On July 26 when starting the car the entire dash lit up and all possible dash lights began to flash, the RPMs would go up and down, speedometer would move up and down. It literally looked possessed. Turned the car off. Later that day went to start the car again (to bring it in if possible) and the car started fine. Started driving towards the dealership. At 30mph the car began to accelerate on its own. Thankfully no cars in front as I slammed the breaks and pulled over. The dash lights again went haywire. Pulled over and shut car off. Turned back on and drove the mile home (13 miles to dealership). Next day we again brought it into the dealership and the car made it. They were unable to duplicate and stated there were 100's of codes and they don't know where to begin. The dealership sent me home. Almost a month later exactly the vehicle went crazy. This time I grabbed my cell phone and recorded it. Turned the car off and had it towed to the dealership. It did not do it again. They have had it now going on over two weeks and are unable to duplicate the issue. Again, they stated there are 100s of codes and don't know where to start. The car is not safe for driving. Once the dash goes crazy all control is lost. We have an extended warranty. We love the features of the car, but with five kids we do not feel safe driving it on the highway until they look into this. I have read at least two others with similar issues, but the dealership says they only work with Chrysler Engineering and don't look at other websites. VERY FRUSTRATED.

We were about to turn left onto a busy highway. Pulled into traffic and vehicle stalled with 2 adults and 4 children in the car. While stuck in the middle of the high way with oncoming traffic from both sides we started honking the horn and waving traffic to stop. Husband noticed a blinking “D” light on the shifter knob and an alert on the dash that read something like “shift to park”. Due to the incline of the road we rolled backwards off the highway. We luckily were not hit by oncoming traffic but completely terrified by the situation. Had that highway been as busy as it usually is we would be dead. Shifted back to park. Was then able to drive vehicle the half mile home. Brought vehicle into dealership where after 2 weeks they were unable to to recreate issue and found nothing wrong. Stalling with no prior warning is a known recall for this vehicle but the recall was supposedly fixed on January 11th of 2018. This did not fix the stalling issue obviously. We refuse to drive vehicle again and take the chance of killing our family. After many conversations with Chrysler care we are 3 weeks out from incident and they have been of no help. This vehicle is not safe to be driven. After reading many forums there are many complaints of this recall not fixing the issue. Chrysler needs to be held accountable. I refuse to be a test dummy for this vehicle.

I have to drive with alert the whole time because I will not know when the steering wheel will stop working and if it stop working while taking corner or I have to turn the wheel very fast I wont be able to do so.which might cause an accident to me or hit other people. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? we took it to one place but they wont be able to identify it because it happens randomly. and we drop it at the dealership to check it but it has been 2 days since I dropped off but they did not do the inspection yet. 4.Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No 5.Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The power steering wheel warning light will come up randomly while driving slow, fast or start and stay at the same place but it will not show up right away when you first start. It first appear around a month ago.

While traveling on a dry highway, with good tires/new tires, and decelerating from 45 mph and approaching a red light, and a moderate, but firm pressure on the brake to bring the van to a full stop, the speed decreased to 15 mph, with the tachometer stable at 800 rpm and then to 10 mph, with both hands on the wheel, left foot on the dead pedal, and right foot on the middle of the brake pedal. As the speedometer made the transition at 10 MPH, the tachometer surged to 1500 RPM and then to 3000 in under 3 seconds, which cause me to initiate an emergency action drill of stabbing the brake pedal hard three times and rotating the transmission control to the lowest gear. The transmission refused to shift lower, and the control of the vehicle turned on oa wrestling match and the tachometer surged from 1500 RPM, them 3000 RPM, then 4500 RPM, and then 6000 RPM, each time the vehicle acting like it was repeatedly being rear-ended at a speed exceeding 15 MPH. The engine could not be disengaged and aceelaretd uncontrollably to over 40 MPH, through a busy intersection, and required over 24 hard break stages over a 10 second period to actually stop the car, and the engine refused to disengage until stopped. Once the car was restarted it immediately surged forward trying to fight through the hard applied brakes. The cruise control was never on the entire time. I then stut down the engine once I was able to wretle it to a stop with the engine climbing to over 4500 RPM. I then took this vehicle to a safe area, and repeated the conditions, and again as the vehilce speed dropped below 15 MPH, and certainly below 10 MPH, the egine tachometer increased in steps of 1500 MPH, 3000, 4500, 6000 RPM, and no control could be achieved other then violently "stabbing" the brake pedal. Under controlled conditions the behavior was repeatable over 20 times. This is a life threatenign defect, and it appears to be software in origin. I have CAN data streams from the events (23) and video.

The contact owns a 2017 Chrysler Pacifica. The contact stated that the vehicle failed to start after several attempts with the STOP/START unavailable message displayed on the instrument panel. The contact replaced the battery however, the failure returned soon after each battery replacement. The contact also stated that the vehicle independently accelerated while driving at various speeds. The contact stated that the vehicle also stalled while depressing the accelerator pedal. The service shifter and service electronic stability system warning lights were illuminated on the instrument panel. Additionally, the contact stated that the vehicle would not accelerate above 25 MPH while driving. The contact was forced to use the hazard lights and slowly drove the vehicle back to her residence. The contact called the local dealer and was informed that the vehicle was previously repaired under NHTSA Campaign Number: 19V348000 (Electrical System) and 18V395000 (Electrical System, Equipment). The manufacturer was also notified of the failure. The vehicle was not repaired. The failure mileage was approximately 50,000.

I bought my 2017 Chrysler Pacifica in 2018, and it would randomly jump gears while accelerating or stay in a higher gear. The dealership could not duplicate the problem for months. In May 2019, it threw a check engine light code of P0732. This was then repaired at a Chrysler dealer. It started doing that same thing again in January 2021. I have twin boys, I let them out of their car seats after returning from daycare. I took bags in my house and returned to find my door off the upper track and hanging from the lower attachment causing much stress to the bracket and and scratching my exterior paint. From the door opening and closing so many time off the track. This occurred after my son said he hit that button on the door a few times. This door could of easily fallen off or hurt one of my sons. Very scary! Also I found numerous other Pacifica forums that say the side windows have broken on there vehicles after this same type of event has happened. I am very concerned, this is my daily driver for all 3 of my sons who are all under 4yrs old. This heavy door could of caused serious damage to one of my sons. Please help me resolve this problem. Also now I have scratches on my paint from the door going back a forth which is visible from the outside.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Consequence & remedy

Consequence: An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.

Remedy: Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. Chrysler (FCA US LLC) is recalling certain 2017-2019 Pacifica vehicles equipped with engine stop/start systems. A loose battery ground connection may result in an intermittent loss of power steering assist and/or a stall.

Consequence & remedy

Consequence: A loss of power steering assist or a stall can increase the risk of crash.

Remedy: Chrysler will notify owners, and dealers will inspect the battery ground surfaces, clean the contact surfaces as needed, and properly reassemble the ground connection, free of charge. These vehicles are being recalled to address a defect that could prevent the cruise control system from disengaging. Chrysler (FCA US LLC) is recalling certain 2014-2018 Dodge Journey, Charger and Durango, RAM 2500, 3500, 3500 Cab Chassis (more than 10,000lb), 4500 Cab Chassis and 5500 Cab Chassis, Jeep Cherokee and Grand Cherokee and Chrysler 300, 2014-2019 RAM 1500, 2015-2018 Dodge Challenger, 2015-2017 Chrysler 200, 2016-2018 RAM 3500 Cab Chassis (less than 10,000 lb), 2017-2018 Chrysler Pacifica and 2018 Jeep Wrangler vehicles. These vehicles are being recalled to address a defect that could prevent the cruise control system from disengaging. If, when using cruise control, there is a short circuit within the vehicle's wiring, the driver may not be able to shut off the cruise control either by depressing the brake pedal or manually turning the system off once it has been engaged, resulting in either the vehicle maintaining its current speed or possibly accelerating.

Consequence: If the vehicle maintains its speed or accelerates despite attempts to deactivate the cruise control, there would be an increased risk of a crash.

Remedy: Chrysler will notify owners, and dealers will inspect the software, and perform a software flash on the engine or powertrain control module, free of charge. The recall began June 4, 2018. Owners may contact Chrysler customer service at 1-800-853-1403. Note: Owners are advised to stop using cruise control until the software update has been performed. In the event that cruise control cannot be disengaged while driving, owners should firmly and steadily apply the brakes and shift the transmission to neutral, placing the vehicle in park once it has stopped.

Chrysler (FCA US LLC) is recalling certain 2017-2018 Chrysler Pacifica vehicles equipped with the 8-passenger seating option. In certain passenger seating and vehicle situations, the second-row center seating position seat belt buckle could cause the left outboard seat belt to become unlatched.

Consequence & remedy

Consequence: Inadvertent unlatching of the left outboard seat belt can increase the risk of injury in the event of a crash.

Vantage Mobility International, LLC (VMI) is recalling certain 2017 Chrysler Pacifica lowered floor vehicles modified by VMI. The rear pinion bracket on both sides of the vehicle may crack, causing the axle not to be centered and the wheel to wobble.

Consequence & remedy

Consequence: A wheel wobble may cause a possible loss of control, increasing the risk of a crash.

The affected vans have rear brake lines that might have been crossed during the modification. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 135, "Light Vehicle Brake Systems."

Consequence & remedy

Consequence: The crossed rear brake lines will cause the rear anti-lock brake system to be ineffective, increasing the risk of a crash.

The Office of Defects Investigation (ODI) is opening this Preliminary Evaluation to assess the scope, frequency, and potential safety consequences of alleged defects related to the electric power steering (EPS) system in model year (MY) 2017–2018 Chrysler Pacifica vehicles. ODI has received 94 Vehicle Owner Questionnaires (VOQs) reporting a condition in which the steering system exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. Complainants allege that during these events, increased steering effort is required to return the wheel to center. In many cases, this is followed by a sudden reactivation of EPS assistance, which may result in an oversteer condition. This behavior could increase the risk of a crash, especially at higher speeds. ODI has identified multiple VOQs describing similar symptoms. This investigation will assess the frequency, root cause, and potential safety-related consequences of the alleged EPS malfunction. On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high voltage (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery fires, and to increase monitoring of the manufacturer root cause investigation.
