Camshaft bearing failure. Catastrophic engine failure while accelerating at left turn. Required full engine replacement. Incident occured at 59,982 miles on odometer. Even though vehical was under manufacture warranty it took approx. 18 days for GM to assume responsability. I was required to produce all maintenance records and was left without a vehicle for this time. Company was not clear on what work and parts of this repair would be protected under further warranty. Company did not provide exact specifics on replacement engine year and place of origin.

I purchased this vehicle brand new is 2021 with the 6.2 liter engine. I have had all the regular maintenance performed as scheduled with 90% being done at a Chevrolet dealership. My truck has around 90,000 miles on it and now the engine is out. It started with showing low oil at 50% oil life left. When I took it to have it checked, it was 3 quarts low on oil. I did another oil change and was told I had to start an oil consumption test in order for them to review the issue. I had it checked every 1000 miles. The first and second check was fine. The third check was one quart low. They took it in and found that the spark plugs were oil fowled. Upon further investigation, they found that the cylinder walls were scored and had oil bypass in the cylinders. I have done everything right with this truck. Took care of it in all the ways I was supposed to. There is no reason what so ever for the engine to have gone out at 90,000 miles. There has to be more to this. I have looked it up and other people are having the same issue with the same engine. Why is nothing being done about this?

There was no warning light illuminated. The vehicle was taken to the dealer, where the engine lifters were replaced; however, the failure persisted. The vehicle was taken back to the dealer, where the contact was informed that the engine abnormal knocking sound was not sufficiently loud to issue a recall. The vehicle was not repaired. The contact related the failure to NHTSA Action Number: PE25001 (Engine, Engine and Engine Cooling). The manufacturer was not notified of the failure. The failure mileage was approximately 11,000.

What component or system failed or malfunctioned, and is it available for inspection upon request? The component that failed is the lifter in my 2021 GM truck, which caused severe engine damage. The vehicle is currently at the dealership and is available for inspection upon request. How was your safety or the safety of others put at risk? My safety and the safety of my child were significantly put at risk when the engine failure occurred on the freeway while traveling at 65 miles per hour. Multiple warning lights illuminated on the dashboard, and the truck became hard to control. I was fortunate to maneuver the vehicle safely out of traffic without harm, but the sudden loss of engine power could have led to a serious accident. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the problem has been confirmed by the dealership, which conducted diagnostics and determined that the lifter malfunction caused severe engine damage, necessitating a full engine replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle has been inspected by the dealership, which is an authorized service center for GM. However, it has not been inspected by the manufacturer directly, police, or insurance representatives. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Diesel engine continues to overheat, even after the oil temperature sensor failure dealership repair. Truck is very well maintained. However, common and trending issue amongst 1500 Silverado diesel owners is overheating which causes engine to loose power while driving and even get so hot it can catch fire. Warning lights appear only when problem is at engine failure point. Also, we lost power or reduced power on freeway which almost caused an accident and engine was smoking. We had it towed to Epic Chevrolet in Sacramento, CA. Within one week of dealership repair covered under powertrain warranty, dealership said overheating engine is due to a “new” code for coolant pump. In short, fire and driving safety hazard that can cost lives if not addressed by NHTSA.

I was driving my 2021 Silverado when i was stopped at a 4 way waiting my turn. When it was my turn i made a left hand turn and and travelled a short distance going 20-25 mph when my engine made an odd sound and shut off in traffic!. I attempted to restart it and it did noting. Truck was towed to Hendricks Chevrolet where the truck was bought and received regular maintenance. When the truck stopped in traffic there were no warning lights on the dash before during or after. Once the truck was at the dealership for inspection i was advised That the motor was sized and that number 1 and 2 rod bearings failed and were ground in the crank journals. I asked how could that happen when the the truck has had regular oil changes by them and is there an issue with these engines? I was told that they see a few 6.2 engines with these types of issues.

The connecting rod and/or crankshaft engine components may have manufacturing defects that can lead to engine damage and engine failure.

Consequence & remedy

Consequence: Engine failure increases the risk of a crash.

Remedy: Dealers will inspect the engine, and as necessary, repair or replace the engine. For vehicles that pass inspection, dealers will add higher viscosity oil, install 6 oil fill cap, replace the oil filter, and update the owner's manual. Repairs will be performed free of charge. General Motors (GM) is recalling certain 2019-2021 GMC Sierra 1500 and Silverado 1500 vehicles equipped with certain accessory road tires and a 17" spare tire. The 17" spare tire provided with the vehicle is not compatible for use with the accessory wheels, which can cause the anti-lock brake system (ABS) to not function properly under certain conditions.

Consequence & remedy

Consequence: An ABS system that does not function properly can increase the risk of a crash.

Remedy: Dealers will provide a new 17" spare tire and wheel assembly that is compatible with the accessory wheels, free of charge. Dealers will also apply a new spare tire information label, and provide owners with an owner's manual insert clarifying which spare tire to use with the accessory road tires and with the 17" original tires. General Motors, LLC. (GM) is recalling certain 2021 Chevrolet Silverado 1500 vehicles. The front driver's side air bag module may be missing one or more metal tabs that secure the plastic air bag cover to the housing.

Consequence & remedy

Consequence: The missing tabs may prevent the air bag from deploying properly, increasing the risk of injury in a crash.

General Motors LLC (GM) is recalling certain 2020 Buick Enclave, Cadillac XT5, GMC Acadia and Chevrolet Traverse vehicles and 2020-2021 Chevrolet Silverado 1500, GMC Sierra 1500 vehicles. The vehicles may have been manufactured with Continental tires that were overcured.

Consequence & remedy

Consequence: Overcured tires may experience a carcass break in the sidewall resulting in a sudden air loss or could develop a belt edge separation which could lead to partial or full tread/belt loss. Either condition increases the risk of a crash.

Remedy: GM will notify owners, and dealers will inspect vehicles and replace tires that have DOT number and production mold number combinations identified by Continental, free of charge. The recall began December 17, 2020. The vehicles included model year (MY) 2020 through 2021 CT4 and CT5 passenger cars, Sierra and Silverado trucks (1500/2500/3500 models), XT4 and MY 2021 Escalade, Suburban, Tahoe, Yukon SUVs.Technical Service Bulletin (TSB) 21-NA-005, issued in March 2021, addresses air bag MIL illumination with diagnostic trouble codes (DTCs) B0001-1B or B0012-0D. It cites rust particles in the connection terminal interface of the driver air bag inflator housing as the cause of the air bag MIL illumination. Illumination of the air bag MIL under these circumstances may result in a non-deployment of the frontal driver air bag during a frontal collision and increased risk of injury to the driver.GM stated a correlation exists between production delays due to the pandemic shutdown and premature corrosion initiation on the surface of the air bag inflator housing, which, in some cases, caused rust development during a seven (7) month period of vehicle production. In the rare instance when a rust particle dislodges from the driver air bag inflator housing and enters the air bag connector prior to assembly, an intermittent resistance spike at the air bag connector may result. The condition generates excessive resistance in the air bag electrical system when the steering wheel horn cover is manipulated or the steering wheel is turned. This excessive resistance is momentary. A crash commanding air bag deployment would need to occur exactly when the momentary excessive resistance occurs. Thus, there is a very low likelihood the hazard will present a risk during a crash of sufficient severity to warrant a commanded deployment. This failure mechanism occurs early in life and results in an air bag MIL illumination that corresponds to a DTC warning the driver that the vehicle requires service. Air Bags:srs Malfunction Warning Lamp/light

On April 9, 2021, the Office of Defects Investigations (ODI) opened PE21-010 to investigate instances of air bag malfunction indicator light (MIL) illumination and potential for frontal air bag non-deployment in several vehicles manufactured by General Motors (GM). The vehicles included model year (MY) 2020 through 2021 CT4 and CT5 passenger cars, Sierra and Silverado trucks (1500/2500/3500 models), XT4 and MY 2021 Escalade, Suburban, Tahoe, Yukon SUVs.Technical Service Bulletin (TSB) 21-NA-005, issued in March 2021, addresses air bag MIL illumination with diagnostic trouble codes (DTCs) B0001-1B or B0012-0D. It cites rust particles in the connection terminal interface of the driver air bag inflator housing as the cause of the air bag MIL illumination. Illumination of the air bag MIL under these circumstances may result in a non-deployment of the frontal driver air bag during a frontal collision and increased risk of injury to the driver.GM stated a correlation exists between production delays due to the pandemic shutdown and premature corrosion initiation on the surface of the air bag inflator housing, which, in some cases, caused rust development during a seven (7) month period of vehicle production. In the rare instance when a rust particle dislodges from the driver air bag inflator housing and enters the air bag connector prior to assembly, an intermittent resistance spike at the air bag connector may result. The condition generates excessive resistance in the air bag electrical system when the steering wheel horn cover is manipulated or the steering wheel is turned. This excessive resistance is momentary. A crash commanding air bag deployment would need to occur exactly when the momentary excessive resistance occurs. Thus, there is a very low likelihood the hazard will present a risk during a crash of sufficient severity to warrant a commanded deployment. This failure mechanism occurs early in life and results in an air bag MIL illumination that corresponds to a DTC warning the driver that the vehicle requires service. The vehicle's air bag remains functional while the MIL is illuminated, and the DTC is present.Once the connection of the driver air bag module to the clockspring is made at the plant, or at the dealership for service, there is no longer an opportunity for rust migration into the connection point. The connector is sealed and has gold terminals, eliminating the expectation of fretting corrosion. All subject vehicles passed standard corrosion validation testing for extensive corrosion with no issue of air bag non-deployment.There is a low rate of occurrence: 1.41 incidents per thousand vehicles (IPTV) for all related occurrences, and 0.04 IPTV for occurrences with a mention of visible rust and/or corrosion on the air bag inflator housing. The average occurrence mileage for the subject vehicles is approximately 2,100. Every occurrence related to the alleged defect experienced an air bag MIL illumination and there are no verified allegations of air bag non-deployment within all data sources.To date, there have been no reported fires, crashes, injuries, or fatalities related to this issue. In view of the low rate, and the nature of early-life failure, the high detectability of failures, and low potential hazard to drivers, this Preliminary Evaluation (PE) is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.
