The front camera lens cover — located in the grille below the Chevy bowtie logo — has developed a spiderwebbed/shattered crack pattern. There is no impact damage to the surrounding grille or bodywork. The vehicle was garaged and not in use when the damage was discovered. The camera appears to still be working and parts of the image are crystal clear but the cracks in the cover distort the picture and make it impossible to use reliably. This is consistent with a material defect, not road damage. I have opened a case with GM Customer Care regarding this defect. My case number is: XXX. Once I put the car into drive, the forward collision warning went off and locked up my car. The brakes would not release and I could not move out of the way of oncoming traffic. I do not know what allowed the brakes to finally release. I changed over to 4x4, put it into park and back to drive. My car finally was then allowed to move.

I have found multiple times that the Instrument Panel (IPC) display and the center Infotainment displays have gone dark while driving. This has put me in unsafe driving situations because I am unable to monitor my speed and other important information displayed on the dashboard, or to control critical safety controls such as headlights, and other ADAS safety controls. Several software updates have been installed by the dealership, but the problem has persisted, suggesting a hardware issue with the radio unit. The manufacture insist it is a software problem and that they are working on a solution. Declining to replace the radio under warranty, additionally, they want me to wait until a new update is released, and in the meantime, my bumper-to-bumper warranty is about to expire. I have found that the radio unit is the main control module for both displays, multiple ADAS controls, and Headlights, and it should be recalled, with the manufacturer extending the warranty or including the radio as a safety feature under the extended powertrain warranty.

I looked at the cruise control and it said 115mph, so I hit the brakes to override it and slow down. when I reset it for , ~70mph again it worked fine. This happened to me two other times with the same basic scenario. The speed is always over 100mph, and it always occurs when the adaptive cruise control has been in use for at least 30 minutes. I have an appt with the dealer that I bought it from to have it looked at. No warning lights and no other symptoms.

On the evening of [XXX], I was making a left hand turn onto [XXX] from [XXX]. I was the second car in line on the left turn green arrow. The first car went to the right lane. I went into the left lane. My car immediately flashed the light in the front window, the alarm went off, and slammed the brakes as if something was in front of me. Nothing was in front of my car in the left lane. Luckily there wasn't a car on my rear bumper otherwise I would have been rear-ended. This is the 2nd time at the same intersection making the same turn that my Blazer has done that. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On April 11, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24011 after receiving complaints concerning a loss of brake assist in 2023 MY Cadillac Lyriq vehicles. These complaints allege a hard brake pedal, followed by a “Brake System Failure” message at start up or while driving. GM shared records regarding several internal investigations for loss of brake assist due to alleged spindle fractures in the eBoost system. GM has stated that in the event of a spindle fracture, anti-lock brakes (ABS), stability control, and traction control remain functional until the vehicle comes to a complete stop. At this point, ABS, brake assist, stability control, and traction control functions are lost. The loss of these functions causes the vehicle to set one of three Diagnostic Trouble Codes (DTCs) based on failed performance criteria for the Electronic Brake Control Module (EBCM). These DTCs, in turn, activate several driver alerts: an audible chime, a visual “Service Brake System” message in the display, and multiple illuminated Malfunction Indicator Lights (MILs) for the ABS, Traction Control, and Brakes informing the driver of loss of functionality. The vehicle speed will also be limited to 70 KPH (43 MPH). Since opening PE24011, ODI has continued to receive reports of loss of brake assist. Some of these reports describe symptoms inconsistent with GM’s description of a spindle failure. These reports allege an immediate loss of brake assist functionality as opposed to the loss occurring after the vehicle comes to a complete stop. The sudden loss of brake assist during a braking event could result in extended braking distance, which increases the risk of a crash or injury. ODI believes further analysis is necessary to assess the potential for failures in GM’s eBoost system to resulting in safety-related outcomes. At this time, ODI is aware of the following vehicles that are equipped with this eBoost system: 2023-2026 MY Cadillac Lyriq, Chevrolet Colorado, and GMC Canyon; 2024-2026 MY Buick Enclave, Envision; Chevrolet Blazer EV, Equinox EV, Traverse; Cruise Origin; and GMC Acadia; 2025-2026 MY Cadillac Celestiq and Optiq. ODI is also aware of 2024-2026 MY Acura ZDX and 2024-2026 MY Honda Prologue vehicles that were built with this system. The Acura and Honda vehicles were manufactured by GM in a joint venture with Honda, and will be included in the Engineering Analysis as peer vehicles. ODI is opening this Engineering Analysis to further assess the safety-related consequences of a failed eBoost system. In addition, ODI is expanding the scope of the investigation to encompass all GM vehicles that share this eBoost system.
