2026, I documented concerns regarding abnormal starting behavior and specifically questioned whether either vehicle battery was failing. No battery issue was identified. Believing the issue might be battery related, I replaced both batteries at my own expense. The starting concern continued. On [XXX], while covered under a Ford Extended Service Plan, Fuel Injector #4 failed and was replaced under warranty. During the same service visit, the recall software update was performed. Despite the recall remedy being completed, the vehicle subsequently suffered a catastrophic CP4 pump failure in August 2026. Safety Event The failure occurred while traveling between Utopia, Texas and Tarpley, Texas on a rural roadway with sharp curves, steep terrain, significant drop-offs, and little or no roadway shoulder. It’s not fair that we should have to pay for a tsb bc we don’t have warranty left. My truck has 84,000 miles but is out of the 5 years warranty period. Symptoms I am having with my transmission are: Harsh engaging to drive and reverse Flair shifts bad sometimes almost like it goes into neutral between changing gears Has shifted from 4th to 1st instead of 4th to 5th or 6th, then it started shifting normal right after. Had my factory 3250rpm limiter not been moved to 4500rpm the truck would have locked the rear wheels up and possible caused driveline damage or other damaged. Also had a car been behind me when this happened I would have been in a wreck because it slammed to a low speed and the car would have rear ended me. Slips in 4th and 7th gear the most but sometimes in other gears Bumps when it shifts a lot of the gears Sometimes shudders and shakes and will almost make like a pop noise Will clunk on downshift from 5-3 Sometimes clunks when upshifting 3-5 I have had the truck looked at by an independent diesel mechanic I use and he said there is a problem somewhere in the transmission, it’s either planetary gear or cdf bushing has moved or the drum itself has cracked. The component itself has not been inspected because the only was to inspect this is to tear the transmission down. The pan has been dropped and had a lot of metal material on the magnet. No codes on the dash but my mechanic said I had dtcs stored for the transmission. I have created a case claim with Ford trying to see if they will extend the recall for the known issue but I have had zero luck with them. I have videos of the issues but can’t upload them on here.

The contact owns a 2020 Ford F-250. The contact received notification for NHTSA Campaign Number: 22V256000 (Power Train); however, the recall repair could not be completed. The vehicle was taken to a dealer where it was diagnosed that a thrust washer was missing from the output planetary assembly and had disintegrated, contaminating the transmission case, transfer case, torque converter, cooling lines, and radiator. The missing washer caused the planetary gear to shift and damage the transmission case. The manufacturer was notified of the failure, and a complaint was filed. The failure mileage was 80,256.

2020 Ford F250 with 85,000 miles. We purchased new and all maintenance has been done on time and by Ford Service. Transmission began jerking when going from park to drive (or reverse). Noticeable lunging of the vehicle when putting into gear. This began in December 2025. This is a safety concern if anyone is in front of or behind the vehicle when this happens. The truck jumps. Now the transmission is not shifting gears smoothly when the truck is in motion. This is a known problem with trucks manufactured during this time period with 10R80 transmissions. Ford has been less than helpful in helping me come to a resolution. They tell me there was a TSB sent out. They attempted to reprogram software (at a cost of $424 to me). I will upload the invoice from Ford. This only made the problem worse. They quoted me a "special price" of $8,500 to replace the transmission, however their service department is SIX WEEKS out. For this reason we have chosen to have the transmission replaced at a local transmission shop. Their price is $9,932.41. I have attached that invoice. Ford needs to take accountability for this safety problem and attempt to make these vehicles safe. They should absorb at least some of the cost to fix this design problem. We paid full price to purchase a new, safe vehicle. That is not what we received. Rather than a "Technical Service Bulletin", Ford should be forced to do a recall and make this right for customers who paid top dollar for these vehicles.

The transmission would hang up in 7th gear when stopping at red light, caution light or stop signs without warning. It would roll like normal when letting of brakes however when out in the middle of traffic stop signs or caution lights the truck would not move and would not shift as it was hung in 7th gear. The only way to get the truck back in 1st gear was to place the truck in park and turn the ignition off then restart the vehicle. This has happened 3 different times post bringing the truck to silsbee ford for a ecm and tcm reprogram. Originally called dealer back to reports extreme rough shifting and was told they did not mess with anything in the transmission and it was not due to anything they did, never received a call back from manager. No warning lights messages ever appeared to show the transmission had failed to down shift into first gear at the stop lights. All three of these events happened while my 7 year old child was in the truck and two of the times it was a near miss and narrowly avoided accidents. Prior to this since around 50,000 miles truck would shift rough with rough engagement into 3,4th,5th,6th, gears with frequent flare shifts and was told by dealers this was normal characteristic of the transmission.

Biodiesel deposits may form on the pump drivetrain roller components, which could lead to failure of the high pressure fuel pump.
Consequence & remedy
Consequence: High pressure fuel pump failure can cause a loss of drive power, increasing the risk of a crash.
Remedy: Dealers will update the powertrain control module (PCM) software, free of charge. Owner notification letters were mailed mailed April 7, 2026. Consequence & remedy
Consequence: A rearview camera image that does not display properly can reduce the driver’s view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the image processing module software, free of charge.This will be a phased campaign, with the remedy becoming available in different phases based on model years. Interim letters notifying owners of the safety risk were mailed on October 30, 2025. Additional letters will be mailed once the remedy is available on each phase. Consequence: A rearview camera that displays a blank image can reduce the driver's view behind the vehicle. A rearview image that continues to be displayed after the vehicle is shifted out of reverse may distract the driver. Both of these situations can increase the risk of a crash.

The owner's manual does not include instructions for adjusting or removing certain head restraints. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 202, "Head Restraints."
Consequence & remedy
Consequence: Without instructions, the head restraints may be improperly adjusted, increasing the risk of injury in the event of a crash.
Remedy: Ford will mail the Owner's Manual addendums that describe how to adjust and remove the head restraints for the center seating positions, free of charge. Consequence: A fractured driveshaft can cause a loss of drive power, or a loss of vehicle control if the driveshaft contacts the ground. Additionally, unintended movement could occur while parked if the parking brake is not engaged. Any of these scenarios can increase the risk of a crash.

Consequence & remedy
Consequence: Over cured tires may develop a break in the sidewall, resulting in sudden air loss or belt edge separation which could lead to tread/belt loss. Either condition can cause a loss of vehicle control, increasing the risk of a crash.

The transmission torque converter damper plate was not tempered, resulting in a brittle damper plate that may fracture and cause the engine to stall or require increased brake pedal effort in order to stop the vehicle.
Consequence: An engine stall or requiring additional braking effort to stop the vehicle can increase the risk of a crash.
Remedy: Ford will notify owners, and dealers will replace the transmission assembly, free of charge. This recall began July 6, 2020. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 20S33.

The Office of Defect Investigations (ODI) has received 27 complaints alleging diesel fuel leaks due to a fracture in the secondary fuel filter housing in model year (MY) 2015-2021 Ford Motor Company (Ford) F-250, 350, 450, and 550 Super Duty vehicles equipped with 6.7L diesel engines. Of these complaints, 12 describe an engine compartment fire. The proximity of the secondary fuel filter to high heat sources creates a potential for autoignition thermal events and fires. ODI has contacted the vehicle manufacturer, Ford, and the filter manufacturer, Allevard Sogefi USA (Sogefi), and learned that the low-pressure fuel system is capable of a maximum operating pressure of 8.5 Bar, while the secondary fuel filter is only tested to seven (7) Bar during the manufacturing process. Additionally, changes starting in 2022 by both Ford and Sogefi align with a lack of similar complaints on MY 2022 and newer vehicles. ODI is opening this Preliminary Evaluation (PE) to evaluate the severity of the potential problem and to fully assess the potential safety-related problems. On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs. In numerous meetings after receiving their IR response, Ford highlighted the drop in claims since the recall and production update and communicated how they were unable to verify the owners’ complaints or replicate the concern. Despite this, Ford began developing ways to prevent uncommanded tailgate openings from various human, mechanical or electrical causes. Based on testing, Ford issued a Customer Satisfaction program, 21M05-S1/S2/S3, to further address the potential of unintended tailgate openings. This work was applied to vehicles being produced as of December 20, 2021. The program began for already delivered vehicles on July 22, 2022.
