The contact owned a 2017 Ford F150. The contact received notification of NHTSA Campaign Number: 26V237000 (POWER TRAIN). However, the part to do the recall repair was not yet available. The contact stated that while driving at 40 MPH on a country road, the vehicle independently veered to the right, went down a ditch, and hit a tree. Both driver-side and front passenger-side air bags were deployed. The contact sustained minor injuries, a bruise on the left arm below the elbow, a small bump on the forehead, and lower back pain. A police report was filed. The contact's wife was called and stopped by the scene of the accident and picked up the contact. The vehicle was towed to a towing yard. The insurance company was notified and declared the vehicle a total loss. The manufacturer was not made aware of the failure. The failure mileage was approximately 70,000.

Vehicle is a 2017 Ford F-150 equipped with the 5.0L engine and 6R80 automatic transmission. The transmission has developed repeated harsh and abnormal shifting behavior, primarily during upshifts between 3rd, 4th, 5th, and 6th gears. During these shifts, the vehicle lurches/kicks forward abruptly and feels as though it is driving over a bump or being struck from behind. Condition is more pronounced when transmission is cold and worsens significantly under load, including while towing or driving uphill. Issue has persisted despite transmission fluid service/triple flush, adaptive relearn procedure, and replacement of spark plugs/ignition coils to rule out engine-related causes. Independent Transmission Shop has inspected vehicle, confirmed abnormal transmission operation, and advised that internal transmission wear/failure is likely. Transmission fluid was reported to be excessively dark/dirty for mileage. No warning lights or transmission fault codes are currently present. The harsh and unpredictable shift behavior creates a safety concern because the vehicle can surge/lurch unexpectedly during acceleration, particularly when merging into traffic, climbing grades, or towing. Sudden driveline shock during gear changes may affect vehicle stability and driver control, especially in wet/low-traction conditions or while towing. Transmission remains installed in vehicle and is available for inspection upon request. Vehicle has been inspected by an independent transmission repair facility; manufacturer has not inspected vehicle at this time. At 87,336 miles, my 2017 Ford F-150 (3.5L EcoBoost) would not start. The engine clicked and buzzed but failed to turn over, leaving me stranded with no warning. There was no prior check engine light. I brought the truck to Roy Hendrick Automotive in [XXX] (Invoice XXX, completed 03/05/2025). Their diagnosis determined the cam phasers and timing components had failed. The shop performed a full timing chain and cam phaser replacement, including both camshaft sprockets, timing chains, tensioners, timing chain guides, variable timing solenoids, water pump, seals, gaskets, and spark plugs. Total cost: $5,449.36, entirely out of pocket. This is a serious safety defect. Cam phaser failure in the 3.5L EcoBoost can cause catastrophic loss of engine timing, resulting in sudden engine stall or failure to start. A stall at highway speed, in an intersection, or while merging creates an immediate risk of collision, serious injury, or death. Even a no-start condition is a safety issue when it strands a driver in an unsafe location. I use this vehicle daily to transport my two young children (ages [XXX] and [XXX] ) to and from daycare. A sudden engine failure while driving with them in the vehicle could have had fatal consequences. Ford's 3.5L EcoBoost cam phaser failures are extensively documented, with numerous TSBs and owner complaints on file. An engine should not require over $5,000 in internal timing repairs before 100,000 miles. Ford has failed to issue a recall or provide an adequate remedy. I am filing this complaint to ensure NHTSA has a complete record of another 3.5L EcoBoost timing failure that put vehicle occupants, including small children, at risk. At 90,181 miles, the 10R80 transmission in my 2017 Ford F-150 suffered a catastrophic internal failure. The vehicle exhibited violent, hard shifts between gears 2 through 4 with no check engine light or warning indicator, giving zero warning the drivetrain was failing. Sheehy Ford in [XXX] reproduced the condition. Their diagnosis on RO XXX: "HARSH 4-6 SHIFT-INTERNAL FAULT." Teardown revealed a fractured CDF drum with metal debris that migrated into the valve body. The dealer removed the transmission and performed a full overhaul, replacing the CDF drum, clutch cylinder, forward clutches, and valve body, and flushed the cooler and converter. Total cost: $6,846.30, entirely out of pocket. This is a serious safety defect. A fractured CDF drum can cause sudden loss of gear engagement, unexpected downshifts, or transmission lockup at highway speed, any of which can result in loss of vehicle control and severe risk of injury or death. I use this vehicle daily to transport my two young children (ages [XXX] and [XXX] ) to and from daycare. This defect endangered their lives, my life, and other motorists, all with no diagnostic warning. This failure is consistent with widespread defects in Ford's 10R80 transmission, currently the subject of class-action litigation. A transmission should not need a $7,000 overhaul before 100,000 miles. Ford is aware of this pattern and has failed to recall. I am filing to ensure NHTSA has a complete record of another 10R80 failure that directly endangered occupants, including small children. Following this incident, the transmission entered a 'Limp Mode' and remained stuck in 2nd gear, refusing to upshift to any higher gears. This made it impossible to maintain normal traffic speeds, creating a significant safety hazard on public roads. The vehicle is now effectively inoperable. A Ford dealer and Ford Corporate (Case on file) have confirmed this is related to Recall 26V237, but they stated that a software remedy will not be available until July 2026. This delay for an actively failed vehicle is unreasonable and poses a continuous safety risk.

The vehicle gets stuck in gear or "limp mode" and will then suddenly fix itself and switch into the correct gear intermittently. There is often a delay when the vehicle goes into limp mode and the service engine wrench light coming on, so you don't know it's happened until you lose the ability to accelerate. I was almost t-boned when this issue first appeared as I did not realize the vehicle was stuck in 4th gear. I was at a flashing yellow light waiting to turn left across traffic. When I saw a gap in the oncoming traffic I proceeded to make my turn only to realize the vehicle had very little acceleration. Because it was stuck in 4th gear it had virtually no acceleration from a stop. Thankfully the oncoming vehicles were paying attention and noticed my vehicle crawling through the turn otherwise I would have been hit. It also can get stuck in lower gears and not shift up, so when accelerating to get onto the highway it will remain stuck in 1st or 2nd gear and as a result drive very poorly at highway speeds. I brought this to a Ford dealership and they had the truck for 3 weeks and told me they couldn't reproduce it and bring it back to them when it was more consistent since it is a very intermittent issue. Even though I showed them my Ford app with over 100+ notifications that stated the issue and they were able to see the vehicle codes they told me there was nothing they could do. I took it to another dealer and their recommendations were: Replacement of Trans Internal Wiring Harness (Molded Leadframe) & Pan Gasket & Filter, Transmission Flush The many notifications in the Ford app that I presented to the dealerships stated: "Powertrain Malfunction / Reduced Power - The transmission control module has detected that one of the transmission sensors is not sending its information to the module. Bring to authorized dealer as soon as possible" This issue has manifested itself many times for me and I have been going back and forth with Ford about it for months.

Ford Motor Company (Ford) is recalling certain 2017 F-150 vehicles previously repaired incorrectly under recall number 17V053. The instrument cluster may lose power after initial vehicle start-up in cold ambient temperatures. Several gauges, including the transmission gear position indicator, may not illuminate. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 102, "Transmission Shift Lever Sequence, Starter Interlock, and Transmission Braking Effect."

Consequence & remedy

Consequence: An instrument cluster that fails to display critical safety information, such as gear position or warning lights, increases the risk of a crash or injury.

Remedy: Dealers will update the Instrument Panel Cluster Assembly (IPC) software, free of charge. Owner notification letters were mailed July 17, 2026. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 26C26. Ford Motor Company (Ford) is recalling certain 2015-2017 F-150 vehicles. A loss of signal between the transmission range sensor and the powertrain control module can cause the transmission to unexpectedly downshift into second gear.

Consequence & remedy

Consequence: Unexpectedly downshifting into second gear may result in a loss of vehicle control, increasing the risk of a crash.

Remedy: Dealers will update the powertrain control module software, free of charge. Owner notification letters were mailed July 24, 2026. Owners may contact Ford customer service at 1-866-436-7332. On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift.  ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. ODI is opening this Engineering Analysis (EA) to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified. On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift.  ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. Based on NHTSA’s analysis of consumer data, information provided by Ford, and preliminary testing conducted by NHTSA’s VRTC, this Preliminary Evaluation is being upgraded to an Engineering Analysis (EA26001). During this Engineering Analysis, the agency plans to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified from this Preliminary Evaluation. PE18008 · Seatbelt Pretensioner Fire

On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge.
