Engine oil filter adapter assembly failure of Manufacturer OEM Part Numbers: 68105583AA, 68105583AB, 68105583AC, 68105583AD, 68105583AF, 68105583AE This failure results in many safety concerns to include engine bay fires. The inferior plastic OEM part is prone to failures causing pressurized oil leaks at the top of the engine that contaminate many other parts to include the exhaust manifold, exhaust components inclusive of the catalytic converter which operates at normal temperature exceeding 650 degrees F. Flash fires accrue frequently in this regard and engaging the occupants, others, environment, and may result in fatalities. There are no check engine lights or safety warnings of any kind for this failure unless oil pressure is lost after the fact. The OEM parts is not a suitable material for typical engine temps exceeding temps of 200F and will break down on a molecular basis. There is currently an aluminum upgrade manufactured by Dorman, part number 926-876 that is not supported nor provided by Mopar. I am a retired ASE Master Technician with over 40 years of experience. I have seen over 30 instances of this failure effecting a wide rage of 2014 Chrysler vehicles with the 3.2 and 3.6 liter gas engines. Vehicle mileage is not a factor in this regard. Inferior simply put, OEM Product, plus time, plus normal engine temps will cause the molecular material breakdown and thus component failure. It is aggravated by oil change procedures when even skilled technicians try to change the filter by removing the cover or over torque the cover and fracture the now brittle inferior materials. Internally we have also noticed severe chemical softening of the housing sealing surfaces that remain in constant contact with engine oil. This further proves the inferior product material incompatibilities. Please validate ASAP to begin a Recall campaign that will surely prevent future fatalities or catastrophic damage to personal property or the environment.

1. My engine light came on. I plugged in my code reader and it stated that the temperature sensor failed. 2. My engine failing causes risk for my safety on the highways to and from work. 3. Both dealer and independent service center. I brought it into the local mechanic, since the dealership had no appointments for 2 weeks. Since my van has 113,096 miles on it, we had them check the temp sensor, replace the spark plugs and boots, serpentine belt, air filter and battery. Engine light went off for a few hours. Car was idling rough. Came back on again, this time stating cylinder 5 was misfiring, made an appointment with the dealership for 2 weeks out, in the meantime, I had the mechanic replace the fuel injector. Engine light came back on again, car running extremely rough. Brought the van to the dealership, they replaced the rollers and lifters and installed new valve cover gaskets. When I asked the dealership if this was a common problem, he admitted it was with this engine. I know recalls have been made on 2011-2013 Caravan engines. I would add 2014 to that as well. 4. I was not inspected by any of the listed agents above. 5. No, they began the end of Nov. No prior problems.

Due to Thanksgiving, took van in today on 11/29/21. There are no codes coming up. They asked if they were cleared by another. No one has seen this van other than an oil change. They were unable to help, but verified and documented the ticking. Possible lifter. Stated to see about recalls on this. Nothing. Unfixed.

At approximately 100,220 the engine began making a tiking/tapping sound. This sound is from faulty bearings in the rocker arms. The bearings in the rocker arms in the 3.6L V6 engine are indeed faulty and the 2014 Dodge Grand Caravan should be covered for replacement. 2013 models have a 10 year 150,000 warranty. Why are the 2014 model Grand Caravan not covered as well. If you listen to any 2011 and up 3.6L Dodge engine you can hear this tapping noise. Eventually the rocker arm will cause wear on the camshaft and that will have to also be replaced. Our vehicle is now currently at 104,000 miles and I’m still unable to get Dodge to cover these parts.

I would like to report a problem with my 2014 Dodge Grand Caravan 30th anniversary edition: 3.6L engine .There is a problem with the Oil Filter Housing Cooler that has a terrible leak.According to internet this problem has been around from the very beginning that these parts have been installed in various Chrysler models. There is a bulletin on various web sites about this problem, many people have complained about this part failing on their vehicles. I feel that this is definitely a Safety Problem due to the fact the oil is leaking onto the hot engine and could cause a fire to start.The Manufacturer and Dealers are aware of this problem but don't want to take responsibllity for the engineering defect of this part. I had a mechanic look at it, and was told they have seen this problem on many other vehicles in their shop. Please look into this issue for me and tell me if you can help me in any way get this repaired by the Manufacturer or dealer. I purchased this vehicle new in April 2014, Thank you.

Engine was running fine, wasn't a "stall". All electrical components shut off. While attempting to put car in neutral to push, the vehicle than started. 3. The day prior to this, my wife reported that while driving on the highway the dashboard lights completely shut off, they then started flashing on and off while driving. 4. This same issue happened on 10/30/21 The car has not yet been taken in for service for this problem. After the incident listed in #4 the battery warning light remained on.

Was driving onto a narrow 2 lane elevated bridge when, without warning, the engine just stopped. There were no warning lights or sounds before or after the engine stopped. I was stranded on this bridge on busy route U.S 17 and no other drivers could go around me because of construction barriers. Eventually some other drivers that were stuck behind me helped me push vehicle to crest of bridge and I coasted to the bottom of the bridge and got out of the roadway. Vehicle was towed to Dodge dealer because it would not restart. Dealer diagnosed problem as Totally Integrated Power Module (TIPM). Cost to repair was $1382. It became quite apparent to me that Chrysler-Dodge has made a fortune replacing these parts on many of their vehicles, to the level that would indicate that there is some defect in the design or manufacturing of these parts. I have since read hundreds of stories about other drivers left stranded in the roadway over this issue. Certainly seems dangerous enough to warrant a safety recall over these numerous part failures.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. A list of the affected trailer models is available at: https://static.nhtsa.gov/odi/rcl/2017/RMISC-17V824-0103.pdf. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Consequence & remedy

Consequence: If the fire extinguisher does not function properly, it can increase the risk of injury in the event of a fire.

FCA US LLC (Chrysler) is recalling certain model year 2015-2016 Chrysler Town and Country, and 2014-2015 Dodge Grand Caravan vehicles manufactured August 16, 2014, to December 5, 2015. The windshield on the affected vehicles may have been installed using expired urethane primer, allowing the windshield to become displaced in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 212, "Windshield Mounting."

Consequence & remedy

Consequence: If the windshield dislodges during a crash, there is an increased risk of occupant injury.

Remedy: Chrysler will notify owners, and dealers will replace the windshield, free of charge. The recall began March 4, 2016. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S02.

Chrysler Group LLC (Chrysler) is recalling certain model year 2014 Chrysler Town and Country and Dodge Grand Caravan vehicles manufactured March 20, 2014, to April 22, 2014, and 2014 Jeep Wrangler vehicles manufactured March 21, 2014, to April 22, 2014. Due to a software error, the Tire Pressure Monitoring System (TPMS) may fail to learn the locations of the individual sensors while the vehicle is being driven. As a result, the low tire pressure warning light will illuminate, despite the the tire pressures being within specification. Should one of the tires lose air pressure, the driver would not be notified of the change in air pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard No. 138, "Tire Pressure Monitoring Systems."

Consequence & remedy

Consequence: If the TPMS light illuminates because the sensors cannot be located, it could mask an actual low tire pressure condition, possibly resulting in tire failure, increasing the risk of a crash.

Remedy: Chrysler has notified owners, and dealers will reprogram the TPMS module, free of charge. The recall began on October 23, 2014. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is P63.

Chrysler Group, LLC (Chrysler) is recalling certain model year 2010-2014 Chrysler Town and Country and Dodge Grand Caravan vehicles manufactured August 25, 2010, through October 31, 2013. The affected vehicles may experience overheating of the vent window switch in the driver's door armrest.

Consequence & remedy

Consequence: An overheated switch may result in a vehicle fire.

Remedy: Chrysler will notify owners, and dealers will replace the vent window switch with a newer version, free of charge. The owner notification letter was issued and the remedy campaign launched on on December 31, 2014. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is P25.

The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. In a letter dated August 21, 2014, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged failures associated with the Totally Integrated Power Module (TIPM) installed in Chrysler SUV?s, trucks, and vans beginning in the 2007 model year. The petitioner alleges that TIPM defects may result in engine stall, airbag non-deployment, failure of fuel pump shutoff resulting in unintended acceleration, fire, and other symptoms. On September 25, 2014, the Office of Defects Investigation (ODI) opened DP14-004 to evaluate the petition for a grant or deny decision. ODI analyzed complaint data provided by CAS as well as complaints submitted to ODI from consumers. In total, there were 296 complaints submitted by the petitioner in the original petition and five supplements, including 271 complaints related to the subject vehicles equipped with TIPM-7. Approximately 3 percent of CAS complaints are related to vehicles equipped with TIPM-6 and ODI?s review of these complaints did not identify any safety defect trends. ODI conducted a detailed review of complaints narratives submitted by CAS and consumers including careful analysis of vehicle repair histories, warranty claims obtained from the manufacturer and any available Customer Assistance Inquiry reports (CAIR). Analysis of the field data indicated that MY2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles exhibited significantly higher complaint rates related to fuel pump relay (FPR) failures than other subject vehicles. In a September 3, 2014 letter to NHTSA, Chrysler submitted a Defect Information Report (DIR) identifying a defect in the FPR within the TIPM-7 which can result in a no start or stall condition in approximately 188,723 model year (MY) 2011 Jeep Grand Cherokee (WK) and Dodge Durango (WD) vehicles manufactured from January 5, 2010 through July 20, 2011 (14V-530).In a February 24, 2015 letter, Chrysler submitted a second DIR expanding the scope of the FPR defect condition to include an additional 338,216 MY 2012 through 2013 Jeep Grand Cherokee vehicles manufactured from September 17, 2010 through August 19, 2013 and MY 2012 through 2013 Dodge Durango vehicles manufactured from January 18, 2011 through August 19, 2013 (15V-115).ODI analysis of the CAS allegations of TIPM defects resulting in stall while driving, airbag non-deployment, unintended acceleration, fire and other faults identified a single defect condition related to 1 of over 60 different circuits in the TIPM assembly. The most common effect of this defect condition, related to the fuel pump relay, was a no-start concern, but it could also result in stall while driving (76 complaints). No valid evidence was presented in support of claims related to airbag non-deployment, unintended acceleration or fire resulting from TIPM faults and these claims were found to be wholly without merit based on review of the field data and design of the relevant systems and components.Except insofar as the petitioner's contentions relate to the defect condition addressed by the Chrysler recalls, the factual bases of the petitioner's contentions that any further investigation is necessary are unsupported. In our view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists or a NHTSA order for the notification and remedy of a safety-related defect as alleged by the petitioner at the conclusion of the requested investigation.
