MY HUSBAND AND I BOUGHT A NEW 2008 DODGE RAM 2500 DIESEL A YEAR AGO. WITHIN THE FIRST 500 MILES THE CHECK ENGINE LIGHT CAME ON. WE CALLED THE DEALER AND WERE TOLD THIS WAS NORMAL AND NOT TO WORRY, JUST DRIVE THE TRUCK, STOP, TURN IT OFF, RESTART AND DRIVE IT, UNTIL THE LIGHT GOES OFF. THE LIGHT TURNED OFF THE NEXT TIME WE STARTED THE TRUCK, SO WE FIGURED IT WAS OK, MAYBE WE WERE OVERREACTING. WELL A MONTH LATER IT HAPPENS AGAIN, WE CALLED THE DEALERSHIP AND WERE TOLD THE SAME THING, DON'T WORRY, JUST START IT, DRIVE, TURN IT OFF, AND TURN IT BACK ON. WE TOOK THE TRUCK IN THE THIRD TIME THIS HAPPENED, THEY TOLD US IT WAS NO BIG DEAL, THAT THIS WAS "A NORMAL PROBLEM" AND THE EMISSIONS SENSOR JUST NEEDED A CLEANING, WHICH THEY DID. WE WERE TOLD WHEN WE PICKED IT UP THAT EVERYTHING WAS FINE, BUT TWO WEEKS LATER IT HAPPENED AGAIN, WE TOOK IT BACK TO THE SHOP AND THEY CLEANED THE SENSOR AGAIN. RIGHT AFTER WE GOT THE TRUCK BACK, WE HAD ABSOLUTELY NO POWER, IT STRUGGLED TO GET UP A SMALL HILL, WE TOOK IT BACK TO THE SHOP, AND THEY HELD IT FOR FOUR DAYS. WHEN WE GOT IT BACK THEY TOLD US THERE WAS ABSOLUTELY NOTHING WRONG WITH IT, BUT THE NEXT TIME THIS HAPPENED BRING IT RIGHT BACK AND THEY WOULD REPLACE THE TURBO AND THE EMISSIONS SENSOR AGAIN. WE HAVE ONLY OWNED THIS TRUCK FOR A YEAR, AND I HAVE CALLED DODGE WITH MY COMPLAINT BUT WAS TOLD TO EMAIL THE COMPLAIN, AND WHEN I EMAILED IT, I WAS TOLD TO CALL, WHICH TO ME SEEMS LIKE A RUN AROUND. WE WERE TOLD BY THE MECHANIC AT THE DEALER THIS WAS A ONGOING ISSUE BUT THERE WAS NOTHING HE COULD DO ABOUT IT, EXCEPT FOR CONTINUING TO CLEAN THE SENSORS OR REPLACING THE TURBO, WHICH WILL STILL NOT STOP THE PROBLEM. *TR

2008 DODGE RAM 2500 HEAVY DUTY 4X4,DIESEL ENGINE,QUAD-CAB,SHORT BED. THE WINDSHIELD WIPERS FAILED TO WORK . WHEN I FIRST NOTICED THE PROBLEM A COUPLE OF WEEKS AGO I WAS LEAVING A CAR WASH AND JUST HAPPENED TO TURN THE WIPERS TO THE"HIGH" POSITION ON THE SWITCH AND THEY STOPPED WORKING RIGHT WHERE THEY WERE ON THE WINDSHIELD. I CHANGED THE SWITCH BACK TO THE LOW SPEED AND THEY STARTED WORKING AGAIN. I MADE AN APPOINTMENT WITH THE DEALERSHIP TO HAVE IT FIXED,WHILE I WAS WAITING FOR THE DAY OF MY APPOINTMENT THE WINDSHIELD WIPERS STILL WOULD NOT FUNCTION AT ALL ON THE HIGH SETTING, I LOOKED IN THE OWNERS MANUAL TO SEE IF IT WAS A FUSED ITEM THAT MAYBE I JUST NEEDED TO REPLACE A FUSE AND MONITOR FURTHER INCASE IT WAS BLOWING THE FUSE FOR A REASON , BUT THERE WAS NO LISTING FOR THE WIPERS IN THE FUSE SECTION OF THE OWNERS MANUAL, I THEN NOTICED THE WIPERS WERE NOT WORKING IN ALL THE POSITIONS ON THE SWITCH INTERMITTENTLY SOME TIMES THEY WORKED SOME TIMES THEY DIDN'T, AND IT DIDN'T HAVE A PATTERN OF WHEN THEY WOULD WORK OR NOT ,JUST IF THEY CAME ,ON THEY CAME ON ,IF NOT THEY WOULDN'T ,NO MATTER HOW MANY TIMES YOU TURNED THE SWITCH OFF AND ON OR THE TRUCK OFF AND ON, ONCE OR TWICE THEY SEEMED TO BE MOVING SLOWER THAN NORMAL IN THE LOW SETTING THEN SPED UP TO THE RIGHT SPEED. MY PROBLEM IS WITH SAFETY OF IT .HERE WE ARE GETTING BAD WEATHER,ICE ,SNOW RAIN ,SALT SPRAY FROM OTHER DRIVERS ETC. AND NOT KNOWING WHEN OR IF THEY ARE GOING TO FAIL IS A SAFETY ISSUE ,YOU CAN DRIVE ALL DAY USE THEM TEN TIMES,THEN SOME CAR PASS SPRAY ROAD GRIME ALL OVER YOUR WINDSHIELD AND HAVE THEM NOT WORK AND LOOSE TOTAL VISIBILITY. MAY EVEN CRASH. I TOOK MY TRUCK IN ON ITS APPOINTMENT ONLY TO FIND THREE OTHERS THERE FOR THE SAME REASON... PARTS ARE ON BACK ORDER AND NO TIME LINE ON WHEN THEY WILL ARRIVE AND HOW LONG IT TAKES TO FIX ONCE IT IS DROPPED OFF , THEY SAID I COULD LEAVE THE TRUCK OR TAKE IT. NO LOANER VEHICLE OR NOTHING . EA21002 · Desiccated Air Bag Inflator Rupture

Opened Sep 17, 2021 · No close date supplied

Status: open (inferred from source dates) · Air Bags:frontal:driver Side:inflator Module; Air Bags:frontal:passenger Side:inflator Module

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

Air Bags:frontal:passenger Side:inflator Module

RMD opened this audit query (AQ) investigation in order to better understand and evaluate Chrysler's process(es) for timely and efficient execution of its safety recall campaigns, including its oversight and management of its parts division Mopar.Prior to opening this investigation, RMD had raised concerns with the company related to its execution of two safety recalls, 13V-528 and 13V-529, both of which involved safety defects that could cause the involved vehicles, FCA?s heavier Dodge Ram trucks, to lose steering control suddenly and without warning.Specifically, RMD inquired about the availability of parts for recall repairs, by email and phone on January 15, 2014 and February 19, 2014, due to the volume of complaints it was receiving from owners alleging parts scarcity and parts restrictions. FCA steadfastly denied it had placed parts restrictions on its dealers to manage issues of parts scarcity.FCA shared material showing it had uncommitted stock of parts available for its dealers. Nevertheless, consumer complaints of lack of parts continued.RMD made independent inquiries tosome dealers tobetter understand the conflicting claims between owners alleging lack of ability to repair their defective vehicles due to lack of parts, and FCA?s information showing availability of parts.Dealers informed and provided documentation to RMD on February 21, 2014 and February 25, 2014 showing FCA had, in fact, put into place parts restrictions and, further, had in some instances canceled dealerships? back orders for parts. In addition, during the course of its dialogue with dealers, RMD discovered that FCA had notified their dealerships on February 27, 2014 to return the recall remedy parts over quality concerns, and had therefore effectively canceled its safety recall program without notification to NHTSA. RMD issued a request for information (IR) on October 21, 2014.The company failed to respond to a number of the questions RMD asked that required it to produce specifics surrounding the repairs and parts ordering process with the recalls, including a question about whether a dealer order for recall parts could be canceled and under what circumstances.
