The contact stated that the rear driver's side window failed to open or close while depressing the buttons on the driver's door and on the passenger's door to control the window. While depressing the brake pedal to attempt to slow down, and while in reverse, the brakes were squeaking. The vehicle was not taken to the dealer for the window failure. The dealer diagnosed that the brake pads had failed and needed to be replaced. The vehicle was not diagnosed or repaired. The manufacturer was informed of the issue. The failure mileage was approximately 43,632.

The contact owns a 2024 Chevrolet Traverse. The contact stated that while driving out of the dealer's parking lot at 4-5 MPH and depressing the accelerator pedal, the messages "Service ESC System" and "Brake System Failure - Press Hard to Brake - Do Not Drive" were displayed, and the check engine warning light illuminated. The contact stated that the vehicle failed to respond while depressing the brake pedal, and the vehicle rolled to a stop. The vehicle was taken back to the dealer, where the failure was associated with the braking system. The dealer disconnected and reconnected the battery, and the dealer cleared the codes. The dealer informed the contact that the vehicle was repaired and returned the vehicle to the contact; however, the contact stated that the dealer had failed to determine the cause of the failure and to perform an adequate repair of the vehicle. Additionally, the contact stated that the brakes had been making an abnormal squeaking sound while the brake pedal was depressed. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure, and a case was filed; however, the contact had not yet been contacted. The failure mileage was 16,876.

The contact stated that while his wife was driving 60 MPH, the brake pedal was depressed and went to the floorboard, and the vehicle failed to respond. The driver engaged the parking brake; however, the vehicle failed to respond. The driver swerved to avoid the vehicle in front, drove over an incline, jerked the steering wheel, and the vehicle eventually stopped. The message "Braking Module System Failure" was displayed. The contact stated that the incident was frightening for his wife and his two children, who were also inside the vehicle. The vehicle was towed to the dealer, where it was acknowledged that the braking system module and the parking brake had failed; however, no official diagnostic information was provided. The vehicle was repaired under warranty, and the contact was advised that the vehicle was drivable; however, the contact advised the dealer that there were safety concerns because the cause of the failure was not yet determined, and there was no guarantee that the failure would not recur. The vehicle was driven back to the residence, although the contact remained concerned. The manufacturer was notified of the failure, and a case was filed, but no further assistance was provided. The failure mileage was approximately 28,000.

The contact applied more pressure to the brake pedal for the brakes to properly engage. The contact was able to drive the vehicle home, while needing to apply excessive pressure to the brake pedal and allow for additional stopping distance while depressing the brake pedal. The dealer was contacted, and an appointment was scheduled, but the vehicle was not taken to the dealer to be diagnosed or repaired. The manufacturer was not informed of the failure. The vehicle was towed to the dealer however, the vehicle was not diagnosed. The contact stated that the vehicle had been at the dealer for thirty days. The contact was informed that a diagnostic test provided no information of a failure. The manufacturer was not made aware of the failure. The failure mileage was approximately 9,010. The contact stated while driving 25-30 MPH at the airport, the failure recurred. The contact stated that upon turning off and restarting the vehicle several times, the failure persisted. The messages "Brake Assist Failure" and "Forward Collision Not Available" were displayed, and the check engine warning light was illuminated. The contact was able to drive 5 MPH and pulled over to a safe location. The vehicle was towed to the residence. The vehicle was then towed to the dealer, where it was diagnosed that the brake master cylinder had failed and needed to be replaced. The dealer was awaiting the parts to repair the vehicle. The vehicle was not repaired. The manufacturer was notified of the failure and a case was opened. The failure mileage was approximately 1,900.

EA26006 · Electronic Brake Module Component Failure

On April 11, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24011 after receiving complaints concerning a loss of brake assist in 2023 MY Cadillac Lyriq vehicles. These complaints allege a hard brake pedal, followed by a “Brake System Failure” message at start up or while driving. GM shared records regarding several internal investigations for loss of brake assist due to alleged spindle fractures in the eBoost system. GM has stated that in the event of a spindle fracture, anti-lock brakes (ABS), stability control, and traction control remain functional until the vehicle comes to a complete stop. At this point, ABS, brake assist, stability control, and traction control functions are lost. The loss of these functions causes the vehicle to set one of three Diagnostic Trouble Codes (DTCs) based on failed performance criteria for the Electronic Brake Control Module (EBCM). These DTCs, in turn, activate several driver alerts: an audible chime, a visual “Service Brake System” message in the display, and multiple illuminated Malfunction Indicator Lights (MILs) for the ABS, Traction Control, and Brakes informing the driver of loss of functionality. The vehicle speed will also be limited to 70 KPH (43 MPH). Since opening PE24011, ODI has continued to receive reports of loss of brake assist. Some of these reports describe symptoms inconsistent with GM’s description of a spindle failure. These reports allege an immediate loss of brake assist functionality as opposed to the loss occurring after the vehicle comes to a complete stop. The sudden loss of brake assist during a braking event could result in extended braking distance, which increases the risk of a crash or injury. ODI believes further analysis is necessary to assess the potential for failures in GM’s eBoost system to resulting in safety-related outcomes. At this time, ODI is aware of the following vehicles that are equipped with this eBoost system: 2023-2026 MY Cadillac Lyriq, Chevrolet Colorado, and GMC Canyon; 2024-2026 MY Buick Enclave, Envision; Chevrolet Blazer EV, Equinox EV, Traverse; Cruise Origin; and GMC Acadia; 2025-2026 MY Cadillac Celestiq and Optiq. ODI is also aware of 2024-2026 MY Acura ZDX and 2024-2026 MY Honda Prologue vehicles that were built with this system. The Acura and Honda vehicles were manufactured by GM in a joint venture with Honda, and will be included in the Engineering Analysis as peer vehicles. ODI is opening this Engineering Analysis to further assess the safety-related consequences of a failed eBoost system. In addition, ODI is expanding the scope of the investigation to encompass all GM vehicles that share this eBoost system.
