Vehicle drove fine to work and upon getting in to return home and staring the vehicle, the brake pedal was instantly spongy and low brake fluid alert displayed on dash. There was a difference in 9 hours from when the brakes were fully functional to having brakes were completely out and nonfunctional. Had vehicle towed to local dealership and their diagnosis was "master cylinder is bleeding into the booster" and booster "full of brake fluid." This exact issue has been known and was supposed to be addressed but there have been numerous issues with the new parts installed after the recall leading to subsequent master cylinder/booster failures. Since it has already been "repaired" under the initial recall, the subsequent repair cost $1,300 including diagnostic.

The contact owns a 2014 Ford F-150. The contact stated that while driving 60 MPH and depressing the brake pedal, the brake pedal went to the floorboard before the vehicle came to a complete stop. The vehicle was not taken to the dealer; however, the dealer offered a diagnosis over the phone and believed that the brake master cylinder was leaking brake fluid into the brake booster. The vehicle was not repaired. The contact related the failure to NHTSA Campaign Number: 20V332000 (SERVICE BRAKES, HYDRAULIC); however, the VIN was not included in the recall. The manufacturer was notified of the failure and opened a case. The failure mileage was 144,000.

The vehicle was taken to an independent mechanic, where it was diagnosed that the electronic power steering module had failed and needed to be replaced. The vehicle was not repaired. The dealer was contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and referred the contact to the NHTSA Hotline for assistance. While leaving the VA parking lot, and after breaking 3 times on the way out, as I approached a major intersection, the break pedal dropped to the floor and a low break fluid warning appeared on the display. My slow approach allowed me to stop. Checked the reservoir and it was empty. I walked to the gas station and purchased break fluid, when I returned, I checked under the vehicle for a leak, to no avail. I walked back to the spot I was parked but there was no indication of a leak. I was able to get my car home, and filled the break container intending on bleeding the lines; however, Florida weather forced to wait a day. Today, Saturday, I proceeded to bleed the lines but the reservoir was empty. Again, no signs of leakage around tires, break lines, reservoir, master cylinder. Everything I have searched regarding this indicates a large population of 2024-18 F150 with a master cylinder failure.

The contact owns a 2014 Ford F-150. The contact stated while driving at an undisclosed speed, the transmission unexpectedly downshifted without warning. The contact called a dealer and was informed that the vehicle was covered under NHTSA Campaign Number: 24V444000 (Power Train). The dealer sent a technician to the contact's workplace for the recall repair, and the technician performed the powertrain control module software update; however, the failure recurred approximately a week later, and the failure persisted. The contact stated that the instrument panel occasionally went blank during the failure, requiring the contact to restart the vehicle for the vehicle to operate as intended. An unknown dealer was notified of the failure and the contact was provided an estimate for a diagnostic test. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 160,000.

Vehicle Information: 2014 Ford F-150 VIN [XXX] Transmission: 6R80 Description of the problem: My 2014 Ford F-150 has a dangerous transmission issue. It suddenly downshifts from higher gears (like 4th or 5th) into 1st gear while driving, without warning. This creates a serious risk of loss of control or rear-end collision. Ford performed the recall for the 6R80 PCM reprogramming, but the issue persists. The problem was not resolved, and the vehicle still downshifts abruptly. My vehicle has over 200,000 miles, so the extended warranty for the lead frame (program 19N01) no longer applies, but this is a serious safety defect that continues after the recall. I’m requesting that the NHTSA investigate this issue, as it seems the recall fix is not effective in all cases. Many other owners report similar symptoms even after the recall. Safety concern: I feel unsafe driving this vehicle. The transmission can cause a sudden deceleration in traffic, and I fear it may lead to an accident. After receiving a call from Mullinax Ford in Mobile, AL regarding a recall for our 2014 Ford F-150 Raptor (recall related to unexpected downshifting), I brought the vehicle to my workplace on [XXX], where a technician performed the software update (flash) on-site. Before the update, the vehicle was functioning without issue. That same evening while driving home, I experienced extremely hard downshifting, initially in lower gears, and worsening over time. The truck now violently downshifts, primarily into 4th gear, and sounds as if the transmission is slipping. I drove the truck again the next morning and the symptoms persisted. I contacted Zachary, the original representative at the dealership, who said it may be part of the “relearning” process. My husband drove it a week later after returning from a business trip and he confirmed the issue. It has progressively worsened to the point of being undrivable. We returned the truck to Mullinax Ford for evaluation on 7/29/25, and they claimed the failure was unrelated to the update and that the transmission needed to be replaced, at our expense. The truck unpredictably drops gears at high speeds, creating a sudden jerk that could lead to loss of control. This poses a serious safety risk to both the driver and others on the road. The dealership performed the recall software update and later diagnosed the truck with transmission failure, but is denying responsibility. No other third-party inspections have been done at this time. There were no warning lights or issues prior to the update. The issue started immediately after the recall software was flashed and worsened over the next 24–48 hours. The vehicle is currently at the dealership and available for inspection. A loss of signal between the transmission output shaft speed sensor and the powertrain control module (PCM) can cause the transmission to unexpectedly downshift into first gear.

Consequence & remedy

Consequence: Unexpectedly downshifting into first gear may result in a loss of vehicle control, increasing the risk of a crash.

Remedy: Dealers will update the PCM software, free of charge. Owner notification letters were mailed July 8, 2026. Owners may contact customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain model year 2013-2014 F-150 vehicles manufactured August 1, 2013, to August 31, 2014 equipped with 3.5L engines. The master cylinder rear cup seal may roll, resulting in brake fluid leakage from the primary reservoir into the brake booster.

Consequence & remedy

Consequence: Loss of brake fluid from the primary reservoir results in a complete loss of front brake function, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain model year 2014 Ford F-150 and Flex vehicles manufactured March 1, 2014, to May 31, 2014. The affected vehicles were manufactured with a reduced clearance between the seat frame and the seat track assembly. This can cause the Occupant Classification System (OCS) to inaccurately register passenger weight and improperly suppress the passenger front airbag when the passenger seat is occupied by an adult passenger. Thus, these vehicles fail to comply with Federal Motor Vehicle Safety Standard (FMVSS) No. 208, "Occupant Crash Protection."

Consequence & remedy

Consequence: Improper suppression of the passenger airbag increases the risk of an injury to the seat occupant in the event of a crash necessitating deployment of the front passenger airbag.

Ford Motor Company (Ford) is recalling certain model year 2014 Ford F-150 trucks manufactured May 26, 2014 to June 19, 2014, and equipped with Electronic Power Assist Steering (EPAS). The affected vehicles may have an incorrectly installed EPAS gear motor position sensor magnet that can lead to a total loss of steering control while driving.

Consequence & remedy

Consequence: A loss of steering control while driving increases the risk of a vehicle crash.

Remedy: Ford will notify owners, and dealers will replace the EPAS steering gear, free of charge. The recall began in July 2014. RQ24005 · Unintended Transmission Downshift

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
