Issue 1: Infotainment display failed or malfunctioned. Would not power on, turned off during travel. Pulled over to power the vehicle off and back on to try to remedy the situation but that did not work. Continued driving and after a few minutes the infotainment center powered back on. We were not put at risk during this experience, but it was inconvenient and not happened again. Issue 2: Remote started vehicle so it could warm up. After a few minutes I got into the vehicle and began to pull out of the driveway when the vehicle just powered down. I'd assumed it has something to do with remote starting the vehicle.... Until I saw Manufacturer Recall A03 NHTSA Recall 23V-010. Our family now feels very unsafe driving this vehicle knowing that this particular recall may have occurred while we were pulling out of our driveway. This incident occured approximately one week ago and has not happened since, so I did not take it in to the dealership to look into, but perhaps I should. This vehicle has not been inspected by the manufacturer, police, insurance representatives or others. There were no warning lamps, messages or other symptoms of the problem prior to the failure. Incident 3: Blind spot notification light on driver's sideview mirror has failed once resulting in swerving to avoid an accident.

Hello so we bough a car on 12/14/2023 and within 24 hours all these issues shows up with the car they didn’t give us to option to give it back and get something else they wanted to try and fix it but I was also looking at the paper work and they messed the paper work up for the odometer they said 60630 miles on it when I got it at almost 62400 miles and they reported it wrong I’m not sure of next steps to take and on a service report for an oil change it states the miles on 12/8/2022 was 62247

Me and my spouse - The engine failed in the middle of road [ freeway ] making me to pull on to the right lane showcasing the indicator "Engine failure " and Charger system malfunction. - Recovery : Stop the car , turn off all the engine and open the door and close, then turn on the engine - it works. but this is on freeway and i am worried of my family driving the car. - Once i took the vehicle to service center and they analyzed and said - nothing they could see which was wrong as the indicator goes away after some time surprisingly - Happened 4 times so far on free way , first time cop had to push the car out and later i realized the steps for recovery - There are warning lamps on the panel - " engine failure "

While traveling on the highway (>70mph), my Chrysler Pacifica Hybrid (~44k miles) suddenly lost power and the "service charging system" error appeared along with the instructions to "shift to park". The check engine light began to flash as well. Fortunately, I was able to cross three lanes of traffic in a van with an unresponsive accelerator to get to the shoulder. Once safely on the shoulder, I still could not shift to park and I could not turn the car off either. After sitting for 25 minutes or so, I was able to shift to park and turn the car off and then on again. I immediately took the car to the dealership (using surface roads). They kept the van for 10 days and diagnosed a software issue. I got the van back and for about 3 days it was fine. Then we were driving the same freeway (at speeds of 70mph or more) and the exact breakdown happened again. Chrysler has had the van for 4 weeks now and just informed me that the issues is metal shavings in the transmission; they will be replacing the transmission. This is related to the ODI investigation # PE 22-008

Second occurrence of 2021 Pacifica Hybrid stalling while driving. Same error messages with check engine light, Service Charging System, battery light. The recommended software update was done a month ago at a dealership after the first incident. The van worked fine until today when I had to unexpectedly maneuver to the roadside again as it stalled out at 35 mph. I waited several minutes and the van started again but the yellow engine light stayed on. I took it to the dealer where I bought it and it's there now. Yes, my safety was at risk again.

Vehicle stalled on the interstate multiple times, dashboard shows service charging system. Had to coast to the side of the road each time. Sometimes the vehicle would restart immediately, other times it would need to sit before restarting. I was able to recreate the stalling issue by idling the van in my driveway by opening the hood to force the ice to run. The engine would stop and the same fault would show up on the dashboard after about 25 minutes of idling with the air conditioner running. Vehicle has now been at a dealership for 3 months and they haven't been able to fix it.

While driving on the interstate at 65mph, the vehicle suddenly beeped, showed a “service charging system” warning and simultaneously lost propulsion. Pressing the gas pedal did nothing, so I had to coast over to the shoulder. My attempts to restart the vehicle were unsuccessful for a few minutes, but then it did restart and I was able to get off the freeway. It was an extremely dangerous situation. On February 10, 2022 at about 4:30 in the afternoon, my wife and I had just merged onto the 101 at Scottsdale Road in Scottsdale, Arizona. We had not gone far when we heard a loud explosion. It sounded like a ballon popping but at least 10 time louder. It sounded like it came from inside the car and there was no indication (wobble, hard steering, etc.) that it may have been a tire. My wife was driving so I began to see if I could spot any sign that would tell us what it was - nothing. Soon we felt air rushing into the cabin. I thought the sunroof may have popped open for some reason. It is a power operated sunroof so I pressed the control switch to pull back the ceiling cover and open the sunroof. When I did that shattered glass came pouring down on us. After getting it closed I called 911 and was put in contact with the Scottsdale police. They told me to call the State Police but because no one was injured it may take a long time for someone to contact us. We continued on to our house in Mesa. The next morning I contacted the Chrysler dealership in Mesa that I bought the car from. I was told to bring it in and it would be covered under warranty. I've had the car about 10 months and it has about 7,700 miles on it. Turns out Chrysler won't cover it under warranty. I have had it repaired at a cost of $842.45. I'm not sure at this time how much of the cost I will have to pay. An article by Consumer Reports identifies this as a widespread safety issue that the car manufactures want to ignore and that our government is doing little about. I can't imaging what our reaction would have been if the ceiling cover had been pulled back. Basically the explosion and the glass scattering all over us would have happened at the same time. I can easily relate to someone losing control of the car while traveling in heavy traffic at 65 MPH. Let's hope no one dies before this is resolved. And the cost of repairs must be paid by the car manufactures not the car owners.

Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.

Consequence & remedy

Consequence: A vehicle fire increases the risk of injury.

Remedy: Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. All repairs will be performed free of charge. Owner notification letters were mailed beginning June 23, 2026. Owners may contact FCA US LLC customer service at 1-800-853-1403. FCA US LLC's number for this recall is 52D. The Vehicle Identification Numbers (VINs) involved in this recall will become searchable on NHTSA.gov on June 11, 2026.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Chrysler (FCA US, LLC) is recalling certain 2020-2023 Pacifica and 2020-2021, 2023 Voyager vehicles. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."

Consequence & remedy

Consequence: An air bag that does not deploy during a crash increases the risk of injury.

Remedy: Dealers will secure the connector, free of charge. All vehicles have been remedied. Owner notification letters were mailed October 22, 2024. Owners may contact FCA customer service at 1-800-853-1403. FCA's number for this recall is A8B.

The radio software may prevent the rearview image from displaying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera image that does not display decreases the driver's visibility, increasing the risk of a crash.

The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."

Consequence & remedy

Consequence: An unsecured seat increases the risk of injury in a crash.

RQ24004 · Loss Of Motive Power Due To An Internal Wiring Connector Short.

On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV).  On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown.  An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.  In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008.  The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode.  The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010.  These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010.  The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down.  When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced.  Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. PE22008 · Transmission Fault Leading To Loss Of Motive Power

On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances.
