The contact owns a 2016 Chevrolet Suburban. The contact stated while driving at approximately 70 MPH, the vehicle brakes independently activated. The contact stated there were no warning lights illuminated. The contact pulled off the highway and stopped the vehicle. The vehicle was turned off and restarted and regained braking functionality. The failure was intermittent but occurred at various speeds. The failure occurred mostly while driving up an incline and depressing the accelerator pedal and the independent braking failure would occur. The contact had not taken the vehicle to a dealer or independent mechanic to be diagnosed. The vehicle was not repaired. The contact stated that prior to the failures the vehicle had been taken to a dealer to be repaired under NHTSA Campaign Number: 19V645000 (Service Brakes, Hydraulic). The manufacturer was not informed of the failure. The failure mileage was approximately 175,000.

The contact owns a 2016 Chevrolet Suburban. The contact stated while reversing at approximately 2 MPH and attempting to depress the brake pedal, the brake pedal was very difficult to depress. The contact stated that extra effort was needed to depress the brake pedal before the vehicle stopped. The contact stated no warning light was illuminated. The contact stated that he shifted into park(P) and turned off and restarted the vehicle and normal brake functionality was restored. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The contact owns a 2016 Chevrolet Suburban. The vehicle had previously been serviced under NHTSA Campaign Numbers: 19V761000 (Service Brakes, Hydraulic, Service Brakes, Electronic Stability Control) and 19V645000 (Service Brakes, Hydraulic) at the dealer; however, the failure recurred after the recalls were repaired with a stiff brake pedal and increased braking effort while driving. The contact stated that while his wife was driving at an undisclosed speed, the vehicle went over a curve and the front passenger's side crashed into a tree, then came to a complete stop in a ditch. There were no air bags deployed. The contact's wife sustained neck and back injuries but did not receive medical attention. The vehicle was towed to Coughlin Hyundai (2300 Hebron Rd, Heath, OH 43056) where it remained and was not repaired. The vehicle was not deemed totaled by the insurance company. The manufacturer was made aware of the failure and a case was opened. Prior to the failure, the contact was receiving notifications from OnStar that NHTSA Campaign Number: 19V761000 (Service Brakes, Hydraulic, Service Brakes, Electronic Stability Control) was unrepaired. The contact owns a 2016 Chevrolet Suburban. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 19V645000 (Service Brakes, Hydraulic) in 2019. The contact stated that while driving approximately 10-25 MPH, the brake pedal was depressed but failed to respond. There were no warning lights illuminated. The contact continued to depress the brake pedal, which responded just before the vehicle crashed into the rear of another vehicle. The contact veered to the side of the road and parked the vehicle. The vehicle was towed to the dealer where it was diagnosed that the vacuum pump and belt needed to be replaced. The vehicle was repaired under an unknown warranty. The manufacturer was notified of the failure and informed the contact that they could not assist because the VIN was not included in a recall. The failure mileage was approximately 72,779.

If a wheel-speed sensor fails, while the vehicle is traveling between 41 and 60 mph in four-wheel or automatic mode, the Electronic-Brake Control Module (EBCM) software may activate the driveline-protection system.

Consequence & remedy

Consequence: Unintended activation of the driveline-protection system will cause unintended braking on the wheel on the opposite side of the failed sensor, causing the vehicle to pull to one side unexpectedly, increasing the risk of a crash.

General Motors LLC (GM) is recalling certain 2015-2017 Cadillac Escalade, 2014-2018 Chevrolet Silverado, GMC Sierra, 2015-2018 Chevrolet Suburban, Tahoe, GMC Yukon vehicles. The amount of vacuum created by the vacuum pump may decrease over time.

Consequence & remedy

Consequence: As the vacuum level drops, the brake assist decreases, increasing braking effort, extending the distance required to stop the vehicle, thereby increasing the risk of a crash.

Remedy: GM will notify owners, and dealers will reprogram the Electronic Brake Control Module, free of charge. The recall began October 8, 2019. General Motors LLC (GM) is recalling certain 2016-2017 Chevrolet Suburban HD vehicles. These vehicles were assembled with a convex right-hand outside mirror, instead of with a non-convex (flat) right-hand outside mirror. As such, these vehicles fail to comply with the requirements of Federal Motor Safety Standard (FMVSS) number 111, "Rearview Mirrors."

Consequence & remedy

Consequence: If equipped with the incorrect right-side rear-view mirror, the driver may misjudge the distance of other vehicles, increasing the risk of a crash.

Remedy: GM will notify owners, and dealers will replace the convex mirror with a non-convex mirror and provide a new page for the owner's manual with the customer notification letter, free of charge. General Motors LLC (GM) is recalling certain model year 2016-2017 Chevrolet Silverado 1500 and 2016 Cadillac Escalade, Cadillac Escalade ESV, Chevrolet Suburban, Chevrolet Tahoe, GMC Sierra, GMC Yukon and GMC Yukon XL vehicles. The front upper control arms may have inadequate welds near the control arm bushing.

Consequence & remedy

Consequence: Inadequate welds may allow the control arm to separate from the bushing, compromising steering and increasing the risk of a crash.

Remedy: GM will notify owners, and dealers will replace the left and right front upper control arms and realign the vehicle, free of charge. The recall began on May 18, 2016. Until the repairs have been made, owners are advised not to drive their vehicles. On November 20, 2018, the Office of Defects Investigations (ODI) opened PE18-012 to investigate instances of sudden onset of a hard brake pedal that could result in extended stopping distance due to an issue with the brake assist vacuum pump in model year (MY) 2014 through 2016 GMC Sierra 1500 and Chevrolet Silverado 1500 series pickup trucks and MY 2015 through 2016 Cadillac Escalade, Chevrolet Suburban, Chevrolet Tahoe, and GMC Yukon SUVs.If the brake assist vacuum pump fails to operate as intended, the amount of brake power assist supplied to the driver can be significantly reduced, extending the subject vehicle stopping distance. GM stated that the root cause of pump failures was due to foreign debris such as oil sludge in the engine oil. GM stated that this debris can become trapped by the oil inlet filter screen. Under normal operation, this screen allows a small amount of oil into the brake vacuum pump. The foreign debris can, over time, clog the pump which can cause a loss of lubrication and a reduction in the amount of vacuum produced leading to a hard pedal and extended stopping distance.On December 13, 2018, GM issued a Special Coverage (N182202780) for the brake vacuum pump issue in MY 2014-2017 trucks and 2015-2017 SUVs on the K2XX platform for 6 years or 72,000 miles. Special Coverage repair includes the replacement of the vacuum pump and vacuum pump belt. Consumers received notification of the coverage via mail. On September 6, 2019, GM notified NHTSA that it was conducting safety recall 19V-645 which reprograms the electronic brake control module (EBCM) and included approximately 3.5 million trucks and SUVs. In October 2019, GM extended the terms of special coverage N182202780 to 10 years or 150,000 miles as well as now including the entire scope of recall 19V-645. In October 2020, GM expanded the scope of coverage N182202780 to include the MY 2018 Escalade and 2019 Tahoe.The remedy implemented in GM recall 19V-645 appears to be sufficient in addressing the safety-related concerns of this issue. Loss of vacuum assist now occurs at a progressive rate as well as there being a more timely warning of a brake problem to the driver through the driver information center (DIC). In view of the recall action and revision to the special coverage, this Preliminary Evaluation is closed. The agency reserves the right to take additional action if warranted by new circumstances.
